PROJECT FORMS
PART 1 - GENERAL
1.1 SCOPE
A. This Section lists specific, KC VAMC prepared or approved forms that are required to be used by the Contractor during construction of the Project.
B. Before starting work on-site, obtain the most recent version of the following forms from the
Government Project Manager (GPM).
C. Copy and fill-in all required values indicated as applicable to the Work of the Project, and submit to the GPM.
1.2 KC VAMC ADMINISTRATIVE FORMS:
A. SCHEDULE OF VALUES – PROGRESS PAYMENT: For the Contractors submittal of material and associated labor quantities and costs for project management and Progress Payment justification and accounting.
B. PROJECT MASTER SCHEDULE: For the Contractors submittal of project timeline with milestones and phasing to manage project execution over time.
C. CONTRACTOR DAILY LOG: For the Contractors submittal daily of weather conditions, work underway, trades and staff on site, and project status.
D. AUTOMATIC PAYMENT FORM: For the Contractor to enroll in the Automatic Clearing House
(ACH) system for the purpose of submitting Progress Payment Requests, Invoices, in order to receive payment of same.
E. CERTIFIED PAYROLL FORM: For Contractor to submit Certified Payroll information to the GPM as required by law, for all Contractors employees, Sub-contractors employees, and all others working under this contract not employed by the Government.
F. CONTRACTOR COST PROPOSAL: For Contractor to submit cost proposals in response to
Government Request for Price (RFP).
G. RELEASE of CLAIM: For Contractor to submit with final payment request.
H. STORED MATERIAL TRACKING FORM: For Contractor to submit and track material stored on and off the job site.
I. BACKGROUND DATA COLLECTION FORM: For the Contractor’s submittal of proposed Workers intended to be working on KC VAMC property.

J. HOT WORK PERMIT: For the Contractor’s submittal of proposed dates and time for any Work involving use of heat or a flame (including brazing of copper pipes), to include proposed means and methods to be used to mitigate risks and to eliminate fire-system alarms caused by that
Work.
K. INTERIM LIFE SAFETY MEASURES (ILSM) FORM: A document for initial submittal, that includes an assessment of risks and description of deficiencies that will be caused or affected by construction activities.
L. INFECTION CONTROL RISK ASSESSMENT (ICRA) FORM: A document for initial submittal, describing risks and mitigation measures to be used during construction in regards to infection control.
M. ILSM & ICRA DAILY MONITORING FORM: For daily submittal of life-safety and infectious disease control measures being maintained by the Contractor during construction activities.
N. NEW EQUIPMENT FORM: A concise form to list the description, type, model number, serial number, utility/service connections, operational loads, and other salient characteristics of all new “equipment” provided by the Contractor as a part of the Work of the Project. Equipment includes, but is not necessarily limited to operational items with a service life, such as door or gate operators, lifts, elevators, fire-suppression systems and devices, HVAC equipment (not ductwork), plumbing and electrical equipment (not piping or conduit), and light fixtures.
O. OUTAGE ACTION PLAN: A document for submittal by the Contractor for each existing utility service disconnection or outage, describing measure planned to be undertaken to minimize risks and disturbance of existing utility service conditions.
P. PAST PERFORMANCE QUESTIONNAIRE: The form for reporting five (5) past projects of the same class (scope & complexity) as this project.
Q. PLANNED UTILITY SYSTEM SHUTDOWN REQUEST FORM: The “request” form to be submitted by the Contractor for each existing utility service proposed for disconnection or outage, to include the “Outage Action Plan” above, and to include specific dates / times proposed for the affected Work.

PART 1 - GENERAL
This project covers the acquisition of equipment, component parts, material, and the provision of labor, and supervision for installation of equipment and component parts to affect a complete replacement and upgrade of the control systems for Central Boiler Plant boilers # 1, # 2, # 3, and # 4, located in
Building 4.
1.1 PROJECT IDENTIFICATION:

