Description

A collaborative effort between IHS and the tribe to provide diabetes prevention and management activities and services to reduce the incidence of diabetes on the standing rock reservation - diabetes is one of the more prevalent public health issues that has a direct impact on individuals who reside on the standing rock reservation. Statistics that measure risk factors that contribute to the development of diabetes in this region are alarmingly high. Health behaviors that increase the risk of developing diabetes include obesity, food environment index, physical inactivity, and access to exercise opportunities. The incidence and prevalence of diabetes negatively impacts tribal members, as uncontrolled diabetes leads to numerous health complications that can be prevented through support, education, and increased access to physical activity opportunities. The goal of fort yates IHS sdpi is to provide diabetes prevention and management education on-site in all communities and reservation schools and to increase access to physical activity opportunities in every community within the standing rock sioux tribe (srst). Proposed activities include: (1) group & individual diabetes self-management education using the ADCES7 self-care behaviors framework in every community on the reservation, (2) virtual grocery store tours to educate individuals on how to shop for healthy foods while navigating through our local grocery stores, (3) hosting a diabetes conference to provide individuals in our community an opportunity to participate in diabetes prevention and management education in a non-clinical setting, (4) collaboration with the standing rock diabetes program (srdp) to offer primary prevention services through community screening events, health fairs, and physical activity events, (5) nutrition education with gardening to promote healthy eating choices in second and third-grade students, (6) assistance in obtaining measurements for childhood screenings in all reservation schools, (7) hosting diabetes bingo in all communities to increase knowledge of diabetes self-management and care goals, (7) assisting srdp in implementing diabetes education in tribal schools (dets) for students who attend a school within the srst. Proposed program plan (1) identifying that the ultimate desired result is decreasing the prevalence of diabetes on the srst, (2) identifying that diabetes prevention is a priority in health issues within all communities on the srst, and identify behavioral/environmental factors that can hinder achieving diabetes prevention, (3) identifying the predisposing, enabling, and reinforcing factors that can affect the behaviors, attitudes, and environmental factors given priority in phase 2, (4) recognize the tribal administrative and policy factors that influence what can be implemented, (5) implementation of proposed interventions. Our program will select evidenced-based diabetes prevention and management curriculum and provide opportunities for community members to gain knowledge in diabetes prevention and management. Physical activity events will be tailored to all age groups and conducted in all communities, (6) process evaluation of interventions to ensure that target population is reached and program goals are achieved, (7) impact evaluation by conducting pre/post surveys and long term follow up to assess if there is behavior change, monitoring measurements to look for improvements in BMI, glucose control, HGBA1C, and blood pressure in individuals who participate in education or physical activity events, and (8) outcome evaluation by measuring incidence and prevalence of diabetes on the srst over a period of time to assess effective program interventions. Access to healthcare is a barrier in diabetes prevention and management in srst's isolated and medically underserved communities. The proposed project is vital in ensuring that all communities on the srst have access to diabetes prevention and management education and physical activity opportunities. Our program can facilitate services in all communities to improve access to resources that will positively impact overall health and wellness of residents.
