Pricing shall be submitted on a per-job basis for each building's scope of work, exclusive of state and federal taxes, with unit prices prevailing over extended prices. The vendor must be registered with the State of South Carolina on SCEIS and is required to maintain commercial general liability insurance of one million dollars with the South Carolina Forestry Commission named as co-insured. The contractor must be insured, bonded, and able to provide three references for completed work. Vendors may request South Carolina resident contractor preferences or resident subcontractor preferences (2% or 4%), provided they meet eligibility requirements. Upon notice to proceed, the contractor shall have fifteen working days to mobilize and fifteen working days to complete the project. The contractor must comply with the South Carolina Illegal Immigration Reform Act, Title 8, Chapter 14, applicable workers' compensation requirements, and must remove all construction debris according to Darlington County Ordinances and South Carolina DHEC standards. A one-year warranty on workmanship and labor is mandatory.

INSTRUCTIONS TO OFFEROR

*Individual signing quote must be authorized to obligate the submitting vendor.

*Please quote lowest prices, exclusive of State and Federal Tax, on the below listed services.

*All items quoted shall be new unless otherwise stated herein.

*Award will be to the lowest responsible responsive bidder for all items requested.

*Quotation documents must be clearly marked with this "Quotation Number and Closing Date".

*If your envelope does not indicate Quotation Number and Closing Date, your quotation may be unidentifiable, lost or misplaced, and therefore not considered for award.

*Prices shall be firm for thirty (30) calendar days from Closing Date.

*Payment terms will not be a factor in award.

*The South Carolina Forestry Commission reserves the right to reject any and all quotations, and to waive any and all formalities and technicalities, and also reserves the right to reject any quotation where delivery time indicated by vendor will result in delay deemed unsatisfactory to the Department.

*Note to Vendor: By signing and submitting a quote, you are certifying under penalties of perjury that you do not have any liens as described in Section 12-54-120 of the S.C. Code of Laws 1976 as amended, relating to payment of taxes.

TERMS AND CONDITIONS Continued All amendments to and interpretation of this RFQ shall be in writing. The procurement officer shall not be legally bound by any amendment or interpretation that is not in writing.

Payment will be made in accordance with Section 11-35-45 of the South Carolina Consolidated Procurement Code and Disbursement Regulations. Delay in receiving invoices, as well as errors and omissions on the invoices, will be considered just cause for withholding payment without losing discount privileges. The Forestry Commission reserves the right to withhold payment or make such deductions as may be necessary to protect the Forestry Commission from loss or damage because of defective work, claims, damages, or to pay for repair of correction of materials furnished hereunder.

Quoted prices must remain firm for a period of thirty days beyond the RFQ deadline. Unit prices will govern over extended prices unless otherwise stated.

All materials and products offered must be guaranteed to meet and comply with the requirements of all specifications, terms, and conditions indicated in this solicitation.

Award will be made in accordance with Section 11-35-1550 of the South Carolina Consolidated Procurement Code.

The Forestry Commission reserves the right to: (1) reject any and all quotations and to cancel the solicitation; (2) waive any and all technicalities; (3) reject any quotation in which the delivery time indicated is of substantial length to cause disruption and/or delay in operation for which the item(s) is/are intended; (4) reject ambiguous quotations which are uncertain as to terms, delivery, quantity, or compliance with specifications.

The successful bidder assumes sole responsibility and shall hold harmless the Forestry Commission, its directors, officers, employees, and agents from and against any and all claims, actions, or liabilities of any nature which may be asserted against them by third parties in connection with the performance of the successful bidder, its directors, officers, employees, and agents under this agreement..

Contractor agrees not to refer to award of this contract in commercial advertising in such a manner to state or imply that the products or service provided are endorsed or preferred by the user.

Upon award of a contract under this solicitation, the person, partnership, association, or corporation to whom the award is made must comply with the laws of South Carolina that require such person or entity to be authorized and/or licensed to do business in this State. Notwithstanding the fact that applicable statutes may be exempt or exclude the successful offeror from requirements that it be authorized and/or licensed to do business in this State, by submission of this signed quote, the bidder agrees to subject itself to the jurisdiction and process of the courts of the State of South Carolina as to all matters and disputes arising or to arise under the contract and the performance thereof, including any questions as to the liability for taxes, licenses, or fees levied by the State.

SCOPE OF WORK:

BUILDING “A”

Replace existing windows with Energy Efficient Vinyl replacement windows.

