Project Location: VA Phoenix Healthcare System, 650 E Indian School Rd., Phoenix, AZ 85012 1.C. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26225Q0448. 1.D. Set Aside: This solicitation is Unrestricted-Open Market 1.E. Applicable NAICS code: 339112, Medical and Surgical Instruments, Equipment and Supplies 1.F.
