DISPOSITION SERVICES

10. INVOICING

1. INTRODUCTION

a. The primary mission of the Defense Logistics Agency (DLA) Disposition Services (DLADS) is to dispose of Department of Defense (DoD) excess and surplus personal property generated by the military services. This is accomplished through reutilization to other DoD activities, transfer to other Federal agencies, donation to authorized state and local organizations, sale to the public, or ultimate disposal through abandonment or destruction. DLADS is headquartered in Battle Creek, Michigan, with a variety of field sites worldwide.

b. This requirement is for the purchase and delivery of ten (10) total portable, mobile loading ramps for the following sites: three (3) ramps for DLADS Susquehanna, one (1) for DLADS Lewis, two (2) for DLADS San Joaquin, two (2) for DLADS Warner Robins, and two (2) for DLADS Norfolk.

c. This contract is at a Firm-Fixed Price (FFP).

d. Contractors shall price items on the price schedule to include any packing/shipping charges to DLADS Susquehanna, Lewis, San Joaquin, Warner Robins, and Norfolk. For a list of delivery locations see Section 12 of this SOW.

2. FEDERAL HOLIDAYS

a. The Contractor is not required to deliver any equipment on the eleven (11) Federal holidays observed by DLADS unless approved by the Contracting Officer (KO) or the onsite Point Of Contact (POC). The following holidays are observed:

b. When one of the holidays falls on a Sunday, the following Monday will be observed as a legal holiday. When the legal holiday falls on a Saturday, the preceding Friday is observed as the holiday.

3. PORTABLE, MOBILE LOADING RAMPS DLADS SUSQUEHANNA– CLIN 0001:

a. New Equipment ONLY; no remanufactured or “gray market” items.

b. The ramps will have a DoD item unique identification (IUID) in compliance with DFARS 252.211-7003.

c. When CLIN 0001 is ordered, the Contractor shall manufacture or purchase and deliver three (3) portable, mobile loading ramps with the following salient characteristics to DLADS Susquehanna:

1.

Ramp Clamp that is secured by a locking device 2.

Full undercarriage with solid 18” rubber tires

3.

Minimum Load Capacity supporting 16,000 pounds or more

4.

36-foot ramp with a minimum of a 6-foot level off

5.

Constructed of Heavy-Duty steel welded construction

6.

Ramp shall be built to OSHA standards

Anti-Slip Surface

8.

Manual crank Double-action hydraulic pump for raising and lowering ramp to support a service range of 42” to 59”

Safety chains to secure ramp to the dock or trailer 10.

4. PORTABLE, MOBILE LOADING RAMPS DLADS LEWIS, NORFOLK, SAN JOAQUIN, AND WARNER ROBINS– CLIN 0002:

a. New Equipment ONLY; no remanufactured or “gray market” items.

b. The ramp will have a DoD item unique identification (IUID) in compliance with DFARS 252.211-7003.

c. When CLIN 0002 is ordered, the Contractor shall manufacture or purchase and deliver the following: one (1) for DLADS Lewis, two (2) for DLADS San Joaquin, two (2) for DLADS Warner Robins, and two (2) for DLADS Norfolk.

1. A 2 to 1 load rating safety factor

2. Constructed of Heavy-Duty Steel for structural rigidity

3. Ramp built in side rails at the height minimum of 8”

4. Upper-Level Pedestrian railing for safety to OSHA standards

5. Side Steps to Upper-Level for ease of access to conveyance

6. Minimum of 8-Feet-Upper Level-Off Section

7. Anti-Slip Surface

8. Hinged Lip Plate for safe transition in and out of the conveyance

9. 60” Minimum Length Hinged Tail Plate for smooth transition to uneven yard surfaces

10. Minimum Load Capacity supporting 30,000 lbs. or higher

11. Hinged Lip Plate must be able to support up to a 90” width and have a chamfered leading edge

12. Adjustable Ramp Height from a minimum level of 36” dual speed leveling jacks

13. Ramp Incline shall be at a minimum of 24’-8” Long

14. Towable by either way of a tug or forklift to reposition in different locations at the site

15. Provide each ramp with a tow bar and connection pins for ease of mobility and repositioning

16. Solid rubber pneumatic profile tires

17. Minimum Ramp length of 32’-8” (minimum just includes level off and ramp incline not the hinged lip plate or hinged tail plate lengths)

18. Minimum of 78” useable space with an overall width of 90”

5. EQUIPMENT DELIVERY AND WARRANTY:

a. For the Contractor to successfully handoff the ramps at each DLADS location and receive payment from the United States Government (USG), they shall:

1) Be present for delivery.

2) Unload the piece of equipment.

3) Assemble the piece of equipment (if required).

4) Conduct an inspection with USG personnel to verify that all items are present, and that the equipment contains the minimum listed salient characteristics.

b. The Contractor shall complete and sign the provided DD250, Material Inspection and Receiving Report, with USG personnel and attach the completed form in Wide Area Workflow (WAWF) prior to acceptance for payment, see section 13. A picture of the data plate with serial number shall be attached to the invoice prior to Government acceptance.

c. This equipment shall come with the manufacturer’s standard equipment warranty. The warranty date shall begin upon completion of assembly (if needed) and the equipment inspection.

6. MANUALS AND FAMILIARIZATION TRAINING:

a. The Contractor shall provide one digital and one hard copy Operators Manual Guide, and Maintenance and Service Manual for the equipment delivered.

7. BASE ACCESS, SAFETY, AND SECURITY:

a. The Contractor shall coordinate base installation access for delivery. The Contractor shall be familiar with and comply with all Base Command policies regarding security considerations for all Contractor employees.

8. SPILL RESPONSIBILITY:

a. The Contractor is solely responsible for all spills or leaks during the performance of this contract, which occurs because of, or is contributed to by the actions of its agents, employees, or subcontractors. The Contractor shall clean up such spills or leaks to the satisfaction of the USG in a manner that complies with applicable Federal, state, and local laws and regulations. The clean-up shall be at no cost to the USG. The Contractor shall report all such spills or leaks, regardless of their quantity, to the COR and KO immediately upon discovery. A written follow-up report shall be submitted to the COR and KO not later than twenty-four hours after the initial telephonic report. The written report shall be in narrative form, and as a minimum shall include the following:

1. Description of item spilled (including identity, quantity).

2. Whether amount spilled is EPA or state reportable, and if so, whether it was reported. Exact time and location of spill including a description of the area involved.

3. Containment procedures initiated.

4. Summary of any communications Contractor has with press or USG officials other than the KO.

5. Description of clean-up procedures employed or to be employed at the site including disposal location of spill residue.

6. Description of item spilled (including identity, quantity).

DESTINATIONS, QUANTITIES TO BE DELIVERED AND POINTS OF CONTACT:

a. The Contractor shall notify each site location at least 14-business days prior to delivery of the ramp(s) to its destination to ensure the site is prepared to accept the ramp(s). The Contractor shall use dladsopsmhe@dla.mil and Timothy.Zweng@DLA.mil as the official communication link for all delivery functions.

b. The Contractor shall verify the destination, POC and projected CLIN. Prior to final delivery, the Contractor shall verify the shipping address and set up pre-installation meeting date, time, and location with the Site Manager. All destinations and CLINs are subject to change and will be provided on the associated Task Order.

c. Destinations associated with this SOW are listed below and are within the Continental United States (CONUS).

10. INVOICING:

a. The Contractor will invoice through the Department of Defense WAWF system. Invoices must be submitted through WAWF in accordance with DFARS 252.232-7003. Refer to https://wawf.eb.mil for additional information.