Replace Central Boiler Plant Control System, at the existing
Kansas City Veterans Hospital – Boiler Plant Building 4
4801 Linwood Boulevard
Kansas City, MO 64128
PROJECT SUMMARY DESCRIPTION:
This project will replace Controls for the Central Boiler Plant to include but not limited to:
1. Perform combustion control upgrade to #1, #2, #3 and #4 duel fuel boilers to meet VA specs, VA Directive 1810, and VA Safety Device Testing manual current edition, to include parallel positioning operation on #2 and #3 boilers.
2. Perform combustion testing on all boilers for both fuels.
3. Perform upgrade to #2 and #3 boiler water level controls, alarms and trips to meet requirements of VA Safety Device Testing manual third edition.
4. Repair or install Forced draft blower Current relays as outlined in the VA Safety Device
Testing manual third edition.
5. Repair and test Maxon automatic gas valves immediately prior to the burner on #1 and #4 boilers per VA Safety Device Testing manual third edition.
6. Repair and calibrate the Low Flue Gas Oxygen level alarm and shut-down interlock on all four boilers as per VA Safety Device Testing manual third edition.
7. Upgrade, calibrate, and test Condensate tank level controls and alarms as per VA Safety
Device Testing manual current edition.
8. Upgrade the existing SCADA system
9. Upgrade Boiler Plant Master Control panel.
10. Provide 3 full size (30” X 42”) hard copies, one set will be laminated and one electronic copy in dwg & pdf of as-built shop drawings for all modifications/upgrades.
11. Provide 3 hard copy sets and 1 electronic copy in pdf of manuals for all modified/upgraded devices and systems.
12. Provide training for Boiler Plant personnel on all modifications/upgrades
13. Provide 7 hard copies and one electronic copy in pdf of all training materials
14. Provide system diagnostic program with system backup on a laptop connectable to each boiler for uploading program in the event of corruption or loss of operating system.

PRE-BID SITE VISIT: One (1) pre-bid site visit will be conducted by the VA. The date and time will be indicated within the solicitation. Contractors are highly recommended to attend in order to gain knowledge of existing field conditions, as no changes to the Firm Fixed Price or Time will be approved because of lack of familiarity with the existing, visible, and or stated conditions.
CONTRACTING OFFICER’S REPRESENTATIVE (COR): The Contracting Officer will appoint a qualified individual to act as the “Contracting Officer’s Technical Representative”, hereinafter referred to as the
“Contracting Officer’s Representative” (COR).
GOVERNMENT PROJECT MANAGER (GPM): The Medical Center will appoint a qualified individual to act on behalf of the Medical Center for the execution of project 589-17-101, Replace Central Boiler Plant
Control System. This person may also be certified as the COR.
1. After award and prior to receipt of a Notice to Proceed, visits to the site must be made only by appointment with the GPM.
2. Note that the Contracting Officer is the only entity authorized to change the Contract Cost or Contract Time, or to make any changes to the requirements of the Contract Documents.
There is no “implied authority” granted to any other Medical Center personnel (including doctors or hospital management).
1.2 RELATED DOCUMENTS:
A. AGREEMENT & CONTRACT CONDITIONS: The Work of this Project will be issued either as:
1. As a Firm-Fixed Price (FFP) “Task Order” per requirements of the Contractor’s existing
Multiple-Award Task Order Contract for Indefinite Delivery & Indefinite Quantity (MATOC
IDIQ), or
2. As a separate, stand-alone, Contract for construction, as indicated within the Government’s solicitation. The solicitation for a separate, stand-alone Contract will include FAR references and other Contract Conditions equivalent to requirements of a MATOC IDIQ agreement, as determined by the Contracting Officer.
3. Within this and other Specification Sections, and within the Drawings, reference to either contracting method will be stated simply as “the Contract”.
4. Being issued as a part of the solicitation, requirements of this Specification Section and other Division-01 Specification Sections are to supplement and in some cases, modify the requirements of the Contract.
B. ADMINISTRATION OF THE CONTRACT: The Contract will be issued and administered through The
Department of Veterans Affairs - Veterans Integrated Service Network 15 (VISN 15), 4101 South
4th Street, Leavenworth, KS 66048.

