1. SCOPE OF WORK:

The contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, and other items and services necessary to complete the work as described in this Statement of Work. Work location is 180th FW/CE Toledo OHANG

2. AREA OF WORK:

Contractor shall work only in the areas designated by Contracting Officer Representative (COR).

3. DESCRIPTION OF WORK:

The work specified by this section consists of furnishing all labor, equipment, tools, appliances, transportation, materials, disposal and all documentation associated with the project for Building 850

Facility 850 Domestic Water Heating Upgrade:

· Domestic Hot Water shall utilize (1) boiler with storage tank technology to supply the facility with hot water.

· Contractor shall pull any boiler permits and final inspection tags per code.

· Contractor shall provide submittals for approval on all the equipment installed.

· Cap solar water heating system lines being fed from the solar panels located in the room.

· Demo and remove the existing storage tank and the on-demand water heater.

· Any additional piping relative to the solar shall be removed to provide room for the new equipment.

· Provide min. 99k BTU boiler and all condensation kits terminated to the nearest drain. The boiler shall have min. 10 yr warranty.

· Provide an indirect storage tank with min. capacity of 110 gallons with stainless steel construction offering commercial lifetime limited warranty.

· Provide boiler trim kits, air separator, backflow preventor, autofill and expansion tank sized accordingly and charged.

· Contracting shall supply and install the following:

· All piping from the existing supply and return lines to the boiler and storage tank.

· All electrical connections to facility power

· All venting from the unit to the existing roof penetration.

· All natural gas lines from the existing to the new unit.

· All fiberglass insulation on water lines.

· Contractors shall provide all necessary material and workmanship to make a functional hot water system.

· All programming and commissioning are the responsibility of the contractor.

· Closeout documents shall include OM Manuals, Warranty Paperwork and Ohio Dept of Commerce Boiler Cert.

· The warranty shall be 1 year on workmanship on all equipment.

· The contractor shall provide training to govt. personnel on the system.

· All demo materials shall be removed off base.

3.1 All work performed under this SOW shall be conducted in accordance with applicable state and federal laws and regulations, ANG policies and regulations, and this SOW as outlined below.

4. PERIOD OF PERFORMANCE:

Work must be completed in 10 days total contract.

Work hours are 7:00 AM to 4:30 PM Monday to Thursday with all contractors cleared with SF77 and badged.

5. MATERIALS:

Construction materials must meet all required NFPA, NEC and UL requirements.

6. MATERIAL SUBMITTALS:

The contractor shall submit to the CO for approval all items listed on attached Material Submittal Schedule, AF Form 66 within 10 calendar days of the notice to proceed - or - provide a copy of the AF Form 66 indicating when each material submittal shall be submitted for approval. Ensure that any disapproved material submittal will not impact the approved project execution schedule, as disapproved submittal shall not be considered grounds for a time extension. Allow for a minimum of 8 calendar days for the Government’s processing of material submittals. The contractor shall not order, deliver or install any materials without CO’s material approval or remove any materials on site, which have not been properly approved.

· Contractor shall submit their QA/QC plan along with their Safety/Health plan before any work will be started.

7. GENERAL CONDITIONS:

Contractor and all personnel shall comply with the Standard codes as required by the State of Ohio.

7.1. Contractor shall field verify all site conditions, quantities, measurements, and project requirements.

7.2. Contractor shall protect and barricade all work areas at all times to protect workers and the public from injury.

7.3. Contractor shall protect all structures, utilities, vegetation, sidewalks and vehicles from damage at all times. Replace all damaged materials with new materials to the satisfaction of the Government, at no additional cost to the Government.

7.4. Contractor shall apply for hot permit before starting any work. Base Fire Dept

7.5. Contractor shall use LOTO procedures for safety based on their company’s policy.

7.6. Contractor shall have fall protection PPE and training while operating lifts.

8. GOVERNMENT WORKING HOURS:

· See period of performance.

9. DIFFERING SITE CONDITIONS:

Bring any unforeseen project conditions to the attention of the CO immediately. Differing site conditions, which may alter the scope of work, shall be approved by the CO prior to such work being performed.

10. UTILITIES:

The Government shall provide at no cost to the Contractor reasonable amounts of water and electricity to support the contract. Hook up will be to existing outlets and will be the contractors responsibility.

11. DISPOSAL OF MATERIALS AND CLEANUP:

The contractor shall not use any government disposal bins, dumpsters or trash receptacles for any reason at any time. The contractor shall provide appropriate disposal bins/trash receptacles. Approval of disposal bins/trash receptacles location(s) shall be coordinated through government project manager. The contractor shall not allow debris to leave the work area due to potential damage to aircraft engines. Contractor shall coordinate with the base Environmental Management Office (EMO) prior to disposing any materials off base. All debris or trash shall be disposed of off base. All demolition debris shall be disposed of at a permitted landfill, with landfill authorization, unless other arrangements have been coordinated through the EMO. Provide proof of landfill disposal from the landfill that accepts the materials. This may be accomplished by providing the government project manager with landfill receipts; waste manifests, or weight tickets from the landfill that accepts the construction debris. Contractor shall clean up after each work period and dispose of all debris at the close of each day.

12. MATERIAL & EQUIPMENT ACCOUNTABILITY:

Notify the government project manager prior to delivery of any material or equipment to the site. The contractor shall ensure someone is available to receive delivered materials and able to store them safely in an area to be coordinated with the government project manager. Any materials delivered without prior coordination and without having someone to take receipt, shall not be accepted and turned away.

13. SECURITY:

Security Forces shall monitor the incoming and outgoing of work personnel during the duration of the project. All deliveries shall be coordinated with the COR to arrange for their entrance onto Base property. SF66 shall be submitted and approved for all contractors performing work on base.

14. ESCORTS:

A security briefing will be conducted prior to the start of the contract.

All Contractor personnel shall have a minimum of one (1) Form of Photo Identification with them at all times.
