Description
The reinvent stockton foundation (RSF), in partnership with healthforce partners (HFP), community medical centers (CMC), and the stockton strong covid-19 response coalition, is working to replicate the model pioneered by the BAY area's encore physicians program to bring the resources of retired medical professionals and other volunteers of all ages to assist local federally qualified health clinics (fqhcs) in assuring that covid vaccines are delivered to the most at-risk groups in SAN joaquin county. We intend to place 100 volunteers, including 75 rsvp volunteers, in a minimum of 9 or more community health clinics. 65 of the rsvp volunteers will be placed in outcome and 10 in capacity-building assignments. Some of their activities will include supporting federally qualified health centers (fqhcs) with the provision of covid-19 vaccines, communicating with members of the community about the need to become vaccinated, and recruiting other (lay) volunteers to participate in these vaccine events. The primary focus areas of this project are disaster response and healthy futures. At the end of the one-year grant, this program will support federally qualified health clinics (fqhcs) in increasing access to vaccination to approximately 13,440 individuals for covid-19 (based on existing trends). The cncs federal investment of $108,066 will be supplemented by $19,975 in match funds. Necessary training and credentialing will be coordinated by healthforce partners and community medical centers.
