STATEMENT OF WORK

Carpet Replacement Building 486 601 Air Operations Center, 101 Air Operations Group Tyndall AFB, Florida

General: Contractor shall furnish all labor, materials, transportation, tools, equipment, and supervision needed to:

1. Move/lift furniture and return to exact location.

2. Remove and dispose of indicated existing carpet.

3. Remove indicated portions of existing rubber base from wall and columns.

4. Furnish and install new carpet, rubber cove base, door transitions, and LVT.

5. Contractor shall verify all dimensions during site visit.

6. Contractor shall submit physical samples of carpet, transitions, cove base, and LVT to customer prior to installation.

General Requirements:

Conduct all work in accordance with the Contract Requirements/Specifications, all applicable codes, and any construction details or drawings provided. The Contractor shall comply with the Tyndall AFB Design Guide, Tyndall AFB Installation Specifications, current version of UFC 1 200-01 DOD Building Code, and all applicable current UFCs.

Performance Requirements:

Air Force Carpet Program: As of 18 January 2023, carpet installation shall be accomplished in accordance with the Air Force Carpet Program, using the carpet ordering guide and one of the seven contracts awarded for materials. The Air Force Carpet Program includes modular carpet tiles, roll goods, walk-off tiles, and adhesives. Installation will be managed by local installers.

Measurements: The contractor shall be responsible for measuring and verifying the area and perimeter of all spaces receiving carpet, cove base and door transitions.

Product Data: Submit two copies of manufacture’s full technical data and installation instructions for each type of carpet and accessory. Contractor shall submit physical samples to customer prior to installation.

1. The contractor shall move furniture as necessary to remove and install carpet. Furniture shall be returned to the exact location it was moved from.

2. The contractor shall choose one of the vendors to work with and install the carpet selections below in the locations indicated.

3. Cove base: Install resilient rubber cove base, (1/8” thick, 4” high, Flexco, or approved equal) in areas where carpet is being installed to replace all wood base removed. Color to be determined.

4. Transitions: The contractor shall install transitions in areas where there is a change in floor type e or carpet height. Transitions shall match or compliment cove base. Color to be determined.

5. LVT shall be Mohawk Living Local Stonework C0179 985 Stamped Concrete) or Armstrong Unleashed F0019 Quarrytile Raven

Removal and Disposal:

6. Carpet: The contractor shall remove and dispose of carpet indicated on floor plans without disturbing the flooring underneath. Contractor shall lay carpet over the existing tile floor.

7. Cove Base and Transitions: The contractor shall dispose of a wood base from walls and columns and remove existing transitions.

8. Disposal: The contractor is responsible for providing a dumpster at the worksite prior to starting the carpet installation and removing it after completion.

Installation: Contracting Qualifications: All work shall be performed be contractors/installers who are Certified Floor covering Installers (International Certified Floor covering Installer Association) or manufacture’s approved installers. Foreman on the jobsite shall be “Certified C-II Minimum.” For warranties to be valid, all carpet shall be installed in accordance with the installation guidelines set forth by the manufacturer and/or the minimum guidelines set forth in CRI 104-2002, Standard for Installation Specification of Commercial Carpet, and CRI 105-2002, Standard for Installation of Residential Carpet.

1. Carpet: The contractor shall work with one of the vendors listed under General Information and install the carpet choices in the areas indicated. Carpet shall be installed in full accordance with manufacturer’s recommendations to preserve all warranties.

2. Cove Base: The contractor shall install resilient rubber cove base in areas where carpet is being replaced/installed. (1/8” thick, 4” high, Flexco, or approved equal. Color: to be determined). Purchase and install approximately one thousand eight hundred (1800) linear feet of vinyl cove base. All outside corners will be factory made outside corner pieces. Color of the vinyl baseboards will be either, approved by requester prior to vendor purchase. Rubber or synthetic entry thresholds will be replaced with new material. Metal or wooden thresholds will be removed and/or replaced as necessary. The Contractor shall verify all dimensions listed above. The Government does not guarantee the above dimensions are 100% accurate.

3. Transitions: The contractor shall install transitions in areas where there is a change in floor type. Transitions shall match or compliment cove base. Color to be determined but needs to compliment the Cove Base.

4. Additional Material: The contractor shall provide 15% of extra tiles above the amount installed. These will be given to the customer.

Performance Period: Shall not exceed 90 days from date of contract award.

