Project Scope of Work

Title: Replace Carpet Squares

Building/Location: DFAS, Limestone, ME

Date: February 23, 2017(Revised 4/25/2017) Prepared By: Michael Kinney (DFAS COR)

1. GENERAL SCOPE: The work covered in this Scope of Work, as well as the related drawings consists of furnishing all personnel, labor, materials, tools, equipment, transportation, delivery and other items and services necessary to perform all services identified herein to accomplish all work in connection with replacing all Carpet Squares and associated trim located at DFAS Limestone, Maine.

2. PRINCIPLE FEATURES: The primary features of work include removing, disposing and replacing existing Carpet Squares and moldings. Work must meet all applicable Local, State, Federal, UFC, NAPA and NEC Codes.

3. SUPERVISOR: The contractor shall have a designated supervisor on site at all times during work.

4. PERIOD OF PERFORMANCE: The Contractor shall be allowed a total of 90 calendar days upon notice to proceed to accomplish this project, as described herein. At least 50 percent of work shall be completed No-Later-Than July 22, 2017 regardless of contract award date. Note: No work will occur during the months of August, September and October 2017 due to FY17 closeout activities.

5. NOTIFICATION OF COMMENCEMENT: The Contractor shall confirm the performance start date with the Contracting Officer at least 48 hours prior to intention to start construction.

6. MEASUREMENTS: Contractor shall field verify all measurements and or quantities listed in the SOW prior to award. Only one site visit will be conducted; all interested contractors or their representatives should plan to attend the planned site visit. (FAR 52.237-1)

7. CLEANUP: Contractor shall clean up the job site daily. The Contractor shall provide their means for removing debris from the work site. The Contractor shall use their equipment to clean the work area.

The Contractor shall clean the area as many times as necessary after the job is complete to ensure there is no evidence of the work that just occurred.

8. SAFETY: The Contractor shall be responsible for managing the project(s) and maintaining a work site compliant with EM 385-1-1 Safety and Health Requirements Manual. Also, Contractors and Subcontractors must review and sign The DFAS Limestone Contractor Safety and Health Agreement before commencing on-site work. This form will be provided at the Pre-Con meeting after award of contract.

9. SECURITY REQUIREMENTS:

1. Contractor shall comply with all DFAS Personnel security requirements and must submit an initial security background investigation form (DFAS Form 9035), (OF 306) along with a completed fingerprint card (SF-87 Form or FD258) for submission to have a DFAS background investigation for all contractor personnel involved in the project who will be on-site. Contractor must submit security clearance paperwork to DFAS Limestone ASAP after the contract is awarded or 60 days prior to the construction start date. After the initial paperwork is submitted to DFAS Limestone and forwarded to DFAS HQ, an electronic questionnaire (e-QUIP) must be completed by each person to be properly vetted.

No contractor or sub-contractor will be allowed access to DFAS facilities unless properly vetted. No contractor or sub-contractor will be allowed unescorted access to DFAS facilities unless properly vetted.

All contractor personnel must receive a Security briefing prior to starting work on site.

All contractors must comply with DFAS established policy concerning Violence in the Workplace and general rules of behavior while on site at DFAS Limestone. Note: All contract personnel will exercise due diligence in ensuring the DFAS facility remains secure when work is performed in the evenings. The contractor will not “prop open” exterior facility doors and permit unauthorized personnel from entering the facility when DFAS personnel are not present. Any and all violations can and will result in immediate removal of individuals violating this requirement from the facility without exception. All such incidents will be properly investigated by the COR and security personal. Further note: The use of CCTV cameras is used to monitor facility security at all times.

10. SUBMITTALS: Provide submittals for all materials for government approval as listed below for the Carpet Replacement Project using AF 3000, Material Submittal. Submit the prerequisite Quality Control Plan, Accident Prevention Plan with a detailed Activity Hazard Analysis, Project Schedule, Schedule of Values, etc., each of which must be submitted and approved prior to starting work at the site. Ensure all appropriate plans, certifications, MSDS’s, etc. are maintained on the project site and are available for Government review at any time. Provide information for each submittal for government approval as listed below:

a. Manufacturer’s Data: Manufacturer's descriptive and technical data and installation and maintenance instructions.

b. At least one sample Carpet Square(s) for each color approved.

c. Consumer sample of Base Cove Molding and thresholds.

d. Any and all Material Data Sheets for hazardous chemicals used in the project.

e. Warranty information: Provide a written manufacturer’s warranty.

f. Copy of project “As-Builds” will be provided to the COR upon project completion.

