PART 1 GENERAL

1.1 SUMMARY OF WORK

1.1.1 Project Description

This Statement of Work (SOW) defines the effort required for the modernization of the passenger elevator in Building 40-1., Watervliet Arsenal. The existing elevator system has a Montgomery Elevator Miprom microprocessor logic controller with a “dry” power unit installed in 1984. The elevator is designed for general passenger usage and is rated for 3500# at a nominal 110 fpm.

The controller is original to this installation and is provided with Phase I and Phase II fire service operation.

Modules and smoke detectors are currently located in the machine room and the elevator lobbies.

1.1.2 Location of Work

a. The work is located at 40-1, approximately as indicated. The exact location will be shown by the Contracting Officer.

1.1.3 Period of Performance

a. The contractor shall be required to (i) commence work under this contract within 7 calendar days after the date the contractor receives notice to proceed, (ii) prosecute the work diligently, and (iii) complete the entire work ready for use not later than 120 calendar days after notice to proceed was given. The time stated for completion shall include final cleanup of the premises.

1.1.4 Hours of Operation

a. Normal duty hours are 7:00 AM through 4:00 PM, Monday through Thursday, and 7:00 AM through 3:00 PM on select Fridays (the arsenal is closed every-other Friday). Federal holidays are non-work days. If a federal holiday falls on a Saturday or Sunday, the preceding Friday or following

Monday is a work holiday.

1.2 SUBMITTALS

1.2.1 General Submittal Requirements

a. The contractor shall provide submittal documents via e-mail to the Contracting Officer for review and approval. Provide all physical (non-document) submittals (including but not limited to material swatches, samples, etc.) directly to the Contracting Officer for review and approval. All submittals require government approval unless otherwise explicitly stated.

b. The contractor shall reference APPENDIX A “Consolidated List of Submittals” for the complete list of required submittals.

1.2.2 Contractor Safety Plan

a. The contractor shall submit a safety plan which is specific to the work activity being performed under the scope of this project for Government review and approval.

1.2.3 Lockout / Tagout (LOTO) Acknowledgement

a. The Contractor shall sign and submit a “Lockout/Tagout (LOTO) Notification Acknowledgement

Record” prior to working on or de-energizing any electrical equipment.

1.2.4 Asbestos Notification Acknowledgement Record

a. The contractor shall sign the “Asbestos Notification Acknowledgement Record” and submit this form to the Project COR prior to starting project work.

1.2.5 Project Schedule

a. The contractor shall create a project schedule and submit for Government review and approval.

b. The project schedule shall detail the phases of work and the overall timeline for the execution of the work. The contractor shall include delivery dates, appropriate milestones, and shall clearly detail any requirements for work performed outside of regular hours of operation.

1.3 GENERAL REQUIREMENTS

1.3.1 Watervliet Arsenal Information for Contractors (WAIC)

a. The contractor shall perform all work in strict conformance with Watervliet Arsenal Instructions for Contractors (WAIC). The contractor shall read and understand this document prior to submitting a proposal. The WAIC is publically available at:

1.3.2 Utility Outage Requests and Utility Connection Requests

a. Schedule work to minimize outages. For utility outages and connections required during the execution of work that affect existing systems, coordinate schedule with COR. Schedule utility outages and connections to minimize disruptions to the Government. No additional payment will be provided for utility outages and connections required to be performed outside the regular work hours.

b. The contractor shall notify the Contracting Specialist a minimum of two (2) business days before the contractor starts work. The contractor shall notify the COR a minimum of seven (7) business days before the shut-down of a utility or building service.

1.3.3 Safety Requirements

a. The contractor is required to provide the proper control of hazardous energy (lockout/tagout) per

OSHA 29 CFR 1910.147.

b. The contractor shall inspect the project work site prior to starting work, and shall request a copy of the asbestos building inspection records to verify the location and description of known asbestos-containing materials.

1.4 ON-SITE PERMITS

1.4.1 Excavation (Dig Permit)

a. The clearance of buried utilities will be performed by the Government in advance of the Contractor performing subsurface work, unless otherwise explicitly stated.

b. The Contractor shall obtain a copy of the completed Utility Clearance Permit (WVA Form 420-2-E) from the COR prior to performing subsurface work. Verify existing utility locations indicated on contract drawings, within area of work.

1.4.2 Hot Work and Confined Space

a. Obtain permit for welding, burning, use of flame, or entry into confined space from WVA Fire

Department by calling (518)266-5990 each day the permit is to be used.

1.6 CONTRACTOR EQUIPMENT

1.6.1 Equipment Identification

a. The contractor shall create and affix a label to all large equipment, dumpsters, and other mobilized property, which is weather-resistant (laminated, or within a see-thru label hanger), securely affixed to the equipment or visible through a window, and which clearly displays the General Contractor’s:

i. Company Name, POC Name (first, last), POC phone number, Project Number (7PW034).

