Lake Eva Dock Repairs Performance Work Statement (PWS) Page 1 of 6
General Information
1.0 Background and Purpose: This is a non-personal services contract to repair the Lake Eva float plane dock to pre-storm condition, as well as make some minor improvements. All services are on the Sitka Ranger District, Tongass National Forest. The Government will not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn, is responsible to the Government.
1.1 Outline of Services: The Contractor shall furnish all labor, supervision, management, tools, materials, equipment, facilities, transportation, and other items necessary to provide the services outlined below and described in this Performance Work Statement
(PWS).
1.2 Project Location: Lake Eva recreation cabin is located on the east side of Baranof Island.
A vicinity map can be found in Appendix 4 Original Construction Drawings. The cabin is accessible by float plane or by boating to the Lake Eva trailhead on Hanus Bay and walking 1.75 miles on a non-motorized trail then boating across the lake to the cabin. A 15-ft aluminum skiff is stored at the cabin and will be made available to the contractor (See Section 4.0 Government Provided Property). The Lake Eva cabin is not accessible by road.
1.3 Background: The Lake Eva float plane dock, which is used to access the cabin, was damaged by an extreme weather event in the winter of 2020. The gangway was ripped off the shore foundation and partially sunk. The dock drug anchor and is no longer in the correct orientation to the gangway/cabin.
1.4 Period of Performance:
a. Date of Award – 12 months from Date of Award
1.5 Prework Meeting: A pre-work meeting with the successful offeror is required. The meeting will be scheduled and held within 10 business days after the date of contract award. The conference will be held via conference call if a suitable meeting location is not available.
1.6 Government Regular Working Hours: The Government's regular working hours are from 0700 to 1700, five days per week, Monday through Friday, except observed Federal holidays. Exceptions to the regular hours of operation are detailed in subsequent sections of this PWS.
1.7 Federal Observed Holidays: The government observes the following holidays:

1.8 Service Interruptions: If any services must be interrupted (even temporarily) due to maintenance of contract work, the Contractor shall notify the CO, and COR at least three working days in advance. If the service is due to an emergency breakdown the Contractor shall notify the CO and COR as soon as practicable.
1.9 Quality Assurance: The contractor shall comply with the quality assurance program as defined in the Quality Assurance Surveillance Plan. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable quality level(s) (AQL). Government surveillance of contractor performance is not limited to the Performance Objectives as outlined in this PWS. The Government reserves the right to conduct compliance surveillance of any contractual requirement of this acquisition.
1.10 Contracting Officer’s Representative (COR): A COR may be assigned to this contract to monitor all technical aspects and assist in contract administration. The COR may be authorized to perform the following functions: Assure that the Contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communication with the Contractor concerning technical aspects of the contract; issue written interpretations of technical requirements (without increasing or decreasing contract scope), including Government drawings designs, and specifications; monitor Contractor's performance and notify both the CO and Contractor of deficiencies; and provide site entry of Contractor personnel. The COR is not authorized to change any of the terms and conditions of the contract or any task order.
2 Contractor Requirements
General Requirements Applicable to All Tasks: All material shall be delivered, stored, handled, and installed per manufacturer’s specifications and recommendations. Remove and dispose of all project trash, demolition debris, and residue off National Forest System land.
SUBMITTALS
Submit the following plans and product data for COR approval prior to starting services or procuring materials. Timely submission of deliverables is essential to successful completing this requirement. All deliverables shall be prepared and submitted according to format, content, and schedule described in the contract. The Government will review and reply to contractor submittals in no later than ten (10) business days from receipt.
2.1 Materials submittals: Gangway repair materials, Pin hanger assembly, Gangway run-off plates and Dock Bumpers. Submit Material list to COR prior to purchasing materials to ensure correct materials are procured.
2.2 Structural details: If the Contractor’s plan for repair includes a modification to the existing system design, such as a bolted connection where previously a welded connection existed, Contractor shall submit a proposed design for approval. The repair plan shall include a set of plans and calculations stamped by a registered professional engineer with current license in the State of Alaska. Plans shall include enough information for the Contractor to construct the new design. The plan shall include a stamped set of calculations to justify that the new design will meet the structural performance of the original design.