Repair damaged ceiling.

Paint all ceilings and interior walls, doors, and trim.

Replace existing 4’ Fluorescent lights with 4’ LED fixtures.

Replace existing toilets.

Remove rear steps and add a 4’ X 4’ porch with steps and handrails.

BUILDING “B”

Replace exterior door at handicap ramp. New door to include new lock keyed to match the other doors.

Repair subfloor and underlayment under and around exterior door at handicap ramp.

Repair damaged siding and trim around door on handicap ramp. Stain wood to match existing siding color.

Repair a 4’ x 4’ area of damaged ceiling in hallway.

Remove and replace handrail on right side of handicap to fit with new added roof section.

Add an approximately 8’ X 8’ gable porch roof to match existing building. (see specs and attached sheet for details)

SPECS

BUILDING “A”

Windows-

· Windows shall have Double Pane Low-E Glass and meet all Energy Star ratings.

· Windows shall be (White) single hung tilt windows with no grids.

· The contractor shall be responsible for removing any interior trim work necessary for this installation and then re-installing it. The contractor is also responsible for any damaged paint or stain on interior.

· Windows shall have half screens.

· Contractor to provide Lifetime Warranty on windows and one year warranty on all labor.

Paint- Ceilings to be ceiling white.

Walls to be eggshell (owner to select color) Trim to be semi-gloss white.

Doors to be semi-gloss white.

Lights- New lights to be Lithonia Lighting FMLWL LED Wraparound light. (2900 Lumens) or equivalent.

Toilets(2)- White, American Standard Elongated Standard Height, WaterSense Labeled.

Rear deck-

Floor framing to be 2”x 8” PT.

Flooring and steps to be 2” x 6” PT.

Posts to be 4”x 4” PT.

Step Stringers to be 2”x 12” PT Hand rails to be of 2”x 4”, 2”x 6” 2”x 2”.

Building “B”

Door- Door to be 3/0 wooden door with new handle set and keyed to match the rest of the building.

Gable Roof-

Posts to be 6”x 6” PT.

Posts to be set in saddle that is attached to concrete sidewalk.

Posts to be notched for beam to secure from “uplift”.

Beams around perimeter to be a minimum of 2”x 8” SYP or SP.

Rafters, ceiling joists, and ridge to be 2”x 6” SP Pitch to be 4/12.

Soffit to match height and look of existing soffit.

Ceiling of porch to be 3/8” plywood.

Shingles to match existing roof.

Install roof over ridge vent.

See attached sheet for more details.

Miscellaneous

· Vendor to provide one (1) year warranty on workmanship and labor

· All construction debris to be removed according to Darlington County Ordinances and SC DHEC standards.

· Upon notice to proceed, contractor shall have 15 working days to proceed with project.

· Vendor shall have 15 working days to complete project.

· Vendor to be insured and bonded.

· Vendor to provide Certificate of Liability and Workman’s Comp information upon request.

· The Vendor agrees to comply with “The South Carolina Illegal Immigration Reform Act, 2008”.

· (An overview is available at http://www.procurementlaw.sc.gov/immigration/MMO-legal-immigration.phtm) By signing your offer, you certify that you will comply with the applicable requirements of Title 8, Chapter 14 of the South Carolina Code of Laws and agree to provide to the State upon request any documentation required to establish either: (a) that Title 8, Chapter 14 is inapplicable to you and your subcontractors or sub-subcontractors; or (b) that you and your subcontractors or sub-subcontractors are in compliance with Title 8, Chapter 14. Pursuant to Section 8-14-60, "A person who knowingly makes or files any false, fictitious, or fraudulent document, statement, or report pursuant to this chapter is guilty of a felony and, upon conviction, must be fined within the discretion of the court or imprisoned for not more than five years, or both." You agree to include in any contracts with your subcontractors language requiring your subcontractors to (a) comply with the applicable requirements of Title 8, Chapter 14, and (b) include in their contracts with the sub-subcontractors language requiring the sub-subcontractors to comply with the applicable requirements of Title 8, Chapter 14. · The Contractor must be able to provide proof of COMMERCIAL GENERAL LIABILITY insurance for the amount of ONE MILLION DOLLARS ($1,000,000) with the SC Forestry Commission named as co-insured.

· The Contractor agrees to fully comply with SECTION 42-1-10, also known and cited as “The South Carolina Workers’ Compensation Law”.

Contractor must provide at least 3 references for completed work in relation to job requested in Scope of Work.
