This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All vendors must be registered in the System for Award Management SAM)

The Alabama National Guard is seeking to purchase Medical Equipment to conduct Physical Exams for soldier's medical readiness. This requirement is a total small business and only qualified sellers may submit bids. The Alabama National Guard requires the following Brand Name items listed below:


0001, Industrial Acoustics Company, IAC252- HEARING TEST BOOTH, 12 EACH
0002, BENSON, CCA-200M EXPANSION AUDIOMETER, 12 EACH
0003, BENSON, BAS-200- SIMULATOR, 12 EACH


Delivery location 5300 57TH STREET N., BLDG 1 BIRMINGHAM, AL 35217; times will be specified at the time of award. If you have any questions, please email me at amelia.p.davis.civ@mail.mil. All questions must be submitted by 3:00 p.m. on 21 SEP 2018.


New equipment ONLY, NO remanufactured products


Bid MUST be good for 30 calendar days after submission


FOB Destination CONUS (Continental U.S.)

SECURITY REQUIREMENTS
A. Work performed under this PWS is UNCLASSIFIED. The Contractor during the performance under this contract shall comply with all applicable Department of Defense (DoD) security regulations and procedures. The Contractor shall coordinate with the COR all issues dealing with access to Government buildings, facilities, or property.


B. The Contractor will ensure that personnel have proper identification for entry or access to location.


SUBMITTALS: All offers are to include:

Simplified Acquisition Procedures will be utilized. Offers will be evaluated as lowest price technically acceptable. The Government intends to evaluate offers and award a contract without discussions with offerors (except clarifications as described in FAR 15.306(a).


The clause at FAR 52.212-4, Contract Terms and Conditions; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply to this acquisition.
