This is an amendment to combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The purpose of this amendment 0001 is to add part numbers to the description, please see below.
The solicitation number 36C24822Q0776, 0001 is issued as a Request for Quotation (RFQ) in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract.
The provisions and clauses in this document are in effect through the Federal Acquisition Circular 2022-05, March 7, 2022.
This solicitation is set-aside to a Small Disabled Veteran Owned Small Business associated with NAICS Code 339112 with a size standard of 1000 employees. The FSC/PSC 6515.
The Miami VA Medical Center has a requirement for a Rapid Infuser Brand Name or Equal to Belmont Rapid Infuser R1-2 and comparable disposable. Below are the standard items necessary for the quote.

Salient Characteristics: Rapid Infuser
Medical grade stainless steel heat exchanger eliminates risk of aluminum exposure
Two ultrasonic air detectors and patient safety valve safeguard against air emboli
Fully prime in under 1 minute and easily re-prime without patient disconnection
Precise control of fluid delivery with flow rates ranging from 2.5 to 1000 ml/min
Dry, inductive heating technology and two infrared temperature sensors provide consistent normothermic fluid within seconds
Automatic pressure monitoring and flow rate adjustment when pressure exceeds 300 mmHg
High contrast display provides easy to follow on-screen instructions, clear descriptions of alarms and alerts, and quick access to device operations

Comparative evaluations will be performed after all quotes have been received.
Offeror quotations must demonstrate meeting the salient physical, functional, or performance characteristic specified in this solicitation, otherwise it will be considered non-responsive. The information identified provided is intended to be descriptive, not restrictive and to indicate the quality that will be satisfactory. The interested parties bear full responsibility to ensure their submission demonstrates to the government that they can provide the supplies being requested.
Only new equipment is acceptable; no remanufactured, used/refurbished or "gray market" items. No product in development shall be considered. All items must be covered by the manufacturer's warranty.
At a minimum the quote shall contain the following:
The name, address, and telephone number of the vendor.
Tax Identification Number (TIN)
Unique Entity ID (SAM)/ Cage Code
Point of Contact for submission of orders if awarded a contract
Authorized Distributor Letter certified by OEM.
Quoted price shall include delivery.
