COMBINED SYNOPSIS/SOLICITATION

FA4877-22-Q-A189

Massage Therapist Services Davis-Monthan AFB, AZ.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

The Request for Quotation (RFQ) number FA4877-22-Q-A189, Massage Therapist shall be used to reference any written quote provided under this RFQ.

The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06; Effective 1 May 2022.

This solicitation is being solicited as a small business set a side. NAICS Code 621399, with a small business size standard of $9M. All prospective offerors must be registered and active in the System for Award Management (SAM) at www.sam.gov. Lack of SAM registration will make an offeror ineligible for award.

The contractor shall provide all tools, supplies, equipment, transportation, labor, supervision, and management necessary for the 48 RQS Massage Therapy requirement, on Davis-Monthan AFB (DMAFB), Arizona. The government intends to award a base plus option years contract and requests quotes for following line items.

Notice to Vendor(s)/Supplier(s).

a. The Government reserves the right to cancel this RFQ, either before or after the closing date. In the event the Government cancels this RFQ, the Government has no obligation to reimburse a vendor for any costs.

b. The Government asks that all quotations are good for at least 180 days from the date of submission. Please acknowledge this request explicitly within all submissions.

c. Performance on/at Davis Monthan AFB facilities. The Government anticipates performance under contract to start in June 2022

Quotes Submissions: Quotes are due no later than, 14 June 2022, at 2:00 PM Arizona standard time. Offerors are responsible for verifying receipt of their quotes to this office before offer due date and time.

The Government will not provide contract financing for this acquisition. Upon award and satisfactory performance, payment will be through Wide Area Workflow.

Provisions 52.212-1, Instructions to Offerors – Commercial and 52.212-2, Evaluation – Commercial Items, apply to this acquisition along with the listed addenda’s to the provisions.

Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items. For offerors with a current registration in the System for Award Management (SAM) complete paragraph (b) of the provision and submit with the response to the RFQ.

FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (Jan 17), applies to this acquisition with the following Addendum: Paragraph (c) of this clause is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.

Note: The vendor acknowledges that should their quote terms and conditions and/or agreement conflict with mandatory provisions of the Federal Acquisition Regulation (FAR) and other Federal law applicable to commercial acquisitions, the vendors terms and conditions and/or agreement are unenforceable and are not considered incorporated into any resultant contract.

Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders – Commercial Items and selected clauses within the clause apply to this acquisition.

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (JUL 2021) –ADDENDUM

To ensure timely and equitable evaluation of the quotation, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation.

Offerors must be currently registered in the System for Award Management (https://sam.gov) and agree to remain registered while performing services in the support of Davis-Monthan AFB, AZ. The offerors response shall consist of a:

1. Technical Capability: All Offerors shall provide proof of a current massage therapist license or certification from the state of Arizona. Additionally, all massage therapists must have at least two years of experience and have a current certification in basic cardiopulmonary resuscitation (CPR) to include automated external defibrillator (AED) training. Lastly, all massage therapists must have a board certification in therapeutic massage and bodywork (BCTMB), rolfing, active release, trigger point, fascial manipulation and reiki. All certifications must be kept current throughout the duration of the contract.

2. Pricing: Offerors shall provide pricing for all Line Items. The extended amount for each Line Item must equal the unit price multiplied by the number of units. It is imperative the mathematical calculations are correct. Numeric rounding shall be limited to two decimal places. Offerors shall clearly state if any discount terms are offered.

3. Fill-In Clauses/Provisions: Offerors are required to complete all clauses/provisions that require Fill-ins and submit them with their quote. Note: For 52.212-3, If the Offeror has completed the annual, representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. Only paragraph (b) of the provision is required.

4. Provision 52.222-52 Fill In: All Offerors shall submit a filled out provision 52.222-52 in order to be considered for award.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014) – ADDENDUM (Using 13.106 Procedures)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(i) technical capability of the item offered to meet the Government requirement;

(ii) price;

BASIS FOR CONTRACT AWARD: Two factors will be used to evaluate offerors’ quotes: technical acceptability and price.

Contract award will be made to the offeror who submits the lowest total evaluated price quote achieving a rating of “acceptable” for the Technical Acceptability factor. Past performance will be evaluated to assist in determining contractor responsibility in accordance with FAR 9.104-1 but will not be included as a sub factor or rated. By submission of its offer, the offeror accedes to all solicitation requirements including: terms and conditions, representations and certifications, technical requirements, and the evaluation factors and sub factors contained herein. Failure to meet a requirement may result in an offer being determined unacceptable for award. The offeror must clearly identify any exception to the solicitation terms and conditions with accompanying rationale. The evaluation shall proceed as follows.

1. Technical Acceptability – The Government technical evaluation team shall evaluate the technical quotes at the factor level, assigning an “Acceptable” or “Unacceptable” rating as defined below.

Acceptable – The quote clearly meets the minimum requirements of the solicitation and Performance Work Statement. In order to be technically acceptable, the Offeror must provide all current certifications as stated above in the addendum to 52.212-1 Unacceptable – The quote does not clearly meet the minimum requirements of the solicitation and Performance Work Statement. Any Offerors that are unable to provide current certifications at the time of bid will not be considered for award.

2. Price Evaluation – The Government will evaluate the offeror’s price quote, to include all options, for price completeness and reasonableness as described below.

(a) Completeness – The offeror’s price quote will be evaluated for completeness by confirming the offeror has provided unit and total price for all CLINs.

(b) Reasonableness - The offeror’s price will be evaluated for reasonableness through one or more of the methods specified in FAR 13.106-2 & 3.

(c) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(d) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

Discussions: The government intends to award a purchase order without discussions with respective vendors/quoters. The government however, reserves the right to conduct discussions if deemed in its best interest.
