Added: May 28, 2015 8:23 pm Amendment No. Three is hereby issued to Solicitation No. 15-247-SOL-00057 as follows:

1. Extend the Solicitation closing date to 6/5/15 at 12:00 pm, Phoenix, Arizona Time.


2. Add to Section C, Description/Work Statement as follows:


a.) Section C-1, add the following: This is a unit price contract; offeror will provide a price per linear foot. This is a multi-year service contract with a base year and 4 option years to extend.


An option to this service contract is for the contractor to provide all equipment, supplies and services (including overhead, profit, deliver, taxes and miscellaneous expenses) to remove and replace approximately 2,300 square feet of pavement which is cracked at a width greater than 1-inch. This option is a unit price contract, offerer will provide a price per square foot.


b.) Add the following to Paragraph 4.3 as follows: The Facility Manager shall be notified a minimum of 48 hours in advance of any work resulting in the disruption of parking or traffic flow through the parking lot.


c.)Add to Paragraph 7.1, number 4, as follows: Submit Specifications for removing and replacing pavement including a description of proposed methods and material submittals.


d.) Revise Part 8, Detailed Description of Work as follows: SEALING OF CRACKS IN, AND REMOVAL AND REPLACEMT OF, BITUMINOUS PAVEMENTS.


3.Offeror shall complete the Price Schedule, Attachment C, with a Price Proposal.


4.Offeror shall submit a payment bond within 10 calendar days.


5.All other terms and conditions of the solicitation remain unchanged.

1. Extend the Solicitation Closing date to 5/29/15 at 12:00pm, Phoenix, Arizona Time.

Added: May 09, 2015 3:59 pm Modified: May 18, 2015 5:11 pmTrack Changes The Phoenix Indian Medical Center (PIMC) is seeking qualified vendors who can provide all equipment, supplies and services (including overhead, profit, delivery, taxes and miscellaneous expenses) to apply approximately 6000 linear feet of crack seal at Phoenix Service Unit (PIMC) parking lot per year. Work to be performed at PIMC, Phoenix, Arizona.

The Government anticipates an award of a Firm Fixed Price contract. The Government intends to award one contract as a result of this solicitation.

The solicitation will be available on or about May 9, 2015 and will be distributed solely through the General Services Administration's Federal Business Opportunities website at www.fbo.gov. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation.

The Offerors are required to submit the Request for Proposal (RFP) by email to: bradley.platero@ihs.gov.

The RFQ due date is close of business 12:00 pm Phoenix, Arizona Time on May 22, 2015. Late proposals will not be accepted.

All contractors doing business with the Federal Government shall be registered in the System for Award Management (SAM) database. The website for registration is www.sam.gov. If the Offeror does not become registered in the SAM database at time of award the Contracting Officer will proceed to award to the next otherwise successful registered Offeror.

It is the Offerors responsibility to periodically check for any amendments to this solicitation.

The North American Industry Classification System Code (NAICS) is 237310 and the Small Business Standard is $36.5 million.

1. Extend the Solicitation closing date to 5/29/15 at 12:00 pm, Phoenix, Arizona Time.


2. Add the following:


52.236-27 SITE VISIT (CONSTRUCTION) (FEB 1995):


(a) The clauses at 52.236-2, Differing Site Conditions, and 52.236-3, Site Investigations and Conditions Affecting the Work, will be included in any contract awarded as a result of this solicitation. Accordingly, offerors or quoters are urged and expected to inspect the site where the
work will be performed.


(b) An organized site visit has been scheduled for May 20, 2015 at 10:00 a.m., Mountain Standard Time


(c) Participants will meet at --
Phoenix Indian Medical Center (PIMC)
4212 N. 16th Street
Phoenix, AZ 85016-5319


Meet at grassy area just west of the main hospital.


Site Visit participants should check with the Engineering Department, where they will be further directed. The Site Visit will be hosted by the Contracting Officers Representative, LCDR Chris Guess, P.E., Chief of Engineering Services. If a participant has difficulty finding the site visit location or the PIMC, please contact Bradley Platero at (602) 200-5371.
