About this file

This document includes a limited asbestos survey report and related federal contract opportunity. The asbestos survey report details the findings of sampling conducted at the Norfork Powerhouse prior to an elevator installation project. Asbestos was detected in the insulation on a drain pipe in the attic. It is recommended the asbestos removal be performed by an Arkansas-licensed abatement company.

The related federal contract opportunity is to install a power plant elevator at the Norfork Powerhouse in Baxter County, Arkansas. The estimated value of construction is between $1,000,000 and $5,000,000. The solicitation is set aside for small businesses with a NAICS code of 238290 and SBA size standard of $16,500,000. Requests for information must be submitted through ProjNet by May 15, 2020 due to COVID-19 travel restrictions preventing an on-site visit.

CESWL-SO 25 September 2017

MEMORANDUM FOR Mr. Terry Madden, Superintendent, Norfork Powerhouse

SUBJECT: Limited Asbestos Survey at Norfork Powerhouse for the Elevator Project

1. A limited asbestos survey was performed on 10 August 2017 at the Norfork Powerhouse located at 487 Highway 177 South, Mountain Home, Arkansas. The survey was performed by

Laura H. Weems, Arkansas Building Inspector, Certificate Number: 015340 (expires

2017/10/31).

2. The survey was requested to determine if the materials, including ventilation and pipe insulation and a cement-like, troweled-on surfacing material, expected to be impacted by the upcoming elevator project contained asbestos. A summary of all findings can be found in enclosure (1). A log of photographs of the sample locations is included in enclosure (2). The chain of custody is enclosure (3), and the laboratory results are in enclosure (4).

3. Methodology.

a. Arkansas Regulation 21, Arkansas Asbestos Abatement Regulation, and the EPA

Asbestos standards, 40 CFR 763, were followed.

b. Prior to sampling, a visual survey was performed to determine where, if any, homogeneous groups of materials existed. A sampling plan was then devised, and homogeneous materials were identified. For each homogeneous area, a minimum of three distinct samples were taken. For small patches (less than 6 linear feet) of insulation, one to two samples were taken.

c. For the pipe insulation, bulk samples were taken by wetting the material first and then using tools to remove all layers of the insulation from a small section (usually around one square inch). The holes created during the sampling process were covered with duct tape.

d. The asbestos samples were double bagged and shipped with a chain of custody to an

American Industrial Hygiene Association (AIHA) Accredited and National Voluntary

Laboratory Accreditation Program (NVLAP) laboratory for analysis. Analysis of the asbestos samples was performed using Polarized Light Microscopy in accordance with the Environmental

Protection Agency (EPA) method 600/R-93/116.

4. Findings. Asbestos was found in the insulation on the drain pipe in the attic space above the office spaces (in the area where the elevator is anticipated to be installed). The HVAC system insulation and the troweled-on surfacing material do not contain asbestos.

5. Under the Occupational Safety and Health Administration (OSHA) standard for asbestos in the work environment (29 CFR 1910.1001), information from this report should be used to update the Asbestos Management Plan and the Hazard Communication Program. Likewise, employees should be informed of where asbestos was located, and labels or signs should be installed so that employees are notified of what materials contain asbestos. Signs may be posted

CESWL-SO

SUBJECT: Limited Asbestos Survey at Norfork Powerhouse for the Elevator Project at the entrance to an area; however, employees need to be able to recognize what materials contain or are presumed to contain asbestos.

6. It is recommended that all work to remove asbestos containing material is performed by a

State of Arkansas licensed asbestos abatement company. Abatement work affecting the pipe insulation is considered Class I work under the OSHA Asbestos in Construction Standard (29

CFR 1926.1101), a standard that applies when there is repair, maintenance, or removal of materials that contain greater than 1% asbestos. If there are any questions as to whether they are performing the work in accordance with federal and state regulations, please let me know.

7. Thank you for allowing me to inspect your powerhouse for asbestos prior to the elevator project. Please note that this is a limited survey and that contractors may want to take their own samples prior to working in the powerhouse. If you have any questions, please call me at 501-

324-5617 or email me at laura.h.weems@usace.army.mil.

DISCLAIMER NOTES

· "ND" indicates no asbestos was detected; the method detection limit is 1%.

· "Trace" or "<" indicates asbestos was identified in the sample, but the concentration is less than the method quantitation limit. PLM coefficients of variance range from approximately 1.8 at the quantitation limit of 1% to 0.1 at high fiber concentrations.

· Samples are archived for three months following analysis and are then properly discarded.

· These results are submitted pursuant to RJ Lee Group's current terms and conditions of sale, including the company's standard warranty and limitation of liability provisions. No responsibility or liability is assumed for the manner in which these results are used or interpreted.

· This test report relates to the items tested.

· This report is not valid unless it bears the name of a NVLAP Lab Code 101208-0 approved signatory.

· Any reproduction of this document must be in full in order for the report to be valid.

· This report may not be used to claim product endorsement by NVLAP Lab Code 101208-0, any agency of the U.S. Government or any other laboratory accrediting agency.

· Polarized-light microscopy is not consistently reliable in detecting asbestos in floor coverings and similar nonfriable organically bound materials. Quantitative transmission electron microscopy is currently the only method that can be used to determine if this material can be considered or treated as "non-asbestos-containing."

· Sample(s) for this project were analyzed at our: Monroeville, PA (AIHA #100364, NY ELAP #10884) facility.

· If RJ Lee Group, Inc. did not collect the samples analyzed, the verifiability of the laboratorys results are limited to the reported values.

· ((((100-A)/B)*C*100)+0.99)/100 = Asbestos Detected (%), where A=weight loss, B=total # of points counted, and C=total # of asbestos fibers counted.
