About this file
This document contains a past performance survey template for evaluation of an offeror's past performance on a federal contract opportunity. The survey requests ratings on quality of service, schedule, cost control, business relations, and management of key personnel for a past contract. It includes definitions for adjectival ratings of exceptional, very good, satisfactory, marginal, and unsatisfactory. The survey is to be completed by a reference for a specific contractor or key personnel for a prior contract with the contract and project information provided by the offeror.
The related federal contract opportunity is a solicitation from the Department of the Treasury Bureau of the Fiscal Service on behalf of the Department of Housing and Urban Development Office of Inspector General. It requests proposals for medical and drug testing services as described in the attached performance work statement. Proposals are due by June 3, 2021. The award will be a fixed price contract awarded on a best value basis and subject to the supplemental terms, conditions, and clauses in the solicitation.
