This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
Solicitation number is 36C24821Q0892 and is issued as a Request for Quote (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal
Acquisition Circular 2021-05, effective 10 March 2021. This solicitation starts 28 May 2021 and will end 18 Jun 2021 at 4:30 pm EST.
North Florida South Georgia Healthcare System, Network Contracting Office 8, Regional
Procurement Office - East (RPO-E) intends to award a Firm Fixed Price service contract for
Annual DI Water System Maintenance for two (2) systems and one (1) Vascular Lab Humidifier
Water Treatment System located at the Malcom Randall VAMC, Gainesville, FL. This procurement is being conducted under FAR Subpart 13 and simplified acquisition procedures apply. Period of performance is 1 August 2021 – 31 July 2021. This is a base year with four (4) one-year option requirement. The North American Industrial Classification System (NAICS) code for this procurement is 221310 with a small business size standard of $30 million.
This procurement is unrestricted; and if award cannot be made, the solicitation will be cancelled, and the requirement resolicited.
This solicitation will use Comparative Evaluation. Award will be made to the offeror determined to be acceptable, price reasonable, and is determined the best value to the government; all eligible offerors may submit quotes. All offerors shall be registered in the System for Award
Management (SAM) at https://www.beta.SAM.gov ; and www.vip.vetbiz.va.gov , if applicable, https://www.beta.sam.gov/ http://www.vip.vetbiz.va.gov/ to be considered for an award of a federal contract. The record shall be complete and not missing elements such as representations and certifications; and SDVOSB shall be certified.
Site Visit. A site visit will be held on 10 June 2021 at 10:00am EST at the Malcom Randall
VAMC, Engineering Room, 1601 SW Archer Rd, Gainesville, FL 32608. The Point of Contact
(POC) for this visit is Barry Varn.
Wage Determination. The Wage Determination for this requirement will cover the following counties: Alachua. Wage determination can be extracted from the Department of Labor at https://beta.sam.gov.
Contractor Request for Information (RFI). All questions regarding this RFQ shall be in writing and may be sent by e‐mail to reginald.nettles@va.gov NLT 11 Jun 2020 by 4:30pm EST. No further questions will be accepted after that date and time. Answers to contractor questions will be posted on Contract Opportunity no later than 15 June 2021 by 12:00PM Eastern.
Quote Submission. Contractors interested in providing these services should submit their quotes via email to Reginald.Nettles@va.gov . Quotes shall be received NLT 18 June 2021 at 4:30 PM
Eastern Time Zone and reference the solicitation number. Quotes received after this date and time will be considered late in accordance with FAR Provision 52.212-1(f) and will not be evaluated unless the Contracting Officer determines the late quote will be accepted IAW 52.212-
1(f)(2)(i)(A-C).
The Government will award a Firm Fixed-Price contract resulting from this RFQ to the responsible contractor whose quote is determined to be acceptable, price reasonable, and determined to be the best value to the government in accordance with a Comparative Evaluation of the information requested in this solicitation at FAR Provision 52.212-2.
SCHEDULE OF SERVICES
The North Florida South Georgia Healthcare system requires Annual DI Water System
Maintenance for two (2) systems and one (1) Vascular Lab Humidifier Water Treatment System located at the Malcom Randall VAMC, Gainesville, FL. The contractor shall furnish all labor, materials, parts, supervisions, services and incidentals to perform Annual maintenance of two (2)
DI Water Systems and one (1) Vascular Lab Humidifier Water Treatment System as described in the attached Statement of Work (SOW). Contractors shall quote prices for each Line Item

52.212-2 Evaluation-Commercial Items (OCT 2014)
QUOTATION SUBMISSION
1.1 The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2(b)(3), where quotations will be compared to one another to determine which provides the best benefit to the Government.
1.2 The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s).
1.3 Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation.
1.4 The Government will award a contract resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price considered. The following factors shall be used to evaluate quotes: Technical Approach, Past Performance and Price.
Failure to provide ALL requested items is reason to be determined unresponsive.
1.4.1 Technical Approach: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government’s requirements as outlined in the solicitation and based on the information requested in the Instructions below:
a) Provide methodology to be used to meet the requirements of the
SOW for maintenance of two (2) DI Water Systems. Provide details regarding each area to be serviced to include but not limited to: service schedules, products to be used, hazardous material disposal processes, and reporting processes.
b) Provide a list of personnel assigned to provide the titled service.
1.4.2 Past Performance: The past performance evaluation will assess the relative risks associated with a quoter’s likelihood of success in fulfilling the solicitation’s requirements as indicated by the quoter’s record of past performance. The past performance evaluation may be based on the contracting officer’s knowledge of and previous experience with the service being acquired; Contractor Performance
Assessment Reporting System (CPARS) at http://www.cpars.gov/ ; or any other reasonable basis; or the quoter may provide three (3) references of same scope and cost of work requested.
1.4.3 Price: The Government will evaluate the price by adding the total of all line item prices, including all options. The Total Evaluated Price will be that sum.
1.4.4 Options. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement.
This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by adding six months of the quoter's price for the last potential period of performance under this contract to the quoter's total price. For example, if the contract includes option periods that are exercisable under FAR 52.217-9, the quoter's total price for the purpose of evaluation will include the base period, all option periods, and an additional six months of the last option period. Quoters must price only the base and option periods specified in the contract line item numbers (CLINs) and shall not submit a price for the potential six-month extension of services under FAR 52.217-8. The Government reserves the right to exercise the option under FAR 52.217-8 before the end of any of the contractor's performance periods and will be exercised at the price and/or rates in effect at the time the clause is exercised. Evaluation of options shall not obligate the
Government to exercise the option(s).

System for Award Management (SAM) accessed through https://www.sam.gov .

52.217-8 Option to Extend Services. (Nov 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract.
These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months.
The Contracting Officer may exercise the option by written notice to the Contractor within 14 days.
NOTE: The specified rates under this clause will be those rates in effect under the contract each time an option is exercised under this clause.
52.217-9 Option to Extend the Term of The Contract (Mar 20000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5yrs.
