Intent to Sole Source-B2H Consulting Group LLC SPECIAL NOTICE: 36C24923Q0172 The Department of Veterans Affairs intends to issue sole-source firm-fixed priced contract to B2H Consulting Group, 2039 Morgan Ave N Stillwater, MN 55082. In accordance with Federal Acquisition Regulation (FAR) 13.106-1(b)(1)(i) and on behalf of the Memphis VA Medical Center, Network Contracting Office (NCO) 9 intends to negotiate on a sole source basis with B2H Consulting Group LLC located at 2039 Morgan Ave N Stillwater, MN 55082. The rationale basis for the sole source acquisition is that B2HCG is the only responsible source that can provide the original equipment manufacturer (OEM) parts for the existing ACIST equipment. The letter from the manufacturer shall state that the contractor is an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The maintenance shall be provided by original equipment manufacturer (OEM)-certified technicians and shall use only OEM-approved replacement parts and proprietary software. The NAICS code is 339112 and has a size standard of 1,000 employees for this procurement. Period of Performance: 04/21/2023-09/30/2023 Statement of Work Memphis VAMC Summary of Requirement The Memphis Veterans Affairs Medical Center (VAMC), 1030 Jefferson Ave, Memphis, TN 38104, has a requirement for ACIST CVI Contrast Dye Injector System with 7-year warranty. Item provides variable pressurized rate control for all cardiac and peripheral injections. User-responsive, variable, and fixed rate injector from 0.8 to 40 ml./se. Automatic contrast refill settings, monitoring sensors for Air Column detect, Isolation manifold, contrast source empty, contrast syringe refill and contrast source isolation. Scope The Contractor shall be required to furnish all items, to include but not limited to, project management, labor, equipment, materials, travel, and any incidentals required to satisfy the requirements of this Statement of Work. The Contractor shall be required to be responsible for timely delivery, installation, training to ensure the successful completion of the tasks as outlined in this Statement of Work. The Memphis VAMC has been utilizing this current technology for the past 10 years. Along with additional training, a smooth application and install is expected. Delivery Terms: Shipped upon PO received from the account. Installed by the vendor and clinical specialist. Proof of Delivery - The invoice and packing list must be retained as proof of delivery. service and repair of ACIST products. Vendor performance expectations The vendor will be at the Memphis VA to set up equipment and train staff and physicians on the ACIST CVi Contrast Delivery System once the equipment arrives at the hospital and ready to be install once BioMed has reviewed the equipment. The vendor site visit will be set up with the vendor once the equipment has arrived. The Contractor shall provide one (1) set of user manuals and technical manuals to VA Engineering. A copy of the service report shall be provided to both the using Service and Biomedical Engineering. The contractor shall verify to Engineering that the system meets all requirements stated on this contract tough demonstration and validation. Warranty period will start upon sign off and acceptance signature from COR and using services. Sign-in Procedures: All Contractor workers are required to sign in and out at the VA Police Dispatch at the corresponding locations as directed by the COR or designee at each facility. A valid state driver s license or state identification card is mandatory for all employees to have access to these facilities. All contractor employees are required to wear the assigned VA badge always. Work Hours: Normal business hours are 7:30AM to 4:30PM Monday tu Friday excluding Federal Holidays. Work completed outside this time shall be requested tough the COR. Requests for afterhours work shall be submitted in writing to the COR two (2) weeks prior to work. The VA requires that information submitted shall contain extent of work, workers involved, the affected areas, and the estimated times of operation. ID Badges: All workers are required to obtain a time-limited I. D. badge from the VA Police Service located on the ground floor. This badge shall always be worn when workers are on site.
