1 INTRODUCTION
The information enclosed has been compiled from the latest standards for material specifications used by consulting engineers and local agencies. It is our intention that these specifications be used as a guideline for engineers, developers, architects, and contractors to insure uniformity of Public Works and Utilities (PW) construction.
1.1 SITE IMPROVEMENT PERMITS (SIP)
A SIP shall be required by any applicant proposing site work including but not limited to earthwork;
regrading; utility installation or relocation; addition or reduction in impervious area; improving a parcel of vacant land; renovating or rehabilitating existing development; or proposing new development. SIPs cover the following primary categories of work:
Construction of new roads, grading and the associated utilities necessary for approved subdivisions and parcel maps
All commercial site construction (grading, drainage, parking lots, signage, and utilities) including adjoining roadwork and driveways prior to issuance of a building permit
Clearing and grubbing of lots greater than 0.5 acres
Excavating or filling more than 100 cubic yards of earth
Parking lots (both permanent and temporary)
Incident grading, residential structures and miscellaneous residential lot improvements that fall within FEMA Flood Zones A, AE, V or VE as well as activities that exceed the limitations contained in the Land Use Regulations Section 2.2.3.3 and Section 3.4.3.
Predesign conferences with PW staff are available upon request. The permit authorizes on-site construction over private property prior to the issuance of a building permit and may include improvements within a public roadway or right-of-way associated with the on-site construction such as site grading, installation of public utilities, drainage improvements, road improvements including curbs, sidewalks, ground/pavement markings, and related features, and on-site signage.
1.1.1 SUBMITTAL PROCESS AND REQUIREMENTS
Applications for SIPs are available on the City of Bradenton website, www.cityofbradenton.com. Each application must include the supporting documentation necessary to review your project. Questions related to submittal requirements for the project should be directed to the Engineering Section of the
Department of Public Works.
The Engineer of Record (EOR) shall provide a submittal package to the City of Bradenton Department of
Planning and Community Development (City Hall) for processing. Upon receipt of an application, Engineering Section staff will conduct a cursory review of the package. If any item is missing or does not contain adequate detail to review the project; staff will notify you of the deficiency. Incomplete applications will not be processed. Upon determination that an application is complete and sufficient for review, a technical review of the project will be conducted. PW Engineering shall review the plans for conformance to City specifications and standard engineering practices. Comments shall be returned to the
EOR. Subsequent submittals may be sent directly to PW. When the revised plans are returned and the http://www.cityofbradenton.com/

Engineering Section is satisfied that the work complies with City standards, staff will send an email notifying the EOR the permit is approved and signed construction drawings are ready for pick up.
The first submittal package shall contain two sets of full size construction plans signed and seal by a licensed
Professional Engineer registered in the State of Florida and a disc containing all supporting documentation and a pdf of the construction plans. All subsequent submittals shall contain four sets of full size construction plans signed and sealed by a licensed Professional Engineer registered in the State of Florida, a response letter to the comments returned to the EOR by Public Works, hard copies of any additional supporting documentation requested by the City, and a disc containing all supporting documentation and a pdf of the construction plans.
The Construction Plans shall include the following:
All City Council approved stipulations or conditions shall be shown on the cover sheet of the construction plans
Infrastructure Ownership and Maintenance shall be shown on the cover sheet of the construction plans for all utilities (water, sanitary sewer, storm water, roadways, lift stations, lighting, ect) including solid waste service
A table on the cover shall identify the roadways as public or private as well as evacuation, arterial, collector or neighborhood streets
Landscape and Irrigation Plans
Boundary Survey
Topographic Survey
Right of Way and Easement Lines clearly identified and marked as existing or proposed
FEMA Floodplain
Finished Floor Elevations

