Statement of Work

59th Medical Wing, Lackland AFB, TX

Installation of Hologic Panther Plus Analyzers with Option 4

1. SCOPE

This requirement provides for the delivery and installation of 2 Hologic Panther Plus

Analyzers with Option 4.

1.2. Background

Currently the WHASC Microbiology Laboratory is unable to provide testing for GC/Chlamydia and COVID-19 testing. The use of the Hologic Panther Plus with Option 4 will allow the laboratory have this testing capability providing results for COVID-19 and GC/Chlamydia to support the BMT mission within 24 hours.

1.3. Place of Performance. 1100 Wilford Hall Loop, BLDG 4550 Lackland AFB, San

Antonio, Texas 78236, Laboratory, Immunology Room 0B022.

2. REQUIREMENTS.

1 – Adequate space to install 2 Hologic Panther Plus with Option 4 Analyzers

1 – Misc. hardware/tools needed for installation

1 – Installation

2.1. Products and Maintenance Services

The contractor shall provide all transportation, labor, materials, and equipment necessary for the installation of 2 Hologic Panther Plus Analyzers with Option 4. The contractor shall test the analyzers to ensure proper operation. All services are strictly non-personal in nature. All hardware and analyzers shall be new.

3. INSTALLATION AND ADDITIONAL SERVICES

Contractor is responsible for the delivery, installation, and ensuring proper functioning of the analyzers. Installation will be according to Hologic requirements.

3.1. Installation and Programming

Installation of the Hologic Panther Plus with Option 4 will be completed by the Contractor.

4.0 INSPECTION AND ACCEPTANCE

The Facility Manager/BMETS is responsible for inspection and acceptance of all items, incoming shipments, documents, and services.

4.1 Acceptance Criteria

Certification by the Government of satisfactory goods or services provided is contingent upon the Contractor performing in accordance with the terms and conditions of the statement of work, this purchase order, and any modifications.

4.2 Invoice Review

The Facility Manager may reject or require correction of any deficiencies found in the invoice or receiving report. In the event of a rejected invoice or receiving report, the Contractor will be notified via WAWF by the Facility Manager of the specific reasons for rejection.

5.0 DELIVERABLES

5.1 Delivery Address

Delivery of items shall be made to the following location:

Wilford Hall Ambulatory Surgical Center Laboratory, Immunology Room 0B022, 1100

Wilford Hall Loop, Lackland AFB, San Antonio, TX 78236

Work to be primarily accomplished without impact to mission. Intrusive work must be accomplished outside of days/times Mon-Fri 0730-1630.

Delivery is required no later than: 60 days after date of contract with exceptions due to the

Pandemic.

5.2 Shipping

The Contractor shall make use of commercial best practices in the packing and shipment of

Packages. Offers may be submitted on the basis of f.o.b. destination and they will be evaluated on the basis of the lowest overall cost to the Government.

7.0 OTHER TERMS, CONDITIONS, AND PROVISIONS

7.1 Security/Installation Access:

Base passes for all contractor personnel will be requested by the 59 MDTS Laboratory.
