PERFORMANCE WORK STATEMENT
FOR
Trash and Recycling Removal Services 1 April 2017 to 31 March 2018
122nd Fighter Wing
3005 W. Ferguson Road
Fort Wayne IAP, IN 46809-3158 24 February 2017
1.0. PURPOSE: Services under this contract include all plant, labor, and equipment required to remove rubbish and recyclable materials from the Air National Guard Base at 3005 W. Ferguson Road, Fort Wayne International Airport, Fort Wayne, IN 46809-3158.
2.0. DEFINITIONS: The following is a breakdown of terms and abbreviations used in this contract:
2.1. Contracting Officer: Individual who administers the contract and ensures all requirements are met.
2.2. Base Civil Engineer (BCE): Individual responsible for all engineering functions on the guard base including trash removal services.
2.3. Air Guard Base: All facilities and grounds used by the Air National Guard.
3.0. CONTRACTOR RESPONSIBILITIES: The following items are the responsibility of the Contractor:
3.1. The Contractor shall provide waste containers in the sizes shown in paragraph 5.0.
3.2. Containers shall have easy front access that is securable. Containers will have securable sliding door/s as well as flip top hinged plastic panels. Trash material must be easily deposited into the container from the side or top locations. Container, as permissible by cubic yardage size, shall also be of a lower height to allow for concealment behind 5 foot high concrete block storage areas.
3.3. Cubic Yard Capacity. The government reserves the right to change the cubic yard capacity at any collection station or change the number of collection stations as long as it does not increase the overall total cubic yard capacity requirement of this performance work statement.
3.4. Ensure sufficient numbers of Contractor personnel are available and utilized to render all services required of the contract.
3.5. Follow established schedule of emptying trash containers. Once awarded contract, vendor will receive a detailed base map showing proposed locations of containers in relation to the building locations.
3.6. Ensure all containers are completely emptied and any trash/recycling material spilled during removal is picked up and removed by Contractor personnel.
3.7. Contractor shall notify the Contracting Officer or the Contracting Officer’s Representative (COR) in the event that pickup areas are not accessible or have trash/recycling material stored around the container(s) to ensure all efforts can be made for that material to be picked up on the scheduled pickup date. A representative listed in Paragraph 13 will ensure that someone is dispatched to assist in getting any items into the appropriate container and can assist in making an announcement to get a vehicle moved if needed. The Contractor shall give the base 10-15 minutes to rectify the discrepancy prior to departing the base without picking up the container. If this occurs due to our error, the Contractor will not be penalized for failure to pick up the container. However, the Contractor or their representative would need to call and relay the problem to a representative listed in Paragraph 13 so that it doesn’t happen again.
3.8. Notify contracting officer of any deviations required of removal schedule at least twenty-four (24) hours in advance. Only exceptions to the required twenty four-hour notice will be when circumstances are beyond the Contractor's control.
4.0. GENERAL: The Contractor shall return the containers to their original location after servicing, in an upright position with the lids securely in place (closed). The Contractor will also be responsible for immediately cleaning up any spills, debris, etc., which may occur during servicing/emptying of containers. The Contractor shall furnish containers of the same color and in good repair. The Contractor shall maintain all Contractor provided containers ensuring they are free of unpleasant odors, dirt, debris, and pests. All containers must remain in good, serviceable condition. Containers should be marked in a way that indicates which type of material (trash, recyclables, or both) can be put into each container. The Contractor shall perform all cleaning, painting, repair and other maintenance tasks required to keep the units in acceptable condition at an off-base location. Containers must be kept in container storage concealment areas or other designated areas as provided. The Contractor shall empty each TRASH container on Thursday of every week and each RECYCLING container on Thursday of every other week during the term of the contract. Each container (both trash and recycling) shall also be emptied on the Monday after a Regular Scheduled Drill (RSD), normally one time per month. A schedule of known RSDs is provided in paragraph 6.0. Dates of RSDs in Fiscal Year (FY) 2018 and following (beginning 1 Oct 2017) will be provided to the Contractor well in advance to allow for planning.
4.1. Off-Base Disposal. The Contractor shall transport and dispose of all waste and recycling material at their off-base disposal site selected by the Contractor. The Contractor shall sort and utilize all recyclable materials in accordance with their published operational brochures. Disposal of non-recyclable material shall be in accordance with existing local, state, and federal regulations. The Contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations.
4.2. Reports and Records. If feasible, the Contractor shall provide a monthly report detailing total tonnage of refuse collected. The Contractor shall also provide a monthly summary of all materials recycled showing material name and cubic yards/tons recycled.
4.3. Trash and Recyclable Program Literature. If Contractor has electronic media available on their trash/recycling program, it will be made available to the Contracting Officer’s Representative (COR) so that it can be distributed to the base for proper notification of the new program.

6.0 Trash Container Additional pickup schedule.

7.0. PARTS EXCEPTIONS: NONE.
8.0. CANCELLATION: In the event the Government elects to cancel this agreement, it shall provide the vendor, or its authorized service dealer, written notification within thirty (30) days of such cancellation.
9.0. INSPECTION AND ACCEPTANCE OF SERVICE: The Government agrees to provide full access to the vendor to perform required service. Indiana State Maintenance is hereby designated to participate in the administration of this agreement to include inspection and acceptance of the services for the Government at the performance site. The Government will periodically evaluate the Contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The Government representative will evaluate the Contractor’s performance through intermittent on-site inspections of the Contractor's quality control program and receipt of complaints from base personnel. The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the Government may decrease the number of quality control inspections if performance dictates. The Government will also receive and investigate complaints from various customers located on the installation. The Contractor shall be responsible for initially validating customer complaints. However, the Government representative shall make the final determination of the validity of customer complaint(s) in cases of disagreement with customer(s).
10.0. BASE PASSES: Permission for Contractor personnel and vehicles to enter the Air National Guard installation is subject to the approval of the local Base Security Police. The Contractor shall provide a list of personnel to the Contracting Officer of all personnel that will be entering the Base to provide the services of this contract. Each person on the list will submit a background information sheet or will at the minimum have their full name, DOB, and Drivers License # or State Issued ID submitted through the Contracting Officer to the Security Forces Section prior to entering the base. The list must be submitted at least 3 working days prior to start of contract to allow time for a background check. Anytime a new employee is added to the company’s route, a new letter will need to be submitted with the individual’s information as explained above. The request letter must be submitted on company letterhead paper and can be faxed or attached to an email. Personnel that do not pass the background check will not be allowed to enter the Base. All Contractor vehicles are subject to search by Security Forces.
