The PRIME program operates a hub-and-spoke model through a partnership between Connecticut's Department of Mental Health and Addiction Services and Yale's PRIME Clinic. Partner clinics screen all new intakes in the target age range using the PRIME screening tool; positive screens receive comprehensive diagnostic evaluation at the hub; and CHR-P-eligible youth receive individualized stepped care including brief supportive psychoeducation, personalized cognitive-behavioral therapy for early psychosis, and psychiatric consultation. The program will screen over 5,500 youth and serve 32 to 108 unduplicated individuals annually (more than 250 total over the four-year performance period), prioritizing youth involved in juvenile justice, child welfare, and underserved communities. The program builds statewide provider capacity through a tiered training curriculum covering CHR-P detection and stepped care, monthly consultation via the ECHO model, ad-hoc personalized consultation, and dissemination of a co-branded CHR-P provider toolkit through the state mental health authority.
