Request for SF330s (Qualifications)
VA Office of Construction and Facilities Management (CFM) ‐ Central Region is issuing this public announcement for Request for SF330s (RFI: CFM‐24‐000010). This is not a request for a fee proposal.
The request is for Architect/Engineer (A/E) services for a Facility Condition Assessment (FCA) for VISN 5.
A Facility Condition Assessment (FCA) is a tool used to obtain essential data. An FCA is a visual inspection of the technical and physical conditions of a VA Medical Center facility. The firm submits a report detailing identified corrective actions. FCAs are in place for all VAMCs locations and updated every three years.
Set‐Aside: 100% set‐aside for Service‐Disabled Veteran‐Owned Small Businesses (SDVOSBs)

Estimated Magnitude of Contract: between $1,000,000.00 and $2,500,000.00
Please note: the firm selected shall have an experienced multi‐disciplinary team in either the governmental or commercial sector for projects of this magnitude or larger.

PROJECT DESCRIPTION: The contractor shall perform an independent facility condition assessment. The assessment will consist of tours, interviews, inspections, and research at each of the following VA
Medical Centers with the purpose of preparing a Facility Condition Assessment for VISN 5. Presently, the
FCA database for VISN 5 contains approximately 6,138,035 gross square feet. This assessment includes approximately 274 buildings. Some variation of the detailed building lists within the square footage and number of buildings may be encountered during the site assessment that may deviate up to 5% and shall be included in the assessment. The FCA shall be for the following site list:

Baltimore, Loch Raven, MD
Beckley, WV
Clarksburg, WV
Huntington, WV
Martinsburg, WV
Perry Point, MD
Washington, DC
The contractor shall perform an independent facility condition assessment. The assessment will consist of tours, interviews, inspections, and research at each of the following VA Medical Centers and/or NCA location with the purpose of preparing a Facility Condition Assessment report. Example of the tasks includes but not limited to: Provide consulting, assessment, and inspection services utilizing experts skilled in appropriate technical areas necessary for the evaluation of existing VA facilities and/or NCA locations. (Reports will indicate all systems, conditions, life expectancies, etc.)
Some variation of the detailed building lists within the square footage and number of buildings may be encountered during the site assessment that may deviate up to 5% and shall be included in the assessment.
The contractor shall provide a field team which shall visit each site as described in the detailed schedule.
The recommended minimum field team make‐up shall be as follows:
Architect to assess architectural and landscape issues
Electrical Engineer to assess electrical issues
Mechanical Engineer to assess mechanical, plumbing and steam generation issues
Elevator/Transport Specialist
Structural Engineer to assess structural and seismic issues where required
Cost Estimator to establish correction costs
Civil Engineer to address any civil issues. The use of a civil engineer is optional.
Period of Performance: 360 Calendar Days after Notice to Proceed.
BASIS FOR AWARD: The first phase of the award process is the Request for SF330s (Qualifications). A technical review and assessment will be performed by the AE Evaluation Board to determine the three highest qualified firms based upon the evaluation factor responses described below. The CO will then issue an RFP (to the best qualified firm) and upon a determination of fair and reasonable pricing, a contract will be awarded.
EVALUATION FACTORS: selection criteria will be based on the following factors, in their relative order of importance in accordance with FAR 36.602‐1 and VAAM M836.602‐1:
Evaluation Factor 1 – Relevant/Specialized Experience (Page Limitation ‐ 1 Page per Project)
Offeror shall provide a Summary Page of its recent, relevant, and specialized experience in Facility
Condition Assessments (or similar planning effort) showing technical competence which is similar in size, scope, and magnitude to this requirement. “Relevant” is defined as those task requirements identified in the Scope of Work in the geographical area. “Recent” is define as services provided within the past 5
(five) years. Offeror shall provide at least 3 (three) projects. Each project shall include the following.
a. Project title, location and detailed narrative describing the scope of services provided including the type of work performed by the offeror and its role in the project (i.e., prime contractor, teaming partner or subcontractor).
b. Project Owner, owner’s point of contact including telephone number and email address.
c. Services & Deliverables provided under the contract/task order
d. Period of Performance, including start and completion dates
e. Total dollar value of the project.
Evaluation Factor 2 – Professional Qualifications / Team Qualifications
Offeror shall provide professional qualifications necessary for satisfactory performance of required services and the specific experience and qualifications of personnel proposed for assignment to the project and their record of working together as a team. Additionally, provide a brief explanation of duties to be performed to accomplish the scope of work for each personnel.
The offeror shall describe the professional qualifications, registrations, licensure and experience of the following: Project Manager with a desired 5 years of experience Licensed Architect with a desired 5 years of experience Licensed Electrical Engineer with a desired 5 years of experience Licensed Mechanical Engineer with a desired 5 years of experience Elevator/Transport Specialist with a desired 5 years of experience Structural Engineer with a desired 5 years of experience Cost Estimator with a desired 5 years of experience Licensed Civil Engineer, if applicable, with a desired 5 years of experience
Please note: the lead designer/professional in each discipline must be registered but does not have to be registered in the particular state(s) where the assessments are located. It’s highly encouraged for the
SF330 response to include an organizational chart showing the proposed team. Additionally, ensure the
SF330 response includes the following data about the proposed team members: members name, firm name, office location, proposed team assignment, % of time to be spent on this team, education level/discipline (example: Bachelor of Science – Mechanical Engineering), State(s) of professional registration, # of years of professional experience, and # of years with the firm. Also, for project managers and design/discipline team leaders, identify the number of teams (design, consultants, and joint venture partners) they have managed over the past five years.
Evaluation Factor 3 – Schedule and Capacity
Describe your firm’s and subcontractor’s capacity to accomplish the work in the required time and the ability to adhere to schedules. The narrative shall clearly convey the offeror’s understanding of this requirement as well as the resources committed to ensure timely and successful completion of the work. Additionally, please provide the following information:
a. List current projects being completed in the firm’s office
b. Describe experience in successfully delivering projects per performance schedule, providing timely construction support, if applicable, and successfully completing multiple projects with similar delivery dates.
c. The AE shall provide details of its current work (public/private) including the duration of the contracts, and how it anticipates on taking on additional work required by CFM.
Evaluation Factor 4 – Past Performance:
Describe past performance on contracts/task orders with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules. Your firm’s past performance narrative shall include, but not limited to, performance for projects submitted under
Evaluation Factor 1. Submit client references (name, number, and email) for those projects listed in
Evaluation Factor 1. The Government reserves the right to also evaluate past performance on previously awarded federal contracts/task orders via CPARS or with any other information available. Additionally, the Government may review the firm’s previous ability to meet the 50% SDVOSB performance requirement on other contracts/task orders. Note: We recommend you contact every reference listed and let them know we may contact them for information. Their timely replies would be appreciated.
Factor 5 – Location in Geographical Area of the Project
Clearly indicate the location(s) where the majority of the off‐site work will take place. Additionally, describe the team’s knowledge of the project’s geographical area. The evaluation board will evaluate the location of the proposed team against the geographical area of the project and knowledge of geological features, climatic conditions, or local construction methods that are unusual or unique that may be applicable to this project.
Factor 6 – Use of SDVOSBs, VOSBs, and Other Types of Small Businesses
Describe the extent in which your firm identifies and commits to using other Service‐Disabled Veteran‐
Owned Small Businesses, Veteran‐owned small businesses, and other types of small businesses as subcontractors.
PLEASE NOTE: The AE Evaluation Board must be provided with complete and accurate information for
ALL evaluation factors above in order to rank interested and eligible firms.
SELECTION PROCESS: in accordance with FAR 36.6 and VAAM M836.6, qualifications will be evaluated using the evaluation factors identified in this Request for SF330s (qualifications). After qualifications have been evaluated, discussions/interviews will be held with three of the most highly qualified firms.
After discussions conclude, the evaluation board will evaluate the qualifications of the firms using the same evaluation factors, taking into consideration both the submission and subsequent discussions, to select the most preferred firm from the firms considered to be the most highly qualified. Negotiations will then begin with the most preferred firm.
ELIGIBILITY REQUIREMENTS
Firms must also be registered in Veteran Small Business Certification (VetCert) database (at https://veterans.certify.sba.gov/) at the time of SF330 submission, the time of award, and at the time of contract issuance. Additionally, firms must supply type(s) and amount of insurance along with insurance carrier(s) after contract award.
To be eligible for contract award, a firm must be registered in the System for Award Management (SAM) database. Representations and Certifications must be completed electronically via this site as well.