If the EOR finds that revisions are necessary after an SIP has been issued or closed out, additional permitting fees may apply.
1.1.2 SHOP DRAWINGS
The following specifications indicate the materials and installation of systems approved by the City of
Bradenton. All infrastructure being proposed within the city limits are required to meet these standards.
Shop drawing approval is part of the SIP process. When submitting this information provide PW with electronic copies of the shop drawings if less than 50 pages (all disciplines) or hard copies and a disc with a pdf file(s) if greater than 50 pages. All shop drawings submitted to PW Engineering must be previously reviewed and stamped by the EOR for the project. Shop drawings include but are not limited to water, sewer, drainage, roadway, lift station and electrical products.
1.1.3 PERMIT STANDARD OF CARE
General conditions included in every SIP are as follows:
The permittee shall commence work on the permitted installation within 60 days of permit issuance and all work shall be completed within the number of days indicated on the SIP. If work begins more than 60 days after the issuance of this permit or if the work is suspended or abandoned at any time after work has commenced for a period of 180 days (i.e. no City inspections have been requested or occurred), the SIP will expire and become void unless the permittee requests an extension in writing prior to the permit becoming void. If the permit becomes void, before any work can recommence, a new permit application must be filed.
Prior to any groundbreaking or construction activities, the EOR and Contractor must:
Schedule and participate in a preconstruction meeting with City PW and Building
Department staff
Submit all final permits, letters of exemption, from all permitting agencies (SWFWMD, FDEP, FDOT, County).
The EOR and Contractor must provide any shop drawings for approval prior to installation for any utilities that will be maintained by PW.
Sod all areas within right of way and easements disturbed during construction. Seeding is not permissible.
Trees shall be planted a minimum separation of 10 feet from any utility owned and operated by the City. Trees cannot be planted within utility and drainage easements and right of ways. Tree root barriers are required if a variance is approved for planting within the ROW.
Fencing is not permitted in flowage or drainage easements
The EOR shall be responsible for all permitting, testing, inspections and certification of site improvements.
All testing and inspections of City utilities shall conform to the latest requirements (AWWA, FDOT, FDEP, SWFWMD, Public Works Utility Specifications, etc.)
The EOR shall provide the Public Works and Utilities department with the following items prior to a certificate of occupancy being issued:

1.1.4 PERFORMANCE BONDS FOR PUBLIC UTILITY WORK
Before any construction shall take place on facilities that will be dedicated to the City, an approved
Engineer’s Estimate for utility, stormwater, and road improvements must be submitted to and approved by
PW.
Performance bonds for the construction of these facilities (water distribution systems, sanitary collection systems, force mains, lift stations, pump stations, roadways, drainage systems, reclaimed water systems, street lighting, and other related public facilities) shall be based on the approved Engineer’s Estimate of
Probable Cost and are required for all Planned Development Projects as well as any other significant improvements being dedicated to the City being made by a developer/contractor as deemed necessary by the Public Works Director. A performance bond must be on file with the City of Bradenton prior to the commencement of construction.
A project is closed out when:
The final inspection is complete
Utilities and other public facilities are accepted by the City
Record drawings are accepted by the City
Project Close Out letter has been issued by Public Works
When a project is closed out and all of the above criteria are met, the EOR shall send a letter to Public
Works requesting the bond be released. The letter must include the name and address of where to send the bond.
1.1.5 TEMPORARY HYDRANT METER CONSTRUCTION ACCOUNT
The Contractor shall apply to PW using the Temporary Water Service form which will be processed at the Utilities Customer Service office to obtain a temporary hydrant meter construction account.
A Temporary Water Supply Hydrant Meter Assembly will be issued for the account. A deposit will be required for the equipment associated with the assembly.
The meter and appropriate backflow preventer shall be installed, monitored, read and finally removed or re-located by employees of PW only.
The Assembly shall be installed by PW and shall remain in service for no longer than 180 days.
The assembly will be automatically removed by PW after the time limit has expired unless authorization has been obtained from PW for a time extension.
The Assembly’s meter will be read and billed accordingly on a monthly basis, for as long as the
Assembly remains in place.
Individual service meters will not be set in subdivisions until such time that framing construction is complete and approved Backflow Preventers are installed and certified.
When the Contractor returns the meter to Public Works and the meter inspected for damage, Public Works will notify Utility Billing and any applicable deposit will be returned to the
Contractor.

