Description
Special diabetes program for indians based on the yankton sioux reservation - the wagner IHS diabetes program utilizes a multidisciplinary approach to prevent and treat diabetes on the yankton sioux reservation in south central south dakota. The program is located within the wagner IHS service unit and uses both clinical and community approaches to increase community awareness of diabetes and provide culturally appropriate diabetes education. The program has been receiving special diabetes programs for indians (sdpi) funding since 1998. The wagner IHS diabetes program has continued to work towards educating community members about diabetes management and prevention as well as reducing the risk for diabetes related complications. The wagner IHS diabetes program is accredited by the association of diabetes care & education specialists in diabetes self-management education and support (dsmes). For the 2023 community-directed grant our program has chosen diabetes-related education as our best practice. To achieve improvement in the required key measure (RKM) we will implement a multifaceted approach to include diabetes education, community education events, nutrition education, continuous glucose monitoring, and physical activity education. Providing quality diabetes education can help improve glycemic control. The success we have in our best practice will also reduce the risk for complications such as retinopathy, kidney disease, cardiovascular disease, and amputations. The success of the program will have a direct impact on the quality of life in the patients and communities we serve. Other activities for the wagner IHS diabetes program will include retinopathy screening, foot exams, cholesterol evaluations, nutrition education for diabetes prevention, and physical activity education for diabetes prevention. These activities are part of a well-rounded program that can positively impact all aspects of diabetes prevention and management. The program also incorporates youth diabetes prevention activities. We provide health screening in 3 local schools. If a child is found to have risk factors for diabetes, the child and their family is invited to participate in a diabetes prevention program. We also provide monthly lessons on healthy lifestyles to youth in local schools. Focusing on youth diabetes prevention activities will promote positive lifestyle changes and reduce the incidence of diabetes in future generations.
