STATEMENT OF WORK
Remove and Replace Flooring at Building 90
Waco VA Project
Requirements
The contractor shall be responsible for providing all labor and equipment required for the services to Remove and Replace approximately 2528SF of flooring inside Building #90 1st f loor/basement hallway and select rooms. All work performed will be done at the Doris Miller VAMC in Waco, at 4800 Memorial Drive, Waco, Texas 76711. After all work is completed, the contractor will have a walkthrough with a VA representative to ensure all work is completed and ready for invoicing.
The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirement herein contained.
Note: “The C&A requirements do not apply, and that a Security Accreditation Package is not required for this contract.”
Project Requirements
Demo of Existing Tile Flooring
1. The contractor shall provide all barriers, signage and safety equipment necessary to prevent VA employe and patient access during construction.
2. The contractor shall provide the COR and VA tenant sample of tile and base to be installed prior to beginning any work. Additionally, the contractor shall provide (3) layout patterns sketches. The proposed layout should include one base/body tile and one accent/boarder tile. The COR and tenant shall make a selection and provide written approval prior to the contractor beginning their work.
3. Prior to beginning demolition work the contractor shall test existing tiles for asbestos.
If asbestos in identif ied the contractor shall take the proper abetment action.
4. The contractor shall install dust barriers, close and tape off supply/return HVAC (Heating Ventilation and Cooling) register during the duration of the work.
5. Dust abatement shall be provided during the demo process, and daily cleaning performed. Contractor shall utilize the proper Personal Protective Equipment (PPE) while performing this work.
6. Remove approx. 480lnft of existing base, and 2528sqft of tile exposing the existing sub floor. When removing base material, do so carefully to preserve the integrity of the gypsum wall board below (GWB). Dispose of all waste materials in compliance with local, state and federal requirements.
Installation of New Tile
1. Contractor shall repair and clean bottom of wall where base material was removed.
Areas where the GWB has be damage should be repaired and prepped to receive new base material.
2. Clean and remove all debris form the exposed subfloor. Apply primer, then self-leveling material over the entire floor area.
3. Once self-leveling compound has cured apply moisture sealant and prep for tile installation.
4. Place tile matching existing floor pattern. Install f loor transition between dissimilar materials and threshold. All transition must comply with current ADA standards.
5. Install new base material along repair area.
6. Contractor shall clean the entire work area wiping down walls, doors, windows and light f ixtures. Remove HVAC protection, wipe down registers and reopen vents to operating status.
2. Government Holidays:
(a) The Government hereby provides notice and Contractor hereby acknowledges receipt that Government personnel observe the listed days as holidays:
New Year's Day January 1st Martin Luther King's Birthday Third Monday in January Washington’s Birthday Third Monday in February Memorial Day Last Monday in May Juneteenth 19th of June Independence Day July 4th Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11th Thanksgiving Day Fourth Thursday in November Christmas December 25th
(b) In addition to the days designated as holidays, the Government observes the following days:
a. Any other day designated by Federal Statute
b. Any other day designated by Executive Order
c. Any other day designated by the President’s Proclamation
It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.
3. Submit the proper documentation and reports to receive payment on this service.

A. Invoices and Payment
a. Payment to be made monthly, in arrears, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.
b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten‐network.com/us/en/veterans‐affairs/. Additional information regarding Tungsten shall be provided upon award.
c. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.
d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.
i. Time period being invoiced for.
ii. Description of services performed.
iii. Unit Cost billed.
iv. Extended amount due.
v. Invoice number, date.
vi. Contract and obligation numbers.
Other Considerations
• The contractor shall be responsible for the verification of all requirements for this service.
All measurements and conditions must be verif ied by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the Contracting Officer’s attention.
• The contractor’s Period of Performance (POP) is 90days from the NTP.
• Majority of the work shall be performed during normal working hours unless other wised noted by the COR. Normal working hours are 8:00 am – 4:30 pm, Monday – Friday.
• The contractor’s supervisor shall always be available to address any needs the VA COR may have during the POP which is 1 year.
• Contractor shall provide signage, fencing/barriers, ground guides, flagging and PPE as required to complete the work as described in this Scope of Work (SOW).
Codes and Standards
The project shall be in accordance with the latest International Building Code (IBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), ASTM Standards, American Concrete Institute (ACI), VA Specifications/Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.
The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004‐012 and all VA Center Policy Memorandums, including but not limited to the following:
(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.
(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on‐site “competent person” have completed the OSHA 30‐hour construction worker course and that all other general and sub‐contractor’s workers have completed the OSHA 10‐hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verif ication of completion of training will be required prior to performing work.
(c) Compliance with EPA and State environmental regulations.
SAFETY PLAN REQUIREMENTS
Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.
The safety plan shall include, but not limited to the following:
A. Safety and Health Policy (i.e., company policy) B. Safety and Health Objectives (i.e., company goals, procedures, etc.)
C. Job Site Inspections (i.e., safety inspection/potential hazards/prevention, etc.)
D. Contractor Safety Administrator (i.e., competent person/safety officer) E. Personal Protective Equipment (i.e., head, hearing, and eye protection, footwear, clothing, gloves, etc.)
F. Safety Discipline (i.e., correction of violations, etc.)
G. Hot Work Permit (i.e., responsibilities, procedures, etc.)
H. Emergency Procedures (i.e., responsibilities, procedures, etc.)
I. Clothing
• PPE is available and properly worn.
• Clothing is relatively dust free with the construction area and patient areas.
