Description
Haskell indian health center sdpi program will provide diabetes treatment and prevention services to native americans/alaskan natives in douglas county, KS and surrounding areas. - haskell indian health center (hihc) is an indian health service (IHS) service unit located in lawrence, KS. And has been caring for the native community since 1978. Hihc sets adjacent to haskell indian nations university campus and serves the native student, faculty, and community of douglas county and surrounding areas. Our 6000+ user population consists of individuals from any of the 574 federally recognized native nations, including the 4+ local tribes. Our clinic is an ambulatory care center that works closely with local health care providers for services not offered in our facility. Hihc recently (2021) opened the new haskell wellness center which provides a home for the diabetes program, a fitness center, physical therapy services, and a class room space with full kitchen. The hihc diabetes program is in IT's 24TH year of receiving sdpi funding and has been providing diabetes care and prevention services since IT's creation. Hihc IHS diabetes registry has 531 active diabetes patients, which is 8.8% of all registered hihc patients. Our program has chosen glycemic control as its sdpi best practice to focus on in 2023. This is due to only 59% of the haskell IHS diabetes registry has an HBA1C < 8.0%. Our goal is to increase that percentage to 70%. Consistently elevated blood glucose poses serious health risks for diabetics, such as kidney failure, lower limb amputations, and diabetic retinopathy (blindness). Our strategy for treatment will consist of utilizing an integrative healthcare approach to providing comprehensive education and resources to overcome nutrition challenges and/or barriers to getting enough physical activity. We use a targeted approach through rpms screenings to identify individuals with glycemic control issues. We will offer diabetes self-management education (dsme) instruction to individuals struggling with a high HBA1C or any other high-risk indicator of metabolic distress. Our registered dietitian will meet periodically with individuals and provide comprehensive advice and nutrition information. Our program will offer personalized fitness instruction by a certified personal trainer to help individuals start an exercise program. Hihc diabetes program has preliminary CDC-recognition and will offer a CDC-approved diabetes prevention program (DPP). This year-long program will target individuals who are pre-diabetic based on approved criteria and will present weekly instruction either in-person or on-line. This program is designed to delay or stop the onset of diabetes in individuals. We are planning on having (2) 15-person cohorts in DPP classes in 2023. In conjunction with the DPP we will be implementing a farmer's market fresh food token program that allows individuals to obtain fresh food from vendors at local farmer's markets through a token system. This program will lessen the food insecurity and allow access to fresh vegetables on a regular basis during the course. This fresh food token program will be offered to other individuals who may be struggling with hyperglycemia or other diabetes-related issues. In 2023 the hihc diabetes program will also implement at least 4 community events that are focused on nutrition, physical activity or both. The activities will range from walking challenges, weight loss challenges, and nutritious cooking seminars as well as other topics designed to engage public awareness and participation in healthy behaviors. Our goal is to increase the health and wellness of the haskell indian health center's native community.
