Key services include permanent housing, transitional housing, homelessness prevention, and integrated mental health and substance use disorder treatment. DWIHN will collaborate with sub-grantee Southwest Counseling Solutions, Inc. to provide rental and administrative services to program participants.
The Continuum of Care Program is a HUD initiative designed to promote community-wide efforts to quickly rehouse homeless individuals and families while minimizing the trauma of homelessness. The program provides funding to state and local governments, as well as nonprofit organizations, to support a comprehensive approach to addressing homelessness, including enhancing access to mainstream programs and optimizing self-sufficiency.
