FOR

2022 SUNSHADE DISASSEMBLY AND REMOVAL

HURLBURT FIELD, FLORIDA

U-28 Sunshade Disassembly and Removal

HURLBURT FIELD, FLORIDA

1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, labor, and transportation for the disassembly and removal of (10) U-28 Sunshades. The contractor shall accomplish all tasks to meet the requirements of the Statement of Work (SOW).

2. SPECIFICATION REQUIREMENTS.

2.1. The contractor shall complete a full disassembly and removal of ten (10) sunshades, and is responsible for the disposal of the sunshades in their entirety.

2.2. The contractor shall mitigate old anchor points so they do not pose a hazard to personnel or aircraft movement by way of grinding flush with the concrete. Anchor point removal and grinding must be inspected by CES personnel with coordination of 1 SOMXG QAE office.

2.3. The contractor shall perform a thorough and complete site clean-up upon project completion.

4. QUALITY ASSURANCE The 1 SOMXG Quality Assurance Evaluator (QAE) office will perform the final inspection with Airfield Management after removal of all the shelters, to ensure the area is clear of debris and remnants of the sunshades, and that the anchor points have been removed/shaved flush so as to not pose a health and safety hazard to personnel and aircraft. Anchor Points must be inspected by CES personnel, coordination will be completed through QAE office.

5. GENERAL INFORMATION

5.1 CONTRACTOR PERSONNEL

5.1.1. CONTRACTOR EMPLOYEES. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

5.1.1.1. The contractor shall appoint a manager and alternate manager for work performed under this SOW. Contact information for each manager will be provided to the government within three (3) working days of contract acceptance.

5.1.1.2. All contract employees shall be properly trained and certified to perform the required tasks.

5.1.2. CONTRACTOR/EMPLOYEE BASE PASS AND IDENTIFICATION, SPECIAL

CLEARANCES, AND VEHICLE PASSES. The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.

3. PERIOD OF PERFORMANCE.

3.1 The contractor shall complete the disassembly and removal of the sunshades within 30 days of award.

5.1.2.1. Contract personnel who are to operate vehicles on the airfield (within fenced area) are required to attend airfield driver training with 1 SOSS/OSAA (850-884-1537).

5.1.3. CONTRACTOR EMPLOYEE LIST. This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “Escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two).

For work locations use building numbers or specify as base wide if applicable. Once completed, email list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center (VCC).

5.1.3.1. Upon notification of the employee listing being forwarded to the VCC, all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification.

5.1.3.2. If an employee will be driving on Hurlburt Field, they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial Gate Control Center administering this function is closed after 1800 (6:00 P.M.), therefore, employees must arrive prior to this in order for their documentation to be processed.

5.1.3.3. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

5.1.3.4. Upon completion or termination of the contract or the expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.

5.1.3.5. The contractor shall submit the employee listing the Contracting Officer no less than three (3) days prior to the performance start date and at lest one (1) day prior as changes occur.

5.2. SECURITY REQUIREMENTS. Contract personnel shall not enter restricted areas without an escort from authorized personnel. The contractor shall restrict personnel to only those areas required to performing work.

5.2.1. WEAPONS, FIREARMS, AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field, FL.

5.2.2. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment and materials shall be secured.

5.3. SAFETY AND HEALTH STANDARDS. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act concerning handling potential hazardous substances. Each member shall be provided with, and trained in the proper use of personal protective equipment and equipment necessary for contract performance.

5.3.1. Contractor personnel are required to clear the airfield and seek shelter in the event of inclement weather. This includes tornadic activity or when there is the possibility of lightning within five nautical miles from the center of Hurlburt Field’s runway 18/36exists. Notification of inclement weather is issued by the base giant-voice notification system as either a steady tone or by announcing, “LIGHTNING WARNING”. Contractor personnel are clear to return to the work site when the base giant-voice notification system announces, “ALL CLEAR” or “CANCEL LIGHTNING WARNING”.

5.4. REPORTING REQUIREMENTS. The contractor shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, classified or unclassified defense information and damage to government property.

5.5. SMOKING, EATING, AND DRINKING. The contractor shall permit smoking, eating, and drinking only in designated areas.

5.6. DRIVING LICENSE/ON-BASE REQUIREMENTS. The contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Hurlburt Field, Florida.

5.7. WORK LOCATION, PHYSICAL SECURITY, AND OPERATING SCHEDULE. No government furnished repair facility, i.e. workstation, test equipment or work bench, shall be provided on Hurlburt Field, Florida.

5.8. HOURS OF OPERATION. Normal duty hours shall be 0700 – 1800 (7:00 A.M. to 6:00 P.M.), Monday through Saturday. Contractors must contact Airfield Management daily, at beginning and at completion of work day (850-884-7806/7807.
