J-4 ATTACHMENT IV- PAST PERFORMANCE QUESTIONNAIRE FOR REFERENCES

Part I: Please answer the following questions regarding the contractor’s past performance.
1. Did your contractor deinstall, pack, ship, reinsall and certify data processing equipment such as: workstations, fileservers, network printers and LAN equipment in your relocation activities?
2. Did the contractor adhere to the Response Time(s) and the Return to Service Time(s) performing Relocation activities in accordance with the scope of the contract? What was the percentage of Relocations that were completed within the Return to Service timelines?
3. Did the contractor perform satisfactorily, or in accordance with the scope of the contract?
4. Did the contractor/subcontractor provide required replacements parts in the event of damaged equipment in accordance with the scope of the contract and in a reasonable timeline?
5. If a problem arose, did the contractor/subcontractor take the necessary steps to resolve the problem as quickly as possible in accordance with the contract terms and conditions?
6. What are the contractor’s strengths?
7. What are the contractor’s weaknesses?
8. Would you do business with this company again? Why?
Part II: Instructions: Please rate the contractor on a scale of Excellent, Good, Neutral, and Poor. The following information should be considered when rating the identified factors:
Excellent– Performance met all contract requirements and exceeded many to the customer’s benefit. Problems, if applicable, were negligible and were resolved in a timely, highly effective manner.
Good– Performance met all contract requirements and exceeded some to the customer’s benefit. There were a few minor problems which the contractor resolved in a timely, effective manner.
Neutral – Performance met contract requirements. There were some minor problems and corrective actions taken by the contractor were satisfactory. This rating is also assigned when there is no record of past performance or the record is inconclusive.
Poor– Performance did not meet most contractual requirements. There were serious problems and the contractor’s corrective actions were ineffective.