11. WARRANTY: Installer shall warrant in writing that the work shall be free of faults or defects, in accordance with the requirements in this Scope of Work for a period one year after project completion;

materials and products used in the project shall have manufacturer’s warrantees respective of the item used.

12. CONSIDERATIONS:

a. The building shall be available for contractor use and others supporting this project. Although, the building will remain occupied and operational during the repair work during normal business hours; carpet square removal and replacement will occur during evening hours (6:00pm to 6:00am) daily and selected weekends and holidays as coordinated by the COR.

b. The Contractor shall provide signage warning occupants of “Active Work Areas” and pathways around work areas.

c. The contractor shall protect existing wall and furniture finishes and items. Any damage to existing walls and cubical furniture resulting from contractor’s activities shall be repaired by the contractor. (FAR 52.237-2)

d. The contractor will remove existing carpet squares and all glue residue that prohibits proper installation of new materials in accordance with manufacturers installation requirements

e. Contractor will be responsible for the cost and arrangement of dumpsters for disposal and removal of the carpet from the site. Location of dumpster shall be coordinated with

COR.

f. Lights shall be used only in areas where work is actually being performed.

g. Floor surface will be prepped in accordance with industry standards to accept new carpet tile installation. This includes but is not limited to removal of thresholds and patching spalls and other imperfections in the floor service with a permanent concrete patch, primer sealant and adhesive.

h. All system (cubical) furniture, and file cabinets, will be moved or safely jacked up by contractor personnel to facilitate removal/installation. Furniture and file cabinets will be replaced in their original positions.

i. The contractor's equipment shall be of commercial quality, size, and type suitable for accomplishing the various phases of work specified and operated from existing sources of government-furnished electrical power and water. All electrical equipment used by the contractor shall meet all safety requirements of this contract and shall be UL approved. It shall be the responsibility of the contractor to prevent the operation or attempted operation of electrical equipment, or combinations of equipment, which require power exceeding the capacity of existing building circuits. All equipment shall have bumpers and guards to prevent marking and scratching of fixtures, furnishings, or building surfaces. The contractor's equipment shall be in good repair and able to operate efficiently and safely. Equipment shall be maintained clean to present a neat, professional appearance. The Contracting Officer or Representative may inspect the contractor's equipment and/or tools at any time and direct the removal of any improper or unsafe equipment/tools. These items shall be removed from the job by the contractor and replaced with satisfactory equipment.

j. The contractor shall at all times provide adequate supervision of his employees to ensure complete and satisfactory performance of all work in accordance with the terms of the contract. The contract manager, or alternate representative, will be on the site and available at all times when the work of the contract is being carried out to receive instructions from the contracting officer or his representative. The contractor shall provide additional telephone and pager numbers where the contract manager and alternate can be reached during other than normal duty hours should the contracting officer deem it necessary.

k. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for carpet removal and installation via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:

http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2014.

l. DFAS Limestone has a custodial contractor. The requirements of this contract may affect the work of the custodial contractor. The contractor will provide a detailed schedule of carpet removal and installation to the COR to facilitate the performance of both contract requirements. The Carpet installation contractor will be responsible to ensure any construction debris or dust is clean up by the end of each days schedule.

n. Access to locked rooms in areas where the carpet is to be removed/replaced will be provided by the COR or their designated representative.