All firms will be required to comply with Veteran Affairs Acquisition Regulation 852.219‐73, VA Notice of
Total Set‐Aside for Certified Service‐Disabled Veteran‐Owned Small Businesses.

SUBMISSION REQUIREMENTS: Responses shall reference the RFI number CFM‐24‐000010 (Request for SF330s – Qualifications) in the subject line of the email submission. Interested firms having the capability to perform this work must submit the completed SF330 form and any other supporting documentation via email to jason.hardy@va.gov. Include the firm’s SAM UEI and DUNS number in
SF330, Part II, Block 4. Interested firms should submit their qualifications as follows:

a. Size limitation is 20MB;
The submissions are due on December 1, 2023 at 3:00 PM Local Time (CST). Submissions received after this date and time will not be considered and will be handled in accordance with FAR 52.215‐1.
Acceptable electronic formats (software) for submission of Qualifications:
(a) Files readable using the current Adobe Portable Document Format (PDF);
(b) Files in Adobe* PDF Files: When scanning documents scanner resolution should be set to
200 dots per inch, or greater;
(a) Standard letter size 8‐1/2 by 11‐inch paper, limited to a maximum of thirty‐five (35), single‐sided, single‐spaced, and numbered pages as described above. If more than the maximum pages as described in each section above are submitted, the extra pages may not be evaluated. The font for text shall be Times New Roman 12‐point or larger. The submission shall be clear, concise and short. Please note: the 35 page limit does not include a cover page, past performance supporting documents such as CPARS records, or an organizational chart.
