STATEMENT OF REQUIREMENT

GENERAL INFORMATION

1. Title of Project: Autoclave Steam Sterilizer

2. Objective: To procure and fulfill necessary Autoclave Steam Sterilizer which will conform to standardization of equipment, to replace near end of life sterilizer

Scope of Work: The procurement of this equipment is intended to replace existing steam sterilizer that are near end of life. The solutions ensure instruments, trays, are properly sterilized and with newer sterilizers a better quality of instrument sterilization with a self-contained steam generator running off RO Water. The sterilizer will come with a new trolley and rack provided by the quote. The new sterilizer shall be added to the current service contract of the sterilizer that are being replaced. The company shall provide dates when units are shipped and details on what the facility should provide and the details of the installation. This includes all planning, execution, and training for staff. Upon receiving the final purchase order. The company that gets the contract will assign a project manager to act as the single point of contact in all matters involving the implementation. The Autoclave must be self-generating steam system.

GENERAL REQUIREMENTS

3. The Government and the Contractor understand and agree that the services to be delivered under this contract by the Contractor to the Government are non-personal services and the parties recognize and agree that no employer-employee relationships exist or will exist under the contract between the Government and the Contractor’s employees.

4. Hours of Work: This contract is a full-service contract to include all necessary support calls and services. The period of installation for the implementation of a Steam autoclave sterilizer units, from receipt of the purchase order. All work will be scheduled to complete with minimum downtime to the SPS department. Any modifications or extensions will be agreed upon between the company and Marion VA.

5. Place of Performance: The Steam Autoclave Sterilizers will be installed at the Marion VA medical center. The Autoclave company will perform the setup and hook ups, and training of all staff on how the units work.

Design quality and capabilities:

· The Autoclave must be self-generating steam system running with Reverse Osmosis (RO) Water critical water.

· The Autoclave steam sterilizer must work with current service contracts that are on current unit, or new one provided as a add on

· Company must provide training.

· Company on installation in quote provide all filters and descale system for the steam generator or water conditioning system.

· Must be all stainless-steel hook ups and components.

· Must come with a cleaning schedule or company to provide annual cleaning.

· Company must dispose of current sterilizer.

· Company must provide all that is needed prior to install. Space needed for install measurements electrical and all.

· Water consumption

· Must be able to process 16 (¾) size instrument trays.

6. Design considerations for new equipment:

· new trolley and rack provided per quote.

· Company must provide a onsite inspection to see if there is room for installation

7. Project Management and Execution: Upon the receipt of the PO the company that got the contract will be responsible for performing tasks throughout various stages of this project. The following is an overview of these tasks which will result in the successful completion of this project:

· Lead a kick-off meeting with Sterile Processing Department along with the engineers at the facility.

· Create and present a detailed project plan including a schedule.

· Inform SPS and facilities of delivery times and installation requirements.

· After installation training for the whole staff will be provided

CONTRACTING OFFICER'S REPRESENTATIVE (COR)

8. No Government personnel, other than the Contracting Officer (CO), have the authority to change or alter these requirements. The COR shall clarify technical points or supply relevant technical information, but no requirements in this scope of work may be altered as a sole result of such verbal clarification.

SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES

9. Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated.

a. The contractor will deliver and install and dispose of the current sterilizers. The contractor will offer customer service support regarding any issues or concerns with the equipment.

b. The contractor will conduct training for the utilization and troubleshooting in the Sterile Processing Department and end-users’ area for the Sterile Processing Service (SPS) staff and selected staff from the service lines, and Biomed staff.

Deliverable One: The life expectancy of this equipment, barring accidental misuse or abuse, would sustain the Sterile Processing department and staff for many years, and the importance of this equipment is to expand and improve the quality of services provided to Veterans.

Task Two: Provide Technical support that is available.

a. Provide repair service and repair parts/spare parts, as needed.

b. Provide maintenance as a service creates, designs, implements, and/or integrates customized changes to software that solve one or more problems and is not included with the price of the software. Software maintenance as a service includes person-to-person communications regardless of the medium used to communicate telephone support, online technical support, customized support, and/or technical expertise which are charged commercially.

SCHEDULE FOR DELIVERABLES

a. The shipment of deliverables will occur when equipment is ready. Upon receipt of PO to the winning contractor facility COR or SPS chief and Assistant Chief along with the facility engineer should be contacted with the delivery date, and the amount and hours of installation.

b. A copy of a packing slip with purchase order number will be furnished at the time the order is delivered.

c. Unless otherwise specified, the number of draft copies and the number of final copies shall be the same.

CHANGES TO STATEMENT OF WORK

Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
