About this award

The Department of Health and Human Services Health Resources and Services Administration awarded $650,000 to Families Together of Orange County on August 1, 2026, under the Health Center Program (CFDA 93.224) to establish a new access point and Healthcare for the Homeless designation in Westminster, California.

FTOC, a non-profit 501(c)(3) community health center established in 2003, will expand equitable access to primary, preventive, medical, dental, and behavioral health care services, including substance use disorder treatment, at a new facility located at 14571 Magnolia Street, Suite 101, Westminster, CA 92683. The service area is designated a health professional shortage area for primary care, dental health, and mental health. Of the 626,607 low-income residents in the service area, approximately 425,466 lack access to primary care services due to a 32.10% penetration rate among low-income populations. The Healthcare for the Homeless designation responds to a 10.7% increase in homelessness since 2019 across Orange County.

The grant period runs from August 1, 2026, through July 31, 2027, with place of performance in Tustin, California. FTOC currently operates health clinics in Tustin and Garden Grove and has received prior HRSA funding to support COVID-19 vaccination and testing, care access expansion, and family planning and preventive care services.

Description

Ftoc is an existing hrsa health center program look-alike and is applying for the FY 2025 hrsa new access point (NAP) grant. Established in 2003, ftoc's mission is to assist in overcoming obstacles and serving as the bridge between the people within our community and the services that they need as pertaining to health, wellness, and practical services, and while doing so empower individuals to develop further personal and familial success, as well as participate in giving a hand up to their neighbors. Ftoc provides high-quality comprehensive and affordable primary and preventive medical, dental, and behavioral health care services including services for substance use disorder. Ftoc is applying to add a new access point in westminster, CA to be located at 14571 magnolia ST., STE. 101, westminster, CA 92683. The ftoc proposed NAP is located in a health professional shortage area for primary care, dental health, and mental health. Although there are other safety net providers in orange county the there is only a 32.10% penetration rate among the low-income residents of the service area, meaning that more than 67.90% of the low-income population are not receiving any primary care services. There are more than 626,607 low-income residents in the service area leaving roughly 425,466 low-income residents without access to primary care services. Ftoc aims to reduce the number of individuals who are not accessing primary care services. Additionally, families together of orange county is adding healthcare for the homeless (HCH) designation through the hrsa NAP application to better serve the homeless individuals and families throughout orange county as there has been a 10.7% increase since 2019 in homelessness. Ftoc's new access point will expand equitable access to primary and preventive care, with an emphasis on behavioral health and SUD services, for underserved populations. Ftoc is confident they will serve an additional 5,912 unduplicated patients for a total of 27,800 patients by the end of 2026. The proposed site will address critical healthcare barriers identified in the community, such as geographic barriers and transportation difficulties, by offering a central location for in-person services and utilizing telemedicine to reach remote areas. This initiative will significantly improve healthcare access for vulnerable populations, enhancing overall health outcomes in the region and aligning with ftoc's mission to provide high-quality, accessible care to all.