Work Schedule/Hours/Plans: Workdays for this contract will be standard duty hours: Monday through Friday, 7:00 am to 4:00 pm, excluding federal holidays, Wing, and unit down days. Building 486 will have occupants and will have to be worked around by only doing half an office at a time unless we are able to totally move out and relocate. The contractor shall not begin any work until 100% material complete. Written permission from the designated Government Representative (Mr. Steven Johnson) shall be obtained before making any deviation from the schedule.

Vacating Rooms: Furniture shall be moved/lifted and returned to its exact location after installation is complete. Heavy filing cabinets will be emptied by government personnel if necessary. A point of contact (Mr. Steven Johnson) will be assigned to coordinate scheduling and relocations.

Doors: After carpet installation is complete, the contractor shall ensure that all doors affected by the project can open and close properly.

Cleaning: After carpet installation is complete and prior to returning furniture to its original location, the contractor shall remove all debris, scraps, and any other foreign matter. Contractor shall remove any spots or adhesive from the carpet or adjoining surfaces, to include walls, clip any protruding face yard and vacuum the entire work area.

Safety Requirements: Safety requirement shall be the applicable sections of EM 385-1-1, 30 November 2014 and Occupational Safety and Health Administration (OSHA) regulations. OSHA regulations are available for review at the Base Safety Office, Building 105. EM 385-1-1 is available for reference in the Civil Engineering Office, Building 36234. The contractor shall ensure that any lifting devices or ladders meet all applicable safety standards. The contractor shall be required to use those lifting devices that will allow for the existing modular systems furniture to be raised for the installation of the new materials requested. The contractor will not be required to disconnect any electrical system connections to or from the modular systems furniture.

Site Visit: A site visit will be offered pursuant to Federal Acquisition Regulation (FAR) Clause 52.237-1. Regardless of site visit attendance, contractors acknowledge satisfaction as to the nature and location of the work, the general and local conditions, access to job site; disposal, handling and storage of materials; availability of labor, water, electric power or similar physical conditions at Moody Air Force Base; the character of equipment and facilities needed preliminary to and during the performance of the work; and all other matters that can in any way affect the work or costs related to this requirement upon submission of offers. Contractors further acknowledge that any damage to the existing structure in and around facilities will be returned to existing condition prior to the completion of subject work at no additional cost to the Government in accordance with FAR Clause 52.237-2. The contractor shall provide the Government with a one-year warranty on workmanship. The contractor shall provide any manufacturer’s warranty to the Government.

Security

General: The Contractor shall comply with all base security regulations and with actions dictated by force protection condition changes/measures. For real-world situations/exercises, changes to force protection conditions could impact Contractor performance of work and can include denial of access to the installation to perform work.

The Contractor and its employees shall comply with all base traffic regulations.

Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within Contractor-owned or privately owned vehicle while on Air Force installations.

The Contractor shall report suspicious activity (refer to Tyndall AFB Contractor Antiterrorism Awareness pamphlet) to Security Forces at 850-283-2254 in a timely manner.

Additional Security Requirements

Contractor Access to Air Force Installations. All Contractor employees will be subject to a criminal history background check IAW Air Force Manual (AFMAN) 31-113, Installation Perimeter Access Control, as stipulated in the Installation Commander’s memorandum to the 325th Security Forces Office dated 23 Apr 2015 (the AFMAN and memorandum are both limited access documents but more details on the equirements/process in terms of background checks can be provided by the Unit Security Manager).

Vehicle registration, proof of insurance, and a valid driver's license must be presented for vehicles. Installation Commanders may deny access and credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline, and morale of the installation.

Base Access: The Contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles:

a. DBIDS pass. Obtained through submitting personal identifiable information to Security Forces for criminal background check.

b. DoD Common Access Card (CAC), AFI 36-3026(I). Process through USM, Trusted Agent Sponsorship System (TASS) point of contact for a CAC card.

c. AF Form 75, Visitor/Vehicle Pass (AFI 31-204): The Contractor shall adhere to local security policy when applying for short/long term visitors’ passes. Any Contractor requesting a pass for more than 3 days will complete a Local Files Check with 325 SFS/Pass & ID. Vehicle passes (personal vehicles) are no longer required.

Visitors/Contractors with driver’s licenses from ME, MN, MO, MT, and WA (exceptions are EDL-Enhanced driver’s licenses from MN & WA) will not be able to obtain base access without another form of identification as an identity proofing document. The list of authorized IDs can be found in the REAL ID Act of 2005. During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel.

The Contractor shall be responsible for prompt return of credentials and vehicle passes for:

1) any employee who no longer requires access to the work site; 2) upon request by a CO; and 3) upon completion or termination of the contract. The prime Contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. Failure to comply with these requirements may result in withholding of payment.