1.6.2 Waste Dumpster

a. The contractor shall place any waste dumpsters in a manner that does not interfere with thru-traffic.

b. Place waste dumpsters with a minimum of 33’ clearance from any building, unless otherwise approved. The final location shall be coordinated with, and approved by the COR before any dumpsters are mobilized.

c. Cover all waste dumpsters when not in use, and before the end of each work shift. The cover shall be weather-tight.

d. Note: The Contractor shall be required to retain and submit copies of all Bills of Lading and Weight Slips associated with the disposal of construction and demolition C&D) debris.

PART 2 PRODUCTS

2.1 GENERAL

All work shall be in strict accordance with product manufacturer’s instructions and specifications, including but not limited to manufacturer guidance for storage, staging, application, installation, and disposal of the product.

2.1.1 Hazardous Materials

a. The contractor shall submit Safety Data Sheets (SDS) for all hazardous materials to be used.

2.1.2 Asbestos, Mercury, Lead, PCB

a. Unless otherwise stated, all materials for this project shall be new. All products shall not contain any asbestos, mercury, chromium/lead paint, Class I Freon, and Polychlorinated Biphenyls (PCB), and shall state such on the manufacturer’s original packaging.

2.1.3 DoD Green Procurement Program

a. All construction materials shall meet the applicable requirements of the DoD Green Procurement

Program, specifically recycled content products, environmentally preferable products and services, bio-based products, energy-and water-efficient products, alternate fuel vehicles and alternative fuels, products using renewable energy, and alternatives to hazardous or toxic chemicals as found at <http://www.acq.osd.mil/dpap/cpic/cp/docs/USA001967-08-DPAP.pdf>.

2.2 PRODUCTS

2.2.1 New Hydraulic Elevator

a. Pre-Engineered 3500# holed hydraulic passenger elevator; ADA Compliant.

2.2.2 Ancillary Items

a. GFCI protected convenience outlets

b. Sump pump

c. Smoke Detectors

d. Emergency hands-free telephone

PART 3 EXECUTION

3.1 ITEMS OF WORK

3.2 The contractor shall replace solid state elevator controller with new non-proprietary system. The control manufacturer must provide to any service provider: Technical Support, engineering guidance, recommended spare parts list, field training on installation, trouble shooting and continuing maintenance.

3.2.1 The new control shall be installed under the ANSI A17.1 Part 8 providing hoistway access operation, to include, emergency battery lowering.

3.3 The contractor shall prepare new controls for future shunt trip and emergency power operation and shall provide three copies of wiring diagrams, backup software on CD, user and installation manuals, 1 hard copy 1 electronic copy. Electronic copy of all shall be emailed to Government when project is complete. Note:

Montgomery Elevator was purchased by Kone Elevator in 1984 and therefore the existing system components are no longer manufactured and have only limited availability.

3.4 The contractor shall replace the existing power unit in its entirety and provide a new tank, oil control valve and submersible motor. The contractor shall also install a new tank on new isolation pads, and provide new hydraulic oil. The old oil shall be disposed of per Government regulations.

3.5 The contractor shall replace all hydraulic lines with new lines and install new lockable shut off valves in the pit and machine room. The contractor shall insure there are no leaks when complete.

3.6 The contractor shall install all new hoistway door interlocks and door release devices, servicing all the remaining components. All remaining components shall be thoroughly cleaned and lubricated.

3.7 The contractor shall install new car door operator device, complete with header, hanger, and linkages and clutch.

3.8 The contractor shall furnish and install new car doors with stainless steel finish.

3.9 The contractor shall install and furnish all new interior car vertical panels. The contractor shall submit all materials and colors to the Government prior to purchase or install.

3.10 The contractor shall replace the existing car sill with all new nickel silver sill.

3.11 The contractor shall remove the existing car drop ceiling and light fixtures and replace with island type ceiling having six panels and no less than 6 LED lights. The illumination levels shall meet current ANSI code standards. The downward facing portion of the ceiling shall be brushed stainless steel. The ceiling shall have clear and unobstructed access to the emergency hatch.

3.12 The contractor shall install a new car top inspection station with all required devices.

3.13 The contractor shall replace top and bottom roller guide rollers.

3.14 The contractor shall replace the in-car operating panel with a new panel. The contractor shall provide security lock-out feature and all devices required to meet or exceed ADA requirements. The contractor shall include emergency lighting, digital position indicator and hands free auto dial phone.

3.15 The contractor shall remove the existing car flooring/sub floor and install new plywood subfloor as needed.

A rubberized car flooring with new cove base shall then be installed. The contractor shall submit all colors to the Government prior to purchase or installation of materials. The new flooring shall be flush with the existing car sill.

3.16 The contractor shall add an emergency escape hatch to the elevator car top and it shall be wired into the elevator safety control circuits.

3.17 The contractor shall replace all hallway fixtures with new surface mounted fixtures. The new fixtures shall be stainless to match the new doors and shall have stainless steel cover plates. The buttons shall be vandal resistant type with LED indicator lamps. Each hall shall have 1-inch digital display floor indicator installed.

3.18 The contractor shall replace the in-car travel lantern with new LED style system and shall modify the return wall as needed to accept the new backing box. The new fixture is to be flush with the stainless steel cover.

3.19 The contractor shall install all new wiring throughout the installation with the exception of the main power supply leading into the machine room. When installing new traveling cables, there shall be a minimum of 10% spares. The contractor shall include Coax and 3 extra twisted pairs for future security camera use.

