About this file

The document is a standard Internal Revenue Service (IRS) Form W-9 (Request for Taxpayer Identification Number and Certification) dated December 2014. This form is typically used by businesses and entities to provide their taxpayer identification information to entities that will be reporting payments to the IRS. The form allows the taxpayer to provide their Social Security Number or Employer Identification Number, specify their federal tax classification, and certify their tax reporting status.

The W-9 form is a standard tax documentation instrument that requires the taxpayer to provide key identifying information, including name, business name (if different), federal tax classification (such as individual, corporation, partnership), and taxpayer identification number. The form includes certification statements where the taxpayer affirms the accuracy of their tax identification number, confirms their U.S. person status, and certifies their exemption status for backup withholding and FATCA reporting. This particular form is part of a potential documentation package for the Oakland Community Health Network's financial audit services contract in Michigan, serving as a standard tax reporting document for potential contractors.
