The U.S. Department of Health and Human Services' Health Resources and Services Administration (HRSA) awarded a $1,000,000 Project Grant under the Affordable Care Act (ACA) Grants for New and Expanded Services Under the Health Center Program (CFDA 93.527) to Fort Bend Family Health Center, Inc. (doing business as Access Health), a non-profit Federally Qualified Health Center. The grant, awarded on December 1, 2024, will fund efforts to strengthen transitions in care for justice-involved populations during the community re-entry process and provide essential health care services. Specifically, Access Health will hire a re-entry Community Health Worker with lived experience to connect recently released individuals to primary care, behavioral health, and social services through telehealth and in-person support. The goal is to help manage chronic conditions, address mental health and substance use disorder needs, and meet the health-related social needs of this vulnerable population, ultimately reducing recidivism.
