COMBINED SYNOPSIS/SOLICITATION W91ZLK-20-R-0023 This combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Incorporated provisions and clauses are those in effect tough Federal Acquisition Circular (FAC) 2020-08 (14 July 2020). The solicitation number for this request for proposal (RFP) is W91ZLK-20-R-0023. This requirement is under the associated North American Industry Classification System (NAICS) Code 334519, Other Measuring and Controlling Device Manufacturing. The Government contemplates award of a Firm-Fixed Price purchase order in accordance with FAR 13, Simplified Acquisition Procedures. This procurement will be evaluated as lowest price technically acceptable, for the procurement of the following services. The contractor, as an independent party and not an agent of the government, shall provide maintenance, repair, and calibration of Teledyne API & Met One Analyzers to ensure accurate operation. All equipment will be shipped to the company for calibration and repair performed at their facility. No work will be performed at the installation. All work to be performed will be inspected by the government for compliance with requirements by the Contracting Officer’s Representative (COR). Price Restraint: NSP Volume II: Detail Technical Proposal shall be evaluated based on the following factors: Technical Approach: The offeror’s proposal shall adequately describe and present a clear understanding of the requirements specified in the PWS. To be rated as acceptable, the offeror must demonstrate the ability to provide DI Water System Equipment Maintenance Service according to the PWS. Experience/Certifications: Contractor shall be contractor trained and certified to maintain the covered before the effective date of this contract. Contractor shall provide proof of this certification for servicing the specified equipment in the contract and the personnel must have a minimum of tee (3) years’ experience servicing and repairing all equipment covered in the contract in accordance with the PWS. As used in this clause— Covered foreign country means The People’s Republic of China. Covered telecommunications equipment or services means– (1) Telecommunications equipment produced by Huawei Technologies Company or ZTE Corporation (or any subsidiary or affiliate of such entities); (2) For the purpose of public safety, security of Government facilities, physical security surveillance of critical infrastructure, and other national security purposes, video surveillance and telecommunications equipment produced by Hytera Communications Corporation, Hangzhou Hikvision Digital Technology Company, or Dahua Technology Company (or any subsidiary or affiliate of such entities); (3) Telecommunications or video surveillance services provided by such entities or using such equipment; or (4) Telecommunications or video surveillance equipment or services produced or provided by an entity that the Secretary of Defense, in consultation with the Director of National Intelligence or the Director of the Federal Bureau of Investigation, reasonably believes to be an entity owned or controlled by, or otherwise connected to, the government of a covered foreign country. PERFORMANCE WORK STATEMENT (PWS) Title: Teledyne API & Met One Analyzer Annual Maintenance, Repair and Calibration Requiring Activity Name: U.S. Army Public Health Center (APHC) INTRODUCTION The U.S. Army Public Health Center (APHC) is part of the U.S. Army Medical Command (MEDCOM) and focuses on epidemiology, laboratory sciences, occupational health sciences, public health information and toxicology to enhance Army readiness and assure the quality and effectiveness of Army’s Public Health Enterprise. PURPOSE AND BACKGROUND The mission of the APHC is to “Enhance Army readiness by identifying and assessing current and emerging health teats; developing and communicating public health solutions; and assuring the quality and effectiveness of the army’s public health enterprise.” The Army Public Health Center (APHC) operates multiple Mobile Ambient Air Monitoring Stations (MAAMS) in support of the Army's mission to provide Occupational and Environmental Health Surveillance as mandated per DoD 6490.02E, AR 40-5, DA PAM 40-11, and DODI 6490.03. Optimum equipment performance is paramount in APHC's effort to meet air monitoring station data quality and data capture objectives. To meet these performance objectives, the continuous air monitoring equipment requires constant maintenance and repair by certified equipment technicians. Because the equipment runs continuously when installed in the stations, preventive maintenance and/or repair should to be performed on an annual or semi-annually basis for most of the equipment. SCOPE The APHC requires an annual preventive maintenance contract including but not limited to the replacement of internal capillaries, sample lines, batteries, and pump rebuild kits or the replacement of sample pumps. Equipment repairs are case specific and often include the replacement of electronic controls, repair or replacement of hardware, and troubleshooting. Some analyzers may require various hardware replacements and repairs not identified in the initial quotes. All repair and maintenance procedures performed on