Statement of Work (SOW)

DNA Analyzer 3730xl SERVICE

GENERAL INFORMATION

1.0 Scope of Work: Contractor will provide factory trained engineers to perform one (1) preventive maintenance service visits and unlimited emergency visits on the DNA Analyzer 3730xl located at 1815 N University Street in Peoria Illinois.

Covered components will include:

1) DNA Analyzer 3730xl 2.0 Background: Preventive maintenance is required to ensure continued & reliable operation in support of the NCAUR research mission.

CONTRACTOR REQUIREMENTS

3.0 Technical Requirements/Tasks:

Service tasks include, but are not limited to:

1. Parts, labor and travel for remedial repair.

2. No charge for one (1) planned maintenance visit.

The annual planned maintenance (PM) visit is automatically opened and will be performed within the contract period. The PM visit ensures optimal performance of the instrument, often preventing major breakdowns before they happen.

3. Guaranteed priority response time of two (2) business days after receipt of a service call for instrument. If Contractor fails to arrive at the instrument location, Contractor will provide customer a service plan renewal credit in an amount equivalent to one day's pro-rated charge for each day Contractor's response is late.

4. Priority telephone and email access to instrument technical support.

5. Telephone and email access to application technical support.

6. Remote Monitoring and Dx Service.

Repair parts are to be included unless otherwise stated in this statement of work.

4.0 Government Furnished: Government-furnished support is limited to security access to the equipment in the NCAUR facility, replacement parts not otherwise covered under this agreement, AC power (110VAC) and standard office lighting.

6.0 Travel: One (1) preventive maintenance service visit will be provided by the Contractor. All service visits will be coordinated with the 3730xl Operator at NCAUR.

7.0 Contractor’s Key Personnel: Only factory trained engineers are to perform any service procedure on the DNA Analyzer 3730xl.

8.0 Security Requirements: A copy of an official government photo ID (State-issued driver’s license or ID card, passport, green card, etc.) must be provided no later than one week before arrival of contract service technician(s) except in the case of emergency service, were only the name of the technician provided via email or telephone will be necessary. Security access to the NCAUR facility will be provided based on this identification requirement.

9.0 Data Rights: Not applicable.

10.0 Section 508 – Electronic and Information Technology Standards: Not applicable.
