SUMMARY OF CHANGES

The required response date/time has changed from 19-Dec-2016 10:00 AM to 28-Dec-2016 02:00 PM.

ADDENDUM TO 52.212-1

1. General:

The solicitation will be announced through Federal Business Opportunities (FedBizOpps.Gov) at https://www.fbo.gov website.

Whenever, the words “quote,” “offer,” “proposal,” “offerors,” or similar terms are used in this solicitation, they shall be read to mean proposal or similar corresponding term to reflect that this solicitation is a request for proposal

(RFP).

System for Award Management (SAM). In order to receive contract award, offeror must be registered/certified in SAM prior to award, have no active exclusions, and completed on-line representation and certifications. SAM can be accessed at: https://www.sam.gov/portal/public/SAM/.

Once the KO has decided to whom an award will be made, an unsigned completed contract (SF1449 form) will be sent to the company, for signature and return. A contract will be in existence once both the offeror and the KO have signed the document.

2. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the offeror’s proposal.

3. Proposal shall consist of the following items:

a. Signed copy of Solicitation/Contract/Order for Commercial Items (SF 1449) and signed copy of all Amendments (SF 30) to the solicitation, if any.

e. Evaluation Factors - Technical Offer, Past Performance, Compensation Plan, and Pricing Requirements.

Evaluation Factors are located in section titled: Evaluation - Commercial Items, and will be used to evaluate offers based on the written information submitted and shall address the following:

Factor 1. Technical capability to meet the Government’s Requirement:

a. The term “technical,” as used herein, refers to non-price factors other than past performance. More than one “technical” factor can be used and titled to match the specific evaluation criteria appropriate for the RFP. The purpose of the technical factor is to assess whether the offer will satisfy the Government’s minimum requirements.

b. The technical factor limited to ten pages, each side of a sheet of paper constitutes one page, anything in excess of the ten page limit will not be evaluated (this does not include the cover page). The technical proposal shall address the following four items

Confirm that the candidates (LPNs) presented will meet or exceed the performance requirements in paragraph 1.2. of the PWS.

Recruitment Plan. Submit in detail your plan to recruit and retain employee(s), with minimum turnover, including a backup staffing plan.

Submit a detailed plan to provide adequate contingency arrangements in the case of unscheduled absences.

The Contractor shall have adequate pre-credentialed/pre-privileged replacement HCPs available in such instances so a lapse in coverage does not occur.

Your plan under 252.237-7024 - Notice of Continuation of Essential Contractor Services, all areas must be addressed in order to be technically acceptable.

Factor 2. Past Performance (Attachment 2 – Past Performance Information Collection Sheet). **To be returned by references directly.

a. Provide a minimum of THREE references (i.e., name, title, telephone phone number, and project title/description) who can verify that YOUR company have provided LPN Services of similar scope and complexity as required herein. "Recent" is defined as "within the past three years.” The Past Performance Information Collection Sheets must be returned by your company’s references directly. Past Performance references may be contacted. The Government may also consider any additional past performance information that is available.

Offerors are reminded that, while the Government may elect to consider data obtained from other sources, the burden of providing thorough and complete performance information rests with the Offeror. Each Past Performance Collection Sheet submitted in response to this requirement must be for the required services being contracted for herein. Do not submit past performance references for services other than what is being contracted for in this solicitation.

b. The past performance assessment will be evaluated using Attachment 2 - Past Performance Information Collection Sheet information gleaned from the references provided by the Offeror, as well as any other information available to the Government to include, but not limited to the Past Performance Information Retrieval System (PPIRS) and Federal Awardee Performance and Integrity Information System (FAPIIS).

Factor 3. Compensation Plan. Provide a completed compensation plan for the LPN Services being contracted for herein, using Attachment 3 - Compensation Plan for Employees Under Wage Determination - Base Plus Option Periods. The Offeror’s compensation plan, must present enough detail to show that the company will provide compensation levels that reflect a clear understanding of the work to be performed and indicates the capability of the proposed compensation structure to obtain and keep suitably qualified personnel to meet mission objectives. The total compensation for employees under a wage determination has to meet or exceed the wage determination in order to be determined minimally acceptable, However, just meeting the wage determination may not necessarily be enough to be determined adequate to attract and retain LPNs in the Tacoma, WA area. Additionally, the Government shall evaluate the compensation plan’s likelihood to recruit and retain the necessary personnel at Madigan Army Medical Center (MAMC), Tacoma, WA.

Factor 4: Price. Price will not be rated. Submit pricing for all Contract Line Item Numbers (CLINS) to include option years. Provide total pricing for the base and each option year, and total for entire offer.

Price will be evaluated using various price analysis techniques and the following:

Total Price: The total price is the sum of the prices for all the Contract Line Items (CLINs) of this solicitation. The Government will evaluate the offer for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

This solicitation will be evaluated using the methodology of Lowest Priced Technically Acceptable.

4. Submission of Proposal documents: Submit to the attention of Larry Bradbury. Quotes may be submitted in PDF or Word format and shall be submitted via email to larry.a.bradbury2.civ@mail.mil.

5. Questions Concerning Solicitation: Direct questions regarding this solicitation to Larry Bradbury at larry.a.bradbury2.civ@mail.mil. All questions must be received no later than seven calendar days prior to the closing date established in SF 1449 block 8 (as amended). The Government reserves the right to not answer questions not meeting this timeline if doing so would result in an amendment requiring an extension of the solicitation and to extend would not be in the best interest of the Government.