1.2 RIGHT-OF-WAY (ROW) USE PERMITS
If any infrastructure work is proposed within the dedicated ROW that is not part of a SIP, the contractor shall be responsible for obtaining a ROW Use Permit.
1. Permittees are required to obtain an individual ROW Use permit for each street where work will be performed; blanket ROW Use permit applications are no longer acceptable.
2. The City PW shall be notified a minimum of 48 hours prior to commencement of construction operations. Commencement must begin within 60 days of issue date.
3. All excavators shall call Sunshine 811 (1-800-432-4770) before they dig: whether it’s digging with a shovel, digging with a trencher or using equipment that penetrates the ground. A minimum of two full business days before digging, call Sunshine 811 to request a locate ticket so that
Sunshine 811 members that own or operate underground facilities can locate and mark their underground facilities near the dig site.
4. The Permittee is responsible for protection of all existing utilities and for any damage thereto.
Should any damage to existing utilities occur during the permitted installation, the damaged utility owner must be immediately notified.
5. For work involving directional drilling/boring of utilities, the work must be completed in compliance with the Appendix E Directional Bore Guidelines.
If work or signage is being proposed on or adjacent to a State or County roadway the contractor is responsible for obtaining ROW Use permits from the other public entities and submitting the permit to
City of Bradenton Public Works Engineering prior to commencement of work within the ROW.
1.2.1 MAINTENANCE OF TRAFFIC
Maintenance of Traffic (MOT) plans associated with sidewalks, roadway lanes, or full road closures are required with the ROW Use Permit application. The MOT plan shall be completed by an individual who has completed and received certification in the FDOT approved MOT intermediate or higher level course.
1.2.2 CONSTRUCTION AND RESTORATION
All construction and restoration must meet City of Bradenton standards. If the work requires disruption of hardened surfaces (roadways, sidewalk or driveway, ect.), restoration will be as follows:
A. Roadways
1. At their own expense the Contractor shall repair all damage to pavement as a result of work performed in a manner satisfactory to the City. Pavement shall be repaired to match the original surface material thickness and original grade. However, the asphalt concrete thickness shall not be less than 2 inches. The repair shall include preparing the subgrade, placing and compacting the applicable base, priming the limerock base, and placing and maintaining the surface treatment as specified.
2. The length of all repairs shall extend 25 feet in either direction from the pavement cut and the width of all repairs shall extend at least to the center line of the roadway or from curb-to-curb if determined by the City. The edge of the pavement to be left in place shall be cut to a true edge with a saw so as to provide a clean edge to abut the repair. The line of the repair shall
1-6 be reasonably uniform with no unnecessary irregularities. Beyond the utility repair area, the pavement shall be milled to a depth of 1 ½ inches and resurfaced with Type III asphalt.
3. General pavement joints within asphalt or concrete driveways and roadways shall be mechanically sawed butt joints. The edges of asphalt pavement shall be trimmed to straight lines which a roller can follow or form.
B. Sidewalks
At their own expense the Contractor shall replace any sidewalk panels in their entirety (no partial panel replacement), that were removed or damaged as part of the construction, to meet existing grades (no trip hazards) with appropriate expansion joints between existing panels. If the subject panels are located at a roadway intersection, the Contractor shall replace with the appropriate ADA accommodations even if the existing panels are not compliant.
C. Driveways
The Contractor shall replace driveway aprons in their entirety as per City standards.
1.2.3 SIP AND ROW USE PERMITS
If the construction has an approved SIP, no separate ROW Use permit shall be required. The Contractor shall be responsible for adhering to the guidelines listed above as well as any listed within the ROW Use permit application. Although the ROW Use Permit is included in the SIP approval the additional requirement of MOT plans associated with work in the ROW must be submitted by the Contractor to
Public Works for review and approval prior to the Contractor implementing the plan. Any proposed full road closures will require the approval of the Director of Public Works and Utilities and must be submitted 72 hours in advance of the proposed closure.
1.3 TREE REMOVAL PERMIT AND TREE REPLACEMENT REQUIREMENTS
The SIP construction plans include both the civil engineering design site improvement drawings and the landscape and irrigation drawings therefore an additional tree removal permit is not necessary. Per the
City Land Use Regulations, tree removal may have a financial assessment incurred as part of the construction requirements. Tree replacement or relocation shall be in accordance with the City Land Use
Regulations for species, size, and number of replacements required for each tree removal.
1.4 RECORD DRAWING AND INSPECTION PROCESS
The information below outlines the record drawing and inspection process to complete the requirements of the SIP and close out a project with Public Works.
The SIP includes the following language “The (EOR) shall provide the Public Works and Utilities department with the following items prior to a certificate of occupancy being issued: a) Verification of a final inspection of site improvements by Public Works and Utilities; b) Record drawings - two hard copies and one electronic copy in AutoCAD & PDF on a disc prepared in accordance with Public Works and
Utilities Standards.”
The closeout process is twofold; the first step in the process involves the Record Drawing submittal.
Depending on the scope of the project several elements may be described by engineers as record drawings:

