COMBINED SYNOPSIS/SOLICITATION
PROVIDE A MOBILE INTERVENTIONAL RADIOLOGY ANGIOGRAPHY TO THE OKLAHOMA CITY VAMC.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA256-16-Q-0263 and is issued as an (RFQ) Request for Quotation, unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-85. The associated North American Industrial Classification System (NAICS) code for this procurement is 621512 (Diagnostic Imaging Center). The small business size standard is $15 million. This acquisition is Set-Aside for Small Business Concerns.

All Proposal Questions shall be submitted to the Contracting Officer via email to Tammie.chaney@va.gov no later than 3:00 p.m. Central Local Time on Friday, 5, 2016. All responses to questions, which may affect offers, will be incorporated into a written amendment to the solicitation.
SITE VISIT
Offerors or bidders are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award, nor relieve the successful offeror from performing the work in strict accordance with the true intent and meaning of the specifications.
The site visit date is set for Thursday, February 4, 2016 at 10:00 a.m., Central Local Time.