13. SCOPE OF WORK: (Project Description) Work includes furnishing new carpet squares and low VO adhesive per specifications provided below, removal, disposal and installation of approximately 121,100 square feet of carpet squares on the first and second floors of the DFAS facility. (See attachment #1). The majority of the work area involved has existing workstation cubical(s) which require specialized jacking tools in order to raise the cubical(s) enough to effectively replace carpet squares without damaging cubical(s) and workstations. The contractor shall be responsible for replacing wall base cove, adhesive, transition strips and thresholds. The contractor shall ensure floor surface is be prepped in accordance with industry and manufacturer standards to accept new carpet tile installation.

This includes but is not limited to removal of thresholds and patching spalls and other imperfections in the floor service with a permanent concrete patch, primer sealant and adhesive.

The contractor shall move all file cabinets, and all other furniture to facilitate removal/installation of carpet squares. Furniture, file cabinets and cords shall be replaced in their original position or otherwise coordinated by the COR.

13.1 Project Specifications: (Brand Name or equal; FAR 11.104)

The contractor shall provide all personnel, equipment, tools, all material’s, supervision, and other items and services necessary to remove and dispose of approximately 121,100 square feet of existing carpeting and replace with Shaw, Philadelphia Commercial Carpet Tile, Mesh Weave Tile 54458, 58502 Graphite carpet squares (Brand Name or Equal). The work area has UNICOR System 21 cubicles which will require the specialized jacking tool and know how to raise the cubicles to minimize damage to place the carpet tiles. The Shaw, Brown Carpet tiles, Cove Base (Brand Name or Equal) shall be provided by the contractor.

The contractor shall be responsible for providing the cove base adhesive, Floor Primer and Adhesive (submittal required) and necessary transition strips listed in Appendix (B) (Brand Name or Equal).

Appendix A

Project Requirements

1. A site visit on the advertised (walk-through day) is highly encouraged to provide a proposal for this solicitation.

2. Contractor shall be responsible for providing dumpsters and fund disposal (landfill tip) fees.

3. All areas receiving new carpet tile will have existing cove based removed and replaced with new

4” vinyl cove base, color is Medium Grey. Submittal required as identified above.

4. New transition strips and thresholds are required at all transition points of carpet to VCT or tile.

5. Contractor materials shall be stored and secured prior at end of each day. A space shall be provided for storage of tools and materials. Note: All materials associated with this procurement and identified in the SOW shall be delivered to DFAS Limestone and off-loaded by the contractor and stored on-site. DFAS shall ensure able space is provided to the contractor for product and material lay-down.

6. Contractor shall be required to perform moisture test to determine proper level of preparation required to ensure adequate bond of carpet to deck.

7. Contractor shall keep the COR informed of planned work areas at least two (2) days in advance to permit sufficient time to alert employees of dates of work and to allow employees time to remove/secure personnel items (if desired by DFAs employees), and minimize DFAS workplace disruptions. Also, ample time is needed for DFAS maintenance personnel to make planned adjustments to the HVAC controls to provide climate control in affected areas where work is being performed by the carpet contractor.

8. Contractor shall ensure that each nights tear out has received new carpet tile before securing for the day. Employee cubicles will need to be returned to same condition they were in before tear out occurred to include free of dust and debris. The contractor shall ensure every effort to prevent damage and disturbance to DFAS employee personal items located in cubical affected by carpet replacement by the contractor. Any damage shall be reported to the COR immediately.

9. All porous, gritty, chalky and dusty surfaces should be primed in accordance with industry and manufacturer standards. All patched areas must be fully primed. Primer can be applied using a paint roller. Allow the primer to dry completely. Primer turns light blue and will not transfer to the touch when dry.

10. When old adhesives, other than cutback or emulsion adhesives, have been removed, apply floor Primer. This is not a substitute for removal of old adhesive and proper floor debris cleaning, but a safeguard for problems caused by small amounts of old residual adhesive. * Note that the bond of the adhesive may be increased by the presence of any residue of existing adhesive that is not properly removed or encapsulated.

11. All sub-floors should be level. Concrete sub-floors should be troweled smooth and should conform to the standard specifications as recommended by the Portland Cement Association. The floor should be flat to within 1/8” in 10 feet. Cracks, holes and depressions can be filled using Portland Cement/Latex fortified patching material. Do not install over loose tile (VAT, VCT or others loose existing flooring substrates).