Drawings submitted to FDEP for certification of water or sanitary systems are not considered Record
Drawings by the City of Bradenton.
FDEP Certification Requirements
1. If the project includes the construction of a distribution and/or collection system for water and/or sanitary sewer, red lines or record drawings must be submitted to FDEP for certification.
a. Typically the engineer submits the forms to be signed by the City and a copy of the drawings that will be submitted to FDEP.
b. FDEP replies back to the EOR and the City with a form letter that the project has been cleared.
c. The plan submittals to obtain FDEP clearance do not meet the requirements for final record drawings to close a project with the City.
2. The City must have FDEP clearance documents prior to final inspection and to provide PW approval for Certificate of Occupancy.
City of Bradenton Requirements
1. A Record Drawing submittal for review includes two hard copies of the complete plan set and a disc with a pdf of the plan set as well as the AutoCAD files.
2. The hard copies must be:
a. Complete sets of the construction drawings (not a partial set of record information on individual sheets)
b. Signed and Sealed or Stamped by the surveyor
c. Signed and Sealed by the EOR
3. When Public Works receives the record drawing submittal, City staff will review the electronic and hard copy versions and typically provide comments on both. After the record drawing review is complete, the EOR may schedule the preliminary inspection walk through discussed in the next section.
4. Comments on the electronic submittal must be addressed within 30 days. Comments on the hard copy submittal must be addressed prior to scheduling the final inspection walk through discussed in the next section.
The second step in the process is the inspection walk through. Typically the inspection requires two walk throughs; one preliminary and one final.
1. Once the record drawings are submitted, the Contractor shall request a preliminary walk through. The walk through should include the contractors representative, the engineer of record, a Public Works engineer, a Public Works construction inspector, and a Planning & Zoning department representative.
2. In addition to the Record Drawing submittal, the EOR must submit a letter certifying the project was constructed to be in substantial compliance with the approved design drawings.
3. City staff will have the hard copy record drawings for the preliminary walk through and shall work with the Contractor to develop a project deficiency list. The project deficiency list will include both public works and planning & zoning items.

4. When the project deficiency list items have been completed and record drawings resubmitted, the contractor shall request a final inspection. The Public Works construction inspector shall follow up on the project deficiency list items prior to the final inspection request.
5. When all items from the record drawings and project deficiency list have been adequately addressed, Public Works will send a letter to the engineer indicating the City has accepted and closed the project. Note, when the project has been accepted and closed the owner may request return of the performance bond if applicable.
For phased development, the requirements above are applicable to each phase. In the record drawing submittal, information from any adjacencies on the previously completed phases must be included in the current record drawing submittal. While the entire site will not be included in the inspection, adjacencies to ongoing construction and public safety shall be included. If the project deficiency list items in the area that will be publicly accessed are adequately addressed, Public Works and Planning &
Zoning will not have any objection to a Certificate of Occupancy being issued for a specific building or phase of construction.
Before the project permit is closed out by the Engineering Department, a licensed Florida Engineer must submit the following language on the Record Drawings provided to the City: “The potable water, sanitary collection and stormwater systems were constructed in substantial conformance with the approved drawings. Any work in the right of way was completed per applicable City standards.” All required right-of-way and/or easements over water, sanitary sewer and stormwater systems shall be recorded with the City prior to permit close-out or Certificate of Occupancy. A Project Close Out Letter will be issued when the Record Drawing criteria have been met. Any warranties associated with utilities and facilities dedicated to the City will start on the date the letter is issued.

2 ROADWAYS, CURBING, SIDEWALKS, AND DRAINAGE
2.1 ROADWAYS
Unless otherwise specified herein, all streets, curbing, sidewalks, drainage, marking and signing shall be in accordance with FDOT Standard Specifications for Road and Bridge Construction, latest edition, as revised, and City Standard Drawings contained herein. FDOT Specifications are incorporated herein by reference as though printed in their entirety. Where City Specifications differ from the FDOT documentation, City
Specifications shall take precedence. All new road construction, both public and private, shall be built to minimum City standards.
2.1.1 SUBGRADE
The sub-grade construction shall conform to the elevation and cross-sections on the drawings after it has been compacted to 98% of the maximum density as determined by AASHTO 180 (ASTM D1557) with a minimum depth of 12-inches. Material not compacted thoroughly and to the satisfaction of the PW shall be replaced with suitable material and re-compacted to grade.
All roadwork shall consist of constructing a stabilized roadbed having a minimum LBR of 40. The required
