Quality Assurance Surveillance Plan (QASP) for Defense Logistics Agency Disposition Services (DLA DS) Continental United States (CONUS) Performance-Based Material Handling Equipment (MHE) Maintenance for the Southern California Region
1. Purpose:
a. This document is to provide guidance on administering subject contract such that the best possible quality in performance can be assured through appropriate surveillance and addressing of any performance failures as appropriate. The Acceptable Performance Level (APL) is the standard the United States Government (USG) expects the contractor to meet throughout the life of the contract. In general, the DLA DS Southern California Region MHE Maintenance contract will have nine (9) APLs that will be used to measure contractor performance though additional ones may be identified depending on the contract requirements. The standard APL is signing the visitor log, timely and complete Preventive Maintenance (PM), PM sticker affixed to each piece of equipment, dated filters, preventive maintenance sheet, Corrective Maintenance (CM) service, cost estimate for CM service, timely and complete CM’s and call back service.
2. Acceptable Performance Levels:
a. Sign in on visitor log.
(1) “Arrival” in the standard column means every call for service regardless of PM or CM.
(2) The APL is measured throughout the period of performance.
b. Timely and Complete Preventive Maintenance.
(1) “item” in the standard and APL column means a single piece of MHE.
(2) The APL is measured per quarter.
c. PM sticker affixed to each piece of MHE.
(1) “item” in the standard and APL column means a single piece of MHE.
(2) The APL is measured throughout the period of performance.
d. Written date on all filters.
(1) “filter” in the standard and APL column means each filter on a piece of MHE.
(2) The APL is measured throughout the period of performance.
e. Preventive Maintenance Sheet.
(1) “sheet” in the standard column means one sheet for each piece of MHE receiving a PM.
(2) The APL is measured throughout the period of performance.
f. Corrective Maintenance Service.
(1) The APL is measured throughout the period of performance.
g. Cost Estimate for Corrective Maintenance.
(1) “estimate” in the standard and APL column means one estimate per CM service notification.
(2) The APL is measured throughout the period of performance.
h. Timely and Complete Corrective Maintenance.
(1) “timely” in the standard and APL column means the corrective maintenance was complete within the specified timeframe for each CM service required.
(2) The APL is measured throughout the period of performance.
i. Call Back Service.
(1) The APL is measured throughout the contract performance period.
3. Surveillance:
a. The primary method of surveillance for evaluating compliance is by Contracting Officer’s Representative (COR) reporting. The COR is responsible for reporting PM performance to the Contracting Officer (KO) promptly after the performance deadline has passed. The COR will provide when the action was required to be performed by the contractor and the date it occurred. The COR shall also include what the negative impact of the contractor’s failure to meet the APL was on the DLA DS site.
b. In instances where there are late completions of PM or CM, the COR shall determine if the late performance is caused by an occurrence beyond the reasonable control of the contractor and without its fault or negligence such as, acts of God , the public enemy, acts of the USG either its sovereign or contractual capacity, fires, floods, epidemics, quarantine restrictions, strikes, unusually severe weather, and delays of common carriers. In such cases as there is an excusable delay, the contractor will be deemed to have made the APL for the period.
4. Reduction for Failed APL:
a. Throughout each performance period of the contract, including options, the contractor is allowed one (1) failure of exceeding the three (3) late performance standards. After the failures the KO may take consideration for each additional instance of the contractor not meeting the APL.
b. The government may use such measures as partial or full terminations under Federal Acquisition Regulation (FAR) 52.212-4, not exercising options, and ending a contract early without these measures if it is Indefinite Delivery, Indefinite Quantity (IDIQ) and has met its minimum government obligation for the period and a plan for a timely replacement contract is secured.
