Added: Aug 15, 2013 11:41 am REQUEST FOR QUOTEOCSQ-393-2013-0769This is Request for Quotation prepared in accordance with FAR Part 13, as supplemental with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION: A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services (CMS) intends to award on a competitive basis, under the Simplified Acquisition Procedures in accordance with FAR Part 13 (Dollar Teshold of $150,000), a firm fixed price award for "Conversion of OSCAR'S Model 204-based CLIA database (applications, surveys, billing, complaints, proficiency testing, enforcement and the reporting system) to the QIES server-based platform." The Statement of Work and Technical Evaluation Criteria are attached. Responses shall be no more than 10 pages and shall include the following: 1) Technical Understanding and Approach, 2) Personnel, 3) Management, and 4) Price - fully loaded labor rate and anticipated hours (900). The award will be made on the basis of lowest price technically acceptable meeting or exceeding the acceptability standards for non-cost factors. All responsible sources must be registered @ www.sam.gov. The period of performance is award tough 1 year. The North American Industry Classification Code (NAICS) is 541990. Responses must be sent electronically in MS Word Format to Lucille.Lee@cms.hhs.gov no later than 11:00am on August 28, 2013. ALL QUESTIONS SHOULD BE SUBMITTED ELECTRONICALLY BY 11:00AM ON AUGUST 21, 2013. QUESTIONS WILL NOT BE ANSWERED BY TELEPHONE. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated.