C. STATEMENT of LIMITATION of AUTHORITY:
You are hereby notified; The GPM/COR or other local Government personnel DOES NOT h a v e the authority to direct you in any way to alter your contractual obligation. The
Contracting Officer of record for this project holds sole authority for contract changes.
Further, if the Government, as a result of the information obtained from discussion DOES desire to alter your requirements, changes will be issued in writing and signed by the
Contracting Officer. You should take no action on any change unless and until you receive such a contract modification.
D. SAFETY, QUALITY CONTROL, AND INFECTION CONTROL PLAN: Per requirements of the
Government, the Contractor will prepare a “Master Safety, Quality Control, and Infection
Control Plan” for this project to the GPM for review and record within fifteen (15) days of award. Make the “Safety, Quality Control, and Infection Control Plan” available for review and reference at all times at the Project Site, and comply with requirements therein.
E. All equipment, methods, documentation, testing will conform to the current as provided Master
Construction Specifications, all applicable divisions and sections.
F. As built drawing of the existing conditions will be provided to all bidders.
1.3 STATEMENT OF BID ITEM(S):
FOR ALL BID ITEMS:
THE SUCCESSFUL BIDDER WILL BE REQUIRED TO PRODUCE SHOP DRAWINGS OF THEIR INTENT TO
SATISFY THE REQUIREMENTS OF THIS PROJECT AND SUBMIT THEM TO THE GPM AND CO FOR REVIEW
AND APPROVAL PRIOR TO THE COMMENCEMENT OF WORK. AS-BUILT DRAWINGS OF THE FINAL
WORK COMPLETED IS A REQUIREMENT OF ALL BID ITEMS.
A. BID ITEM 1 – BASE BID: Briefly and without force and effect upon the Contract Documents, the
Work of this Project is summarized to include the following, as indicated by this document, Specifications and Drawings:
1. BOILER 1: Hurst FTB 12000 PPH
The scope is to replace existing controls. We are proposing an Allen Bradley Compact Logix based control system or approved equal. The system will handle all air flow / fuel flow processes, startup / shut down sequences, alarm functions, feed water flow, furnace pressure, oxygen trim, drum pressure control and required data for operator interface. The system will be a dual fuel, fully metered and cross limited, using positioner type fuel flow control valves, FD fan inlet damper actuator & FD fan VFD. System will be part of a plant wide control system reporting back to a remote operator interface in the control room.
a. Utilize existing main control panel, modifications to include;

1) Removal of existing controls / devices
2) Installation of a new “blank plate’ to cover existing cutouts
3) New operators / touch screen to be installed in surface of panel facing control room
4) A prefabricated sub panel with installed controls (built off site) will be installed in existing panel
5) New panel lighting system
6) New Panel Air conditioner
b. Relocation of BMS to existing panel
c. Existing burner mounted control panel will be stripped and removed from burner
d. Natural Gas Train modifications
1) A new gas flow meter / piping run will be installed
2) High & Low Gas pressure switches / test ports / pressure gauges
3) New natural gas flow control valve will be installed
e. Oil Train modifications
1) A new oil flow meter / filter will be installed
2) New shut off valves / test ports will be installed
3) New high and low pressure switches / pressure gauges will be installed.
4) New fuel oil flow control valve will be installed

1.5 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR:
A. Electronic Media Formatted (EMF) Contract Documents (including the Drawings and
Specifications) may be obtained from the website where the solicitation for this Contract was posted.
1. The Contactor must provide each subcontractor with a complete set of Contract Documents, and verify their receipt upon request by the COR.
2. Printing of Contract Documents will be at the Contractor's expense.
1.6 PAPERLESS CONTRACTING & ELECTRONIC DOCUMENT MANAGEMENT:
A. ELECTRONIC DOCUMENTS: Notwithstanding requirements to the contrary within the Contract
Documents, and except for printed sets of Operations & Maintenance Manual’s at the Project’s completion, submittal of Documents related to construction of this Project must be made in
Electronic Media Format (EMF) - typically in PDF format unless otherwise approved by the COR.
1. Certain submittals, such as those for employment eligibility, construction company disclosures, and payment processing must be made through specific, secure on-line systems as directed by the VA.
2. Documents related to management of the construction Work, including but not limited to: start-up submittals, insurance certificates, Bonds, Construction Schedule and updates, Incident
Reports, Product Data, Shop Drawings, coordination drawings, daily reports, construction progress meeting minutes, Outage Action Plans, Utility system shutdown requests, hot-work permits, inspection reports, and close-out document submittals.
B. CPMS SYSTEM: The Contractor must utilize Construction Project Management Software (CPMS) system as directed by the VA. When approved by the COR, an equivalent CPMS system as typically used by the Contractor may be used, upon verification and approval by the VA of its security measures.
1.7 CONSTRUCTION SECURITY REQUIREMENTS:
A. GENERAL: The Contractor, all subcontractors, and employees of all are subject to both federally mandated and local facility security requirements, including but not limited to the following:
1. Federal Identity Verification of Personnel, and furnishing identification and contact information for all workers on-site.
2. Federal Security Requirements for Unclassified Information Technology Resources.
3. Local facility security requirements, including compliance with local security management programs and coordination with the on-site VA police department.
B. CONTRACTOR’S SECURITY PLAN:
1. Prepare a Security Plan specific to this Project, to include both physical and administrative procedures that are to remain effective throughout the Project’s duration.