2.3 Work Schedule: Prior to the pre-work meeting, provide a proposed schedule of work showing beginning and completion dates of salient work items for the COR approval. Contractor shall maintain the schedule up to date and provide it to the COR upon request
2.4 Safety Plan: The Contractor shall use measures and precautions necessary to warn and protect the public and Government personnel during work at the project site. Such actions include, but are not limited to, furnishing, and maintaining barricades and signs around the work site and roping off the area. Prior to the pre-work meeting, submit a written Safety Plan to the COR for approval. Plan shall indicate methods of providing for safety for the workers. Plan shall address safety of contractor personnel, protection of improvements, and the public.
2.5 Dive Plan IAW with Attachment 5 Forest Service Handbook (FSH) 4209 Contract Diving (if applicable) prior to diving services to ensure safety.
Technical Requirements / Tasks
2.6 TASK 1: Repair Lake Eva Dock - Repair gangway and pin hanger assembly to like new condition, including: Replacing the two (2) gangway run-off plates in kind, except increase the length of each plate from 1.5 feet to 3.5 feet. Replacing 10 feet of damaged dock bumper in kind. Dock and gangway reset to original configuration.
The Government intends to exercise one of the three options upon placing award:
2.7 OPTION TASK 2: Reset anchor - If exercised, reset existing 70-lb claw anchors in accordance with Appendix 3 Anchor Plan.
2.8 OPTION TASK 3: Remove and replace anchor - If exercised, remove and replace existing dock anchors. Install four (4) pyramid anchors per Appendix 3: Dor-Mor 200-lb or approved substitute. Install new galvanized shackles at anchor to chain connection and secure shackle pins with stainless steel mousing/safety wire. Submit anchor and shackle product data for approval by the COR prior to purchasing materials.
2.9 OPTION TASK 4: The Government is interested in other commercial approaches to secure and ensure the Lake Eva dock remains in place in the event of a 100-year flood and with annual lake level fluctuation of up to 6 feet, when factoring in wind wave height.

For informational purposes: Forest Service personnel used half inch (#4) rebar to probe the lake bottom in the vicinity of the original anchor placements. The bottom was found to be soft in all locations probed. Personnel worked the probes by hand to penetrate the sediment to a depth of approximately 5 feet with no bedrock encountered.
3.0 Deliverable / Schedule

4.0 Government Furnished
4.1 The Government will provide the Contractor with use of the Lake Eva recreation cabin for the duration of the work. The work shall be scheduled with the COR at least 30 days in advance and is subject to cabin availability. 4.2 Typically, a 15 ft aluminum skiff is available for use on Lake Eva. The existing skiff was functional in 2022 and a new skiff will be delivered to Lake Eva summer of 2023. However the condition of the skiff at the time of the work is unknown.
5.0 Travel. The Contractor shall be responsible for all travel to and from the work site location

8.0 Definitions, Acronyms, and Applicable Publications
Contractor: A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.
Contracting Officer (CO): a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the contracting officer acting within the limits of their authority as delegated by the contracting officer. "Administrative contracting officer (ACO)" refers to a contracting officer who is administering contracts. "Termination contracting officer (TCO)" refers to a contracting officer who is settling terminated contracts. A single contracting officer may be responsible for duties in any or all of these areas. Reference in this regulation (48 CFR chapter 1) to administrative contracting officer or termination contracting officer does not-
Require that a duty be performed at a particular office or activity; or Restrict in any way a contracting officer in the performance of any duty properly assigned.
Contracting Officer’s Representative (COR): means an individual, including a contracting officer’s technical representative (COTR), designated and authorized in writing by the contracting officer to perform specific technical or administrative functions.
Defective Service: A service output that does not meet the standard of performance associated with the Performance Work Statement.
Deliverable: Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.
Equipment: A tangible item that is functionally complete for its intended purpose, durable, nonexpendable, and needed for the performance of a contract. Equipment is not intended for sale, and does not ordinarily lose its identity or become a component part of another article when put into use. Equipment does not include material, real property, special test equipment or special tooling.
Government Furnished Property: Property in the possession of, or directly acquired by, the Government and subsequently furnished to the contractor for performance of a contract. Government-furnished property includes, but is not limited to, spares and property furnished for repair, maintenance, overhaul, or modification. Government-furnished property also includes contractor-acquired property if the contractor-acquired
Performance Work Statement (PWS) Page 6 of 6 property is a deliverable under a cost contract when accepted by the Government for continued use under the contract.
Non-Personal Services: The personnel rendering the services are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the government and its employees.
Non personal service contracts are authorized by the government in accordance with FAR 37.012, under general contracting authority, and do not require specific statutory authorization.
Quality Assurance: The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
Quality Assurance Surveillance Plan: An organized document written by the government specifying the surveillance methodology used for surveillance of contractor performance.
Workday: The number of hours per day the Contractor provides services in accordance with the contract.
Work Week: Monday through Friday, unless specified otherwise.