the instruments require a documented multiple point calibration by the service technician following the procedure to ensure the instruments are performing within acceptable limits. This Quality Assurance Surveillance Plan (QASP) is a government-developed document used to determine if the contractor's performance meets the performance standards contained in the contract. The QASP establishes procedures on how this assessment/inspection process will be conducted. It provides the detailed process for a continuous oversight process: 3.1.1. The Government will use the QASP which provides a systematic method to evaluate the services that the Contractor is required to furnish, but not the details of how the Contractor accomplishes the work.. 3.1.2. The QASP has been developed by the requiring activity. It is designed to provide direction to personnel performing contract surveillance activities. Personnel surveying the contract terms and conditions and PWS requirements will periodically review the QASP toughout the life of the contract. 3.1.3. The COR will document the monitoring on the monthly COR report. 3.2. The contractor is responsible for implementing and delivering performance that meets contract standards using its Quality Control Plan. The QASP provides the structure for the government's surveillance of the contractor's performance to assure that it meets contract standards. It is the government's responsibility to be objective, fair and consistent in evaluating contractor performance. 3.3. The QASP is not part of the contract nor is it intended to duplicate the contractor's quality control plan. This QASP is a living document. Flexibility in the QASP is required to allow for an increase or decrease in the level of surveillance necessary based on contractor performance. 3.4. The government may provide a copy of the QASP to the contractor to facilitate open communication. In addition, the QASP should recognize that unforeseen or uncontrollable circumstances might occur that are outside the control of the contractor. Bottom line, the QASP should ensure early identification and resolution of performance issues to minimize impact on mission performance. Authority Authority for issuance of this QASP is provided under Part 46 of the Federal Acquisition Regulation, Inspection of Services clauses, which provide for inspection, acceptance and documentation of the service called for in the contract or order. This acceptance is to be executed by the contracting officer or a duly authorized representative. Roles and Responsibilities Contracting Officer Representative. The Contracting Officer Representative (COR) is responsible for quality assurance guidance and to ensure that contract quality requirements, provisions, standards, and tesholds are defined, practical, enforceable, necessary, and verifiable. The COR evaluates and documents contractor performance in accordance with the QASP and PWS. The COR notifies the Contracting Officer of any significant performance deficiencies using a Contractor Discrepancy Report with supporting documentation. The COR maintains surveillance documentation. The COR recommends improvements to the QASP and PWS toughout the life of the contract. Contracting Officer. The Contracting Officer is responsible for safeguarding the interests of the United States Government in contractual relationships. Only the Contracting Officer is authorized to bind the Government and then, only to the extent of the authority delegated to them tough the issuance of a warrant. The Contracting Officer delegates authority for inspection and/or acceptance in accordance with the terms of the contract. The Contracting Officer informs the contractor of the names, duties, and limitations of authority for all quality assurance personnel assigned to the contract. Performance Requirements and Method of Surveillance This section describes the special requirements for this effort. The following sub-sections provide details of various considerations on this effort. 6.1 Contract Surveillance The goal of the QASP is to ensure that contractor performance is effectively monitored and documented. The COR's contribution is their professional, non-adversarial relationships with the KO, PM and the contractor, which enables positive, open and timely communications. The foundation of this relationship is built upon objective, fair, and consistent COR evaluations of contractor performance against contract requirements. The COR uses the methods contained in this QASP to ensure the contractor is in compliance with contract requirements. The COR function is responsible for a wide range of surveillance requirements that effectively measure and evaluate the contractor's performance. Additionally, this QASP is based on the premise that the contractor, not the government, is responsible for management and QC/QA actions to successfully meet the terms of the contract. 6.2 Surveillance Matrix 6.2.1. The Surveillance Matrix Table 1 is the list of performance objectives and standards that must be performed by the contractor. This matrix details the method of surveillance the COR will use to validate and inspect these performance elements. Inspection of each element will be documented in the COR file.
