This $281,020 Continuum of Care (COC) Program grant, awarded by the U.S. Department of Housing and Urban Development (HUD) on March 11, 2025, will support the delivery of a comprehensive range of housing and supportive services by the non-profit organization Community Impact Central Valley (CICV) in Modesto, California.
The COC Program (CFDA 14.267) provides funding to quickly rehouse homeless individuals and families, minimize the trauma and dislocation caused by homelessness, and promote self-sufficiency. Under this grant, CICV will offer transitional housing, permanent supportive housing, rapid rehousing, and a variety of supportive services such as case management, mental health and substance abuse treatment, and job training. The organization will also implement and operate a Homeless Management Information System (HMIS) to track performance metrics. This funding aligns with CICV's broader mission to address homelessness through community-based strategies in the Central Valley region.
