Notice of Intent to Sole Source
The Naval Medical Center San Diego (NMCSD) intends to award a sole source purchase order under the authority of FAR 13.106-1(b) due to only one source being available to meet the Government's needs to Steris Corporation, 5960 Heisley Rd., Mentor, OH 44060-1834.
The requirement is for a preventive service maintenance and repair of installed Steris equipment at USNH Okinawa Japan. Service to include all repairs and/or replacement parts to the equipment as listed on the Statement of Work. Equipment must be replaced with OEM parts or certified new replacements.
This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to perform the required work. Capability statements shall not exceed 5 (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.

STATEMENT OF WORK
Calibration/Maintenance of Sterilizing,
Decontamination Equipment
BACKGROUND
USNH Okinawa Main operating room requires on-site preventive, repair and emergency services for installed Steris sterilizing, and decontamination equipment. This Statement of Work (SOW) details the effort that must be undertaken to carry out the service and maintenance on the listed equipment described above. This contract includes all repairs and or replacements needed to the covered equipment, including but not limited to normal wear and tear and government-caused damage.
GENERAL SCOPE OF WORK
A preventive maintenance and inspection program shall be implemented to correct and prevent discrepancies and maximize the overall operating efficiency of the listed equipment. Preventive maintenance shall be conducted quarterly by Original Equipment Manufacturer (OEM) trained personnel only. All preventive maintenance will be in accordance with manufacturer service literature. The contractor shall contact and notify USNHO Medical Repair at 646-7376 at least two (2) days prior to providing preventive maintenance services.

DETAILED WORK REQUIREMENTS
The contractor shall provide the following support.
CORRECTIVE MAINTENANCE
The contractor shall provide preventive maintenance and inspection program shall be implemented to correct and prevent discrepancies and maximize the overall operating efficiency of the listed equipment. Preventive maintenance shall be conducted quarterly by Original Equipment Manufacturer (OEM) trained personnel only. All preventive maintenance will be in accordance with manufacturer service literature.
MAINTENANCE REPORTS
The Contractor shall furnish a signed System Maintenance Record written in English to the hospital upon completion of each maintenance (preventive and corrective) service and for each service call placed. The report will itemize all service performed as a result of either preventive or unscheduled maintenance.
SERVICE CALLS
Only Medical Repair personnel are authorized to contact the Contractor for service calls concerning maintenance and repair services. Service calls may be classified as emergency or routine. The appropriate classification will be given to the contractor along with the job service request.
EMERGENCY SERVICE CALLS
Emergency service involves the correction of conditions that require immediate repair to restore essential service. The contractor shall respond by phone within two (2) hours to assess the nature of the situation and provide verbal plan of action to Medical Repair.
EMERGENCY SERVICES
The amount of emergency services shall be UNLIMITED. The contractor shall be at the job site within forty-eight (48) hours after receipt of an emergency service call and shall restore the equipment to the fully functional status within 7 calendar days. Emergency service calls outside normal work hours will only be approved by the Biomed Dept LCPO or LPO, who is also the Technical Representative (TR).
ROUTINE SERVICE CALLS
Routine service calls are for work that is not classified as an emergency. Routine work requests shall be coordinated with Medical Repair and be scheduled during the normal work week Monday thru Friday 07:30 - 16:30. Routine service calls outside normal work hours will only be approved by the Biomed Dept LCPO or LPO. Routine maintenance will be coordinated with Medical Repair with at least two (2) days advance notice prior to the performance of maintenance.
REPLACEMENT AND REPAIR PARTS
The Contractor shall furnish only OEM parts to effect all replacement or repair. The Contractor shall bear all cost associated with obtaining the parts. All parts installed shall become the property of the Government and all replaced parts shall become the property of the Contractor.
TRAVEL AND RELATED EXPENSES
The contractor shall be responsible for all maintenance and repair expenses under this contract, including but not limited to related trip expenses such as travel, mileage, lodging, and meals, if applicable.
GOVERNMENT RESPONSIBILITIES
Government personnel will not perform maintenance or attempt repairs to the equipment while the equipment is covered under this maintenance contract.
The Government personnel will permit the Contractor access to the equipment to be maintained and provide ample workspace for the maintenance.
The TR will furnish to the Contractor the names of the Medical Repair personnel authorized to place service calls. Any service rendered by the Contractor without approval of one of those individuals will be at the Contractor's own expense and the Government will not pay for the service.
CONTRACTOR RESPONSIBILITIES
At least one of the technicians performing the maintenance on site must speak and write English.
The Contractor shall provide a point of contact and phone number(s) to which services may be made during the period of performance, e.g. trouble desk, answering machine, or pager system.
In all instances where Government property or equipment is damaged by the Contractor's employees, a full report of the facts and the extent of the damage shall be submitted to the TR within 24 hours following the occurrences. The TR shall then forward the report to the Contracting Officer within 24 hours of receipt.
The Contractor shall provide a two (2) day advance notification to the TR before performing any annual scheduled preventive maintenance.
The contractor is responsible for providing OEM replacement parts for the repair of the listed equipment in this PWS.
Normal Work Hours:
The contractor shall report to work Monday through Friday, work a minimum of eight hours a day, between the hours of 8:00 a.m. to 5:00 p.m. Specific work schedules will be provided on contract start date.
Delays
The USNHO TR will review any contractor delay or failure to perform to determine if the delay is caused by technical factors beyond the contractor's control and without fault or negligence by the contractor. If the review indicates performance deficiency is not the result of factors beyond the contractor's control, the contractor will be notified. The contractor shall respond within 24 hours of receipt of notification with remedies to correct the deficiencies.