3.20 The contractor shall reuse conduit and replace where needed.

3.21 The contractor shall include new main line feeders from the means of the disconnect to the new elevator controller.

3.22 The contractor shall replace all hydraulic packing seals and head bearings and shall install scavenger pump in the pit with the drain line back to OEM tank. The system shall be pressure tested per code when the installation is complete.

3.23 The contractor shall cover all combustible hoistway materials and provide fire stopping on all hoistway penetrations.

3.24 The contractor shall install a new emergency on/off pit switch.

3.25 The contractor shall provide all required signage and data plates as needed.

3.26 The contractor shall clean the elevator guide rails, leaving the roller surfaces free from dirt and rust.

3.27 The contractor shall HEPA vacuum the elevator shaft and pit.

3.28 The contractor shall paint all exposed pit steel with rust prevention paint.

3.29 The contractor shall lubricate the elevator per OEM recommendations.

3.30 The contractor shall adjust all retained components/devices to ensure that they operate within existing elevator code guidelines.

3.31 The contractor shall test all devices prior to the final inspection.

5. General Construction

6. CLOSEOUT REQUIREMENTS

6.1 Construction and Demolition Debris

a. The Contractor shall submit copies of all Bills of Lading and Weight Slips associated with the disposal of construction and demolition (C&D) debris under this scope of work. Submit documentation to project COR.

6.2 Cleanup of Work Site

b. The contactor shall clean all work areas and remove all tools, equipment, debris, materials, and all waste from the premises.

c. The contractor shall remove all debris from any remote holding locations, to include any excavation and concrete equipment chute cleanout debris.

d. The contractor shall remove their equipment from Watervliet Arsenal premises within five (5) working days after completion of project work.

6.3 Restoration of Work Site

e. The contractor shall repair any items, building surfaces, equipment or miscellaneous items which were damaged during the execution of this SOW. Items that cannot be repaired to restore original function or aesthetics shall be replaced in kind by the Contractor at no additional charge to the Government.

f. Grass, trees, and other vegetation affected by project work shall be restored to same or better condition as existed prior to the start of project work. The Contractor shall prepare the affected areas to eliminate uneven areas and low spots. Remove foreign materials, weeds, and damaged soil. The Contractor shall scratch the area to a depth of three (3) inches where topsoil is to be placed. Spread topsoil to a uniform, compacted depth of less than five (5) inches over areas to be reseeded. Rake until smooth. The Contractor shall seed, fertilize, and water the area to establish grass.

6.4 Warranty

g. The contractor shall provide a [1] year warranty on the following equipment/systems.

i. Workmanship and Construction.

ii. Elevator and all components related to.

6.5 As-Built Drawings

h. The contractor shall provide as-built drawings for the following.

-Elevator install, to include all electrical and systems.

6.6 Operation and Maintenance Manuals

i. The contractor shall prepare new controls for future shunt trip and emergency power operation and shall provide three copies of wiring diagrams, backup software on CD, user and installation manuals, 1 hard copy 1 electronic copy. Electronic copy of all shall be emailed to Government when project is complete

ii. All elevator related components.

6.7 Training

i. The contractor shall provide [4] hours of on-site training for up to ten (10) government employees for the following equipment/systems.

i. Elevator electronics and basic maintenance.

APPENDIX B: Quality Assurance Surveillance Plan (QASP)

PURPOSE

This Quality Assurance Surveillance Plan (QASP) will be used as a guide to Government Quality Assurance Personnel (QAP). It provides a systematic method to evaluate the services the contractor is required to furnish.

SURVEILLANCE PROCEDURES

Timeliness: Progress is maintained to assure compliance with completion time.

Quality: Conformance with statement of work and industry standards.

Tasks: Performed according to statement of work requirements.

Problem Resolution: Contractor submits deliverables in accordance with the contract and resolves any deficiencies identified by the government in the final submission. Following final submission, contractor resolves any additional deficiencies identified by the government and resubmits and or corrects within one week.

Delivery: Contractor submits each deliverable in accordance with the statement of work and terms of the contract.

PERFORMANCE AREAS

Management Responsiveness: Evaluate the adequacy of the contractor’s responsiveness and their ability to keep the government informed of work progress and provides for early problem identification and effective corrective action plans.

Schedule Control: Evaluate the contractor’s adherence to completion schedule.

Performance: The contractor will be evaluated as to the workmanship and adherence to the statement of work.

EVALUATION CRITERIA

(Outstanding). Indicates performance clearly exceeds contractual requirements. The area of evaluation contains few minor problems for which corrective actions appear highly effective.

(Satisfactory). Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.

(Unsatisfactory). Indicates the contractor failed to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions were ineffective.

DOCUMENTATION

Documentation used to perform surveillance will consist of random government inspections and inspection records, as well as the evaluation of final work product delivery by the project COR/COTR.

RATING

Contractor will be given an overall rating of Outstanding, Satisfactory, or Unsatisfactory by the Government Technical point of contact or Contracting Specialist Representative, if appointed, 3 using the above evaluation criteria.
