About this file
This document contains questions and answers regarding a solicitation for emergency room physician services. The solicitation is seeking emergency room physicians on a contract basis to provide services at the Parker Indian Health Service hospital in Arizona. The hospital currently has several full-time staff emergency room physicians but requires additional contract support that is not guaranteed to be 24/7/365. Shift schedules are typically provided to contractors 2-3 months in advance but there can be exceptions for unplanned losses of staffing. The emergency room currently sees 25-30 patients per day on average, with day physicians typically seeing 10-15 patients over 12 hours and night physicians typically seeing 5-10 patients over 12 hours. Staffing varies but mid-level providers only currently assist with lower acuity patients up to 8 hours per day on certain days of the week. On-site housing may occasionally be available for physicians. The credentialing process to onboard typically takes approximately two months for board certified emergency physicians with no adverse reports.

Questions and Answers:
1. Is the contractor guaranteed hours for 24 hours a day for 365 days? Or are the shifts assigned randomly where available shifts are sent to contractor and then contractor physician has to fill?
WE HAVE SEVERAL FULL-TIME FEDERAL STAFF ER PHYSICIANS and WE
CURRENTLY DO NOT REQUIRE CONTRACTOR ASSISTANCE 24/7/365 BUT WE
DO INFORM THE CONTRACTOR OF OPEN SHIFTS - THE OPEN SHIFTS ARE
TYPICALLY SEVERAL IN A ROW TO FACILITATE CONTRACTOR INGRESS
AND EGRESS
2. How far ahead of the schedule is done typically?
TYPICALLY 2-3 MONTHS BUT THERE ARE EXCEPTIONS WITH UNPLANNED
OR UNSCHEDULED LOSSES IN STAFFING
3. What is the current volume of patients in ER per day or per year? ER CURRENTLY IS
SEEING 25-30 PATIENTS PER DAY
4. How many patients are seen by ER physician in 12 hours? (list separately between day and night) THERE IS SIGNIFICANT VARIATION BUT THE DAY PHYSICIAN
WILL TYPICALLY SEE 10 - 15 IN 12 HOURS AND THE NIGHT PHYSICIAN WILL
TYPICALLY SEE 5 - 10 IN 12 HOURS. FLU SEASON AND OTHER FACTORS
CAN DOUBLE THE NUMBER OF PATIENTS SEEN -- EARLIER THIS WEEK THE
ER HAD 40 PATIENTS IN ONE DAY
5. How many providers and midlevel's staffed during day and night? (list separately between day and night) MIDLEVEL STAFFING IS ONLY BEING PROVIDED TO
ASSIST WITH LOWER ACUITY PATIENTS AND THE MLP AVAILABILITY IS
VARIABLE, TYPICALLY ONLY DAY SHIFT, EARLY IN THE WEEK AND
CURRENTLY 8 HOURS PER DAY ON THOSE DAYS WHERE ASSISTANCE IS
EXPECTED TO BE REQUIRED. CURRENTLY WE DO NOT STAFF ANY MLP
ASSISTANCE OVERNIGHT.
6. Is there on-site housing provided for physicians? (ON-SITE HOUSING IS
OCCASIONALLY AVAILABLE – NOT SURE IF CONTRACTOR(S) CAN
REGULARLY AVAIL THEMSELVES DUE TO SAME)
7. What is the current credentialing time to on board? MOST OF THE CREDENTIALING
DELAY HAS TO DO WITH BACKGROUND CHECK CLEARANCE AT THE AREA
OFFICE AS WELL AS VERIFICATION OF PREVIOUS AFFILIATIONS /
PREVIOUS PLACES OF EMPLOYMENT. GAPS REQUIRE EXPLANATIONS AS
DO ALL ADVERSE PROVIDER DATA BANK ENTRIES. ALL PROVIDERS ARE
REVIEWED BOTH LOCALLY AND AT AREA OFFICE CREDENTIALING PRIOR
TO BEING SUBMITTED FOR APPROVAL. ACCORDINGLY, THE INDIVIDUAL
PROVIDERS WILL NEED TO BE RESPONSIVE TO REQUESTS FOR
ADDITIONAL INFORMATION, FINGERPRINT CARDS, IDENTIFICATION,
CERTIFICATES, CME, ETC. ANY REQUIRED LIFE SUPPORT TRAINING CARDS
MUST BE CURRENT AT TIME OF CREDENTIALING REVIEW IN ORDER FOR
THE PROVIDER TO BE CONSIDERED AND ALL PROVIDERS MUST BE
CURRENT IN BLS. CURRENTLY THIS PROCESS IS RUNNING ABOUT TWO
MONTHS FOR INDIVIDUALS WHO ARE BOARD CERTIFIED IN EMERGENCY
MEDICINE AND HAVE NO NEGATIVE PROVIDER DATA BANK ENTRIES
