The Sub-recipient, Ascencia, will use the CoC Funds to operate the CES Reallocation Project from September 1, 2024 through August 31, 2025. The project, based in Ascencia's Access Center located at 1851 Tyburn Street, Glendale, CA 91204-2900, will provide individuals and families experiencing homelessness with centralized intake and supportive services to move them along the Glendale Continuum of Care (CoC) and into eventual self-sufficiency. The Program will serve a minimum of 20 persons at any point in time and a minimum of 500 unduplicated persons throughout the 12-month grant term, targeting chronically homeless individuals, families, veterans, and literally homeless people. The Program is operated by Ascencia, the lead agency for Glendale CoC's Coordinated Entry System (CES) and a partner in the Greater Los Angeles Area, specifically in the Service Planning Area (SPA) 2 and adjacent SPA 4 Coordinated Entry Systems. Ascencia's geographic location is the third largest city in Los Angeles County, adjacent to the second largest Continuum of Care system in the county. Working in collaboration with other federal, state and local government agencies, nonprofit rganizations, and health service providers, Ascencia provides comprehensive homeless services to clients in the area. The Program assists people experiencing homelessness in navigating Glendale's homeless crisis response system so that they can obtain the documents required to quickly become match-ready for housing. Each participant is assessed via the Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT), entered into Glendale CoCAfAE'A'AcAfAcAca'AA'AAfAcAca'AA'Acs Homeless Management Information System (HMIS) and prioritized for housing type based on their score. These housing match-ready prioritization lists are visible to all partner agencies within the Glendale CES so they know whom to house first at all times based on the highest scores and available housing opportunities. Ascencia's team of Outreach, Assessment, Intake, Housing and Employment Navigators, and Case Managers specialize in housing location and placement, employment and income support advocacy, trauma-informed mental health care (shifting to healing centered engagement), substance use recovery services, benefits advocacy, and general case management to give participants individualized attention as they are prioritized for services and matched to permanent housing opportunities. Program participants also utilize Ascencia's mail delivery, showers and laundry services, and attend financial literacy classes at Ascencia's CES Access Center. Some participants are referred to Ascencia's 45-bed Emergency Housing Program, located upstairs to the Access Center, while others are referred to other emergency, interim, transitional or permanent housing programs.
