STATEMENT OF WORK

Biohazardous and Chemical Waste Disposal

A. Background Information

NIAID Vaccine Immunology Program (VIP), Vaccine Production Program Laboratory (VPPL) and Immunology Laboratory (IML) requires packaging, transportation, and disposal of Biohazardous and Chemical waste materials to a federally approved hazardous waste disposal site(s).

Period of Performance (POP): One (1) 12-month period

Quotes/proposals must be broken down into three separate quotes. One quote for VIP, one quote for VPPL and one quote for IML. Please see Bill of Materials for specifics. Service will be scheduled at a minimum twice a week for Biohazardous waste and as needed for chemical waste. In both instances additional pickups will be as needed. Finally, any and all additional cost must be specified, to include fuel surcharges.

B. Contractor Requirements

- Contractor shall follow all state and federal requirements for the handling, packaging, manifesting, transportation and disposal of Biohazardous and Chemical waste.

- Furnish all labor, materials, and equipment necessary to render complete removal of Biohazardous and Chemical waste.

- The Contractor shall supply all bags, burn boxes, labels and appropriate containers for storing/transporting and disposal of Biohazardous and Chemical waste.

- The Contractor is required to dispose of the waste in such a manner as not to cause conditions detrimental to public health or to constitute a public nuisance.

- The contractor will assume full responsibility for compliance with all Federal, State, City, and County Laws, Rules, and Regulations governing removal and disposal of Biohazardous and Chemical waste.

- Infectious Waste material to be disposed of shall consist of the following:

o Sharps, pipettes, pipette tips, broken glass, and anything with the potential of puncturing a waste bag and causing injury.

o Wastes that were in contact in any type of lab work, including collection containers, culture dishes, slides, plates and assemblies of diagnostic tests and devices used to transfer, inoculate, and mix cultures.

- Scheduled pick-up/deliveries are as follows: A minimum of twice a week for biohazardous waste and as needed for chemical waste. In both instances additional pickups will be as needed.

- Maintain any and all licensing and regulatory compliance certifications required to transport and destroy Biohazardous and Chemical waste.

C. Contract Requirements - Handling/transporting

- Contractor shall properly label waste boxes/containers for disposal.

- Contractor shall track waste disposal boxes and provide government with certificate of destruction

- Contractor shall be responsible for the security of all materials in transit such to protect materials from damage or theft while they are in their possession.

D. Additional Contractor Requirements - Must be received on or before RFQ closing.

- Contractor shall provide a short synopsis of demonstrated experience in the safety, handling/packaging, transporting, and disposal of biohazardous and chemical waste.

- Contractor shall provide a short synopsis of safety, handling/packaging, transporting, and disposal training that are provided to their personnel.

- Provide three (3) references

E. Contractor - Personnel:

- Drivers must possess the appropriate certifications/training to transport and dispose of Biohazardous/Chemical waste.

F. Place of Performances:

NIH / NIAID

G. Hours of Operations:

- The contractor is responsible for conducting routine business, between the hours of 8:00AM – 6:00PM Monday thru Friday.

H. Government Responsibilities

- Correctly segregate and store in appropriate waste containers biohazardous and chemical waste awaiting disposal.

- Secure all biohazardous and chemical waste in central storage locations awaiting disposal pickup.

I. Inspection and Acceptance Requirements

- Certificates of destruction will be provided to the government.

- All work shall be completed in accordance with industry standards.
