The combined synopsis solicitation VA101V-16-Q-0321 A00002 is amended as follows.
A. To change the solicitation closing date and time to Monday Aug 22, 2016 at 6:00 AM.
B. To add the following Schedule of Payments:
Maximum amounts payable in arrears:
1. 50% of total contract cost incurred in arears: Contractor must have incurred 50% of the total contract amount in their costs and must submit evidence of the cost incurred with invoice.
2. 20% of total contract cost upon the completion of rough-in / cable pulls. Completed work must be accepted by the government before invoicing.
3. 15% of total contract cost upon the completion of the installation of system components installed. Completed work must be accepted by the government before invoicing.

The following addition Vendors' questions and answers are provided.
1. Question: Will the VA assign a representative who will have the authority to address installation issues which may arise during the installation portion of the project.
Answer: Yes.
2. Question: In reference to the system commissioning checklist (part 5) of solicitation, the document has to be signed by a commissioner. Who assigns this person and what is their role in the overall project.
Answer: The contractor that is awarded the contract assigns its own commissioner---this is an internal quality assurance procedure for the contractor.
3. Question: Can contractors progress bill, such as when they deliver all of the materials to the job site?
Answer: No, the items will need to be installed and accepted by the government before invoicing.
4. Question: Will the contractors have an area to secure their material that can be locked?
Answer: Yes.
5. Question: Are offerors required to submit their itemized equipment list on the Excel form provided by the government in the solicitation or can they submit a different Excel/PDF that will have a line item equipment list and pricing?
Answer: The excel sheet was provided to assist vendors with the pricing. With that said, offerors can submit their own itemized list.

The combined synopsis solicitation VA101V-16-Q-0321 A00001 is amended as follows.
A. To add the following language to the SOW Section 1.3.C Existing Conditions: The contractor is not required to provide firmware updates for Cisco Videoconferencing Codecs and associated Cameras.
B. To change the display in Hearing Room 1-037 to be mounted on the credenza versus the wall. Please see attached updated schematics file with revision clouds around the affected area.
C. To change the connection requirements for Conference Rooms B2-028 and 3-082 as follows: The AV contractor shall make the indicated building network connections for the Digital Audio Processor and the VTC Codec by installing an Ethernet network cable between the data drops located behind the display and the credenza directly below the display. This cable shall be routed through the wall cavity with the other AV cables. The AV contractor shall furnish and install an Ethernet network cable between the data drop located behind the credenza to the Government computer located in a credenza directly below the display. Please see attached updated schematics file with revision clouds around the affected area.
D. To change the connection requirements for Huddle Rooms 3-072 and 3-089 as follows: The AV contractor shall make the indicated building network connections for the VTC Codec by installing an Ethernet network cable between the data drop located behind the display and the credenza directly below the display. This cable shall be routed through the wall cavity with the other AV cables. The AV contractor shall furnish and install an Ethernet network cable between the data drops located behind the credenza to the Government computer and the Digital Audio Processor located in a credenza directly below the display. E. COMBINED SYNOPSIS SOLICITATION
i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The government anticipates a single (all or none), firm?fixed?price award. A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier's quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer.
ii. VA101V-16-Q-0321 the Combined Synopsis Solicitation is issued as a request for quotation (RFQ).
iii. The solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-88.
iv. The combined synopsis/solicitation is set aside for Service Disabled Veteran Own Small Business concerns only. Only quotes submitted by Service Disabled Veteran Own Small Business will be accepted by the Government. Any quote that is submitted by a contractor that is not a Service Disabled Veteran Own Small Business will not be considered for award. Note that only VA verified SDVOSBs in the VIP database at the time of contract award will be considered for award and non-VIP verified firms will be considered non-responsive and ineligible for award. The applicable NAIC Code for this solicitation is 334310 Audio and Video Equipment Manufacturing, and the Small Business Size limitation is 750 Employees.

ix. Offers will be evaluated as follows:
1. Criteria: The Government will evaluate offers submitted in response to this solicitation based on a consideration of the following factors: Price, Delivery Schedule, Experience/Qualifications and Past Performance. Included with each Contractor's quote, the Contractor is to provide a written statement limited to two pages or less demonstrating how the Contractor will comply with the Delivery Schedule and has the required Experience/Qualifications needed to successfully perform this requirement. Contractor shall provide a list of all proposed equipment with description, model numbers, quantities, unit and extended price.
2. Price: Price will be evaluated to determine if it is fair and reasonable.
3. Brand Name or Equal: Contractor shall provide a list of all proposed equipment with description, model numbers, quantities, unit and extended price. The salient physical, functional, or performance characteristics that "equal" products must meet are specified within the Statement of Work. To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must meet the salient physical, functional, or performance characteristic specified in this RFQ. Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the Statement of Work.
4. Delivery Schedule: The Government will evaluate quotes submitted in response to the solicitation to determine whether the Contractor has the capability to meet the 90 day delivery and installation requirement of this solicitation.
5. Experience/Qualifications: The basis of evaluation under this factor will be focused on the firm's experience in performing contracts and /or task orders of similar size, scope and complexity to the work being solicited within the SOW. The quote shall contain information from a minimum of one contract where similar audio visual equipment and installation services were successfully provided. Only quotes that demonstrate relevant experience with providing similar audio visual equipment and installation services will be eligible for award. Government and commercial sources will be accepted. The Contractor must be a Contractor who regularly engages in the furnishing and installation of commercial and industrial audio-visual communication systems. The Contractor must maintain a suitably staffed and equipped service organization and must regularly offer maintenance services for systems of this type and size. At the request of the Government, the Contractor shall demonstrate to the satisfaction of the Government that the Contractor has: Adequate plant and equipment to pursue the work properly and expeditiously; Adequate staff and technical experience; Suitable financial status to meet the obligations of the work. A subcontractor so employed as the Contractor must be acceptable to the Government and shall be identified on the quote.
6. Past Performance: The Contractor shall provide at least one reference that corresponds with the relevant experience provided and demonstrates an acceptable past performance record. The submitted reference's past performance questionnaire responses must not have any question rated less than "good" to be eligible for award. Government and commercial sources will be accepted. The Government reserves the right to consider sources other than just those submitted by the by the Contractor in response to questionnaires in evaluating past performance.
7. Basis for Award: The Government shall make award to that quote that is found to be the lowest priced and meets the Government's stated Delivery Schedule, Experience/Qualifications, and Past Performance requirements based upon the stated considerations above.
x. FAR 52.212-3, Offeror Representations and Certifications - Commercial Items applies to this solicitation. The offeror must have completed the annual representations and certifications electronically via the SAM website access through http://www.acquisition.gov. CL-120 Supplemental Insurance Requirements
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: $500,000.00 per occurrences.
(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.
(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

This solicitation includes 852.219-10, VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Asides. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an "Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement" to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. The contractor guarantees the equipment against defective material, workmanship and performance for a period of 12 Months, said guarantee to run from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States, or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor.

A site visit is scheduled for July 28, 2016 at 10:00 AM. Please meet in the lobby of 400 South 18th Saint Louis, MO 63103 for the site visit. Contractors shall be given access to existing equipment's location within 400 South 18th street and then the site visit shall move to the Prevedel Federal Building's located at 9700 Page Avenue St Louis, MO 63132 for a tour of install room locations.
