Health Tests: Contractor shall submit proof of the following with their proposal. Tests shall be current within the past year.



(a) Tuberculosis Testing - All contractor personnel shall provide documentation of a Purified Protein Derivative (PPD) test performed within the past year. In case of a past positive PPD test, a negative chest radiographic report to rule out active tuberculosis shall be provided. The PPD test shall be repeated annually.



(b) OSHA Regulation Concerning Occupational Exposure to Bloodborne Pathogens - The contractor shall provide training or a self-study training module to its personnel; provide Hepatitis B vaccination series at no cost to its personnel who elect to receive it; maintain and distribute an exposure determination and control plan to its personnel; maintain required records; and ensure that proper follow-up evaluation is provided following an exposure incident. Contractor shall provide documentation that the employee(s) have received the Hepatitis B vaccination series or that the employee(s) declined to receive the series.



VA shall notify the Contractor of any significant communicable disease exposures to its employees as appropriate. The Contractor's occupational health provider shall adhere to current CDC/HICPAC Guideline for Infection Control in health care personnel (AJIC 1998; 26:289-354) for disease control. If the employees of the contracting agency suffer a communicable disease, the Contractor shall provide follow up documentation of employee's clearance to return to the workplace prior to their return.

Added: Aug 08, 2008 12:46 pm The Hunter Holmes McGuire VA Medical Center, Richmond, Virginia intends to negotiate a commercial procurement for Mobile Lithotripsy Services one day a month. The unit must be prepared to perform the first procedure of the day at 8a.m. and continue to perform procedures until the VAMCs schedule for the day is completed. The contractor shall provide a trained, licensed, credentialed technician to assist the VAMC physician. This is a proposed contract action for services for which the government intends to solicit as unrestricted. The NAICS code is 621999, $9.0M. This requirement is subject to the availability of funds. Machine must be able to treat all stones anywhere in the urinary tract. Machine should have a greater than 80% success rate (based on FDA clinical study data). The VA reserves the right to make a single firm-fixed price award. Solicitation documents may be obtained electronically at: http://www.fedbizopps.gov. Award will be made to the offeror whose offer conforming to the solicitation will be the best value to the Government, price and other factors considered. The Government intends to evaluate offers and award without discussions. However, the Government reserves the right to conduct discussions later if determined necessary by the Contracting Officer. Proposals must be received by 4:00 PM, August 29, 2008 local time. Quotations shall be submitted to: Hampton VAMC, 100 Emancipation Drive, Bldg. 27, Supply, Hampton, Virginia, 23667, and directed to the attention of Faye Dillard. Solicitation number shall be listed on the outside of responses for identification purposes.