2. Include requirements for electronic security, describing the means and methods to maintaining the confidentiality of “sensitive information”.
3. Submit the Security Plan to the GPM for review and comment no less than fourteen (14) calendar days prior to start of Work on-site.
4. The Contractor is responsible for assuring that all subcontractors working on the project and their employees comply with the Security Plan prepared by the Contractor.
C. FACILITY SECURITY REQUIREMENTS:
1. Provide a list of workers anticipated to be on-site to the GPM no less than fourteen (14) calendar days before starting Work, so that individual security investigation and preparation of personnel identification badges can be provided.
2. Workers may not enter the Project site without an appropriate personal identification badge.
3. Workers are subject to inspection of their personal effects when entering or leaving the project site.
4. No photography of VA staff or patients is permitted. No photography of existing, operational
VA premises is allowed without written permission of the Contracting Officer.
5. The VA reserves the right to shut down the Project site and order the Contractor’s employees off the premises in the event of a national emergency. The Contractor may return to the site only with the written approval of the Contracting Officer.
PART 4 - KEY CONTROL:
1. The Contractor shall provide duplicate keys, lock combinations to the GPM for the purpose of security inspections of every area of project including toolboxes and parked machines, and take any emergency action.
2. The Contractor shall turn over all permanent lock cylinders to the VA locksmith for permanent installation.
PART 5 - DOCUMENT CONTROL:
1. The Contractor is responsible for the secure safekeeping of all Contract Documents used on the Project site, including Drawings, Specifications, Shop Drawings and other site-specific project information. This information may be shared only with those having a specific need to accomplish the Work of the Project.
2. Certain documents, sketches, videos or photographs and drawings may be marked “Law
Enforcement Sensitive” or “Sensitive Unclassified”. Secure such printed information in separate containers and limit access to only those who will need it for the Project. Return printed information to the GPM upon request and at completion of the Project, and delete all EMF information provided.

a. These printed documents may not be removed or transmitted from the project site without the written approval of the Contracting Officer.
b. Notify the GPM and the Site Security Officer immediately when there is a loss or compromise of “sensitive information”.
c. “Sensitive information” may be attached to e-mail provided that all VA encryption procedures are followed.
3. All paper waste or electronic media such as CD’s and diskettes must be shredded and destroyed in a manner acceptable to the VA upon completion of the Project.
4. All EMF information must be stored in a specified location following VA standards and procedures.
a. Security, access and maintenance of all project drawings, both scanned and electronic must be performed and tracked through appropriate VA required systems.
PART 6 - MOTOR VEHICLE RESTRICTIONS: COMPLY WITH APPLICABLE REQUIREMENTS OF THE
CONTRACT, AND AS FOLLOWS:
1. Authorization for any vehicle entering the site must be requested no less than 24 hours before the anticipated time of arrival. Access is restricted to picking up and dropping off materials and supplies.
2. NO ON-SITE PARKING will be allowed during the normal VAMC facility business hours of:
7:00 AM to 4:00 PM - Monday through Fridays.
1.8 CONSTRUCTION OPERATIONS:
A. Confine operations (including storage of materials) on government premises to areas indicated in the contract documents, or as authorized by the COR, in accordance with requirements of the contract.
B. Temporary facilities, including temporary utilities and temporary buildings (e.g., storage sheds, shops, offices) may be required by the contract documents, in locations and in conditions of use as approved by the cor. Include all costs for temporary facilities, including public utility service fees and permits, when applicable. Temporary facilities will remain the property of the contractor and must be removed by the contractor at its expense upon completion of the work, unless written consent is received from the gpm for temporary facilities to remain in place.
C. Maintain existing utility services at all times. Provide temporary utilities as required to assure uninterrupted services. Comply with referenced requirements for “utility interruptions” within the contract.
1. Where necessary to cut existing water, steam, gases, sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems
(including telephone and data systems), cut and cap at suitable places where shown; or, in absence of such indication, where directed by the COR. Cutting and patching of existing utility services must be coordinated with the applicable Utility Company involved:

2. Remove abandoned utility service lines including wires, cables, conduits, ducts, pipes and similar system components unless indicated otherwise within the Contract Documents, or when specifically directed by the COR. Abandoned lines which are not required to be entirely removed must be sealed, capped or plugged at the main, branch or panel they originate from. The lines may not be capped in finished areas, but must be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or partitions; so, that they are completely behind the finished surfaces.
3. Whenever it is required that a connection fee be paid to a public or private utility provider for new permanent service to the construction Project, for such items as water, sewer, electricity, or gas, payment of such fee will be the responsibility of the Contractor - and not the Government.
1.9 CONSTRUCTION SCHEDULE AND WORK SEQUENCE:
A. CONTINUOUS OPERATIONS: The Medical Center must maintain its operations 24 hours a day, 7 days a week. Therefore, interruption of any existing service must be scheduled and coordinated with the GPM to ensure that no interruption in operation occurs. The Contractor must develop a work plan and Construction Schedule, detailing at a minimum, the procedures to be employed, the equipment and materials to be used, the interim life safety measure to be used during the
Work, and a schedule defining the duration of the work with milestone subtasks. The Work to be outlined must include, but not be limited to:
B. CONSTRUCTION SCHEDULE: To insure such executions, the Contractor must furnish the GPM with a schedule of approximate phasing and dates on which the Contractor intends to accomplish Work in each specific area of the site, building, or portion thereof, as applicable. In addition, the Contractor must notify the GPM fourteen (14) days in advance of the proposed date of starting Work in each specific area of the site, building, or portion thereof.
C. Government occupancy during construction: the existing building(s) may be occupied by the government during construction, but unless otherwise indicated, the immediate areas of construction work will be vacated.
D. ONGOING USE AND OCCUPANCY DURING CONSTRUCTION: Certain rooms or areas will remain occupied by Medical Center personnel for various periods of time, as listed below:

1. The Contractor must take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered.

2. The Contractor must permit access to Department of Veterans Affairs personnel through other construction areas which serve as routes of access to such affected areas and equipment. Isolate routes from the construction area by temporary partitions with walking surfaces, lighting etc. to facilitate staff access. Coordinate alteration work in areas occupied by Department of Veterans Affairs personnel so that Medical Center operations will continue during the construction period.
3. Immediate areas of alterations not indicated above will be temporarily vacated while alterations are performed.
E. When a building or portion thereof is turned over to the contractor, the contractor must accept responsibility for ongoing use and maintenance therefore:
1. Maintain a minimum temperature of 4 degrees C (40 degrees F) at all times, except as otherwise specified.
2. Maintain in operating condition existing fire protection and alarm equipment to the greatest extent feasible. Make arrangements for a pre-inspection of existing fire-protection systems with appropriate representatives of the Fire Department that will respond to an alarm.
1.10 QUALITY CONTROL & CONSTRUCTION TESTING:
A. Comply with Quality Control requirements of the Contract, including preparation of the
Contractor’s Quality Control Plan (CQC) specific to this Project.
B. Include all costs for on-site quality control testing, Testing Laboratory Services, and other quality control activities.
1.11 ALTERATIONS:
A. SURVEY OF EXISTING FACILITIES: Before any Work is started, the Contractor must make a thorough survey with the GPM of areas of buildings in which alterations will occur and areas which are anticipated routes of access, and furnish a report, signed by both, addressed to the
Contracting Officer. This report must list by rooms and spaces, the following:
1. Existing condition and types of flooring, doors, windows, walls and other surfaces not required to be altered.
2. Existence and conditions of items such as plumbing fixtures and accessories, electrical fixtures, equipment, venetian blinds, shades, etc., required by drawings to be either reused or relocated, or both.
3. Note any discrepancies between Contract Documents and existing conditions at site.
