This is a Combined Synopsis Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued. The solicitation number is 36C24718Q9705 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. This procurement will be conducted under full and open competition procedures. The associated NAICS code for this requirement is 325412, and the size standard is 1250 Employees. The solicitation document, and incorporated provisions and clauses are those in effect tough Federal Acquisition Circular FAC 2005-94 and 2005-95, effective 1-17-2017. This procurement will be issued under a total small business set aside. The North American Industry Classification System (NAICS) Code is 339112 and the Small Business Administration s (SBA) size standard of 1,050 employees. The Government is seeking a contractor to acquire a leasing contract in accordance with the attached Statement of Work for a Karl Storz Brand Name only Otolaryngology/ENT video system and video cart equipment to support existing flexible and rigid ENT scopes already in use the operating room at the Overton Brooks VA Medical Center in Seveport, LA. The Overton Brooks VAMC intends to award a base year plus four option years, firm fixed price, contract to a contractor authorized to distribute Karl Storz products with a period of performance of November 1, 2018 tough Sep 30, 2023. EVALUATION OF QUOTE The Government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable. The Government will evaluate the following: Price Following receipt of quotes, the Government will perform a comparative evaluation of the products/services quoted. The Government will compare quotes to one another to select the product/service that best benefits the Government by fulfilling the requirement. The evaluation of Government requirements outlined in this request for quote will determine suitability. Contractors may submit more than one quote; however, contractors are strongly encouraged to submit their best technical solutions and prices in response to this request for quote. Sources Sought/Request for Information 36C25618U0411 Market Research is being conducted, and this Sources Sought is issued to determine the availability of vendors capable of providing the requirement for KARL STORZ Endoscope Lease. The Department of Veterans Affairs, Network Contracting Office (NCO) 16 seeking vendors capable of providing the leasing of Karl Storz Otolaryngology/ENT Video System and Video Cart equipment is needed to support existing flexible and rigid ENT scopes already in use in the operating room at the Overton Brooks VA Medical Center, 510 E.
