ADDENDUM 52.212-1 ATTACHMENTS
Please complete the attached document for the projects/programs you submit as past performance citations in reference to the solicitation. Delete any extraneous pages or addendums not applicable to the specific submission, i.e., for your Past Performance Information Form (PPIF) that you submit to the Government with your response to the RFQ, copy and complete the included Attachment 1 PPIF for up to 3 contracts, then delete the remaining unused pages of Attachment 2 and Attachment 3. For the Past Performance Questionnaires, delete the Attachment 1 pages, fill in the preliminary contract data for your customer, then deliver with the appropriate Attachment 3 Cover Letter to your customer (but do not submit to the Government). DO NOT edit or change the form questions or format.

ATTACHMENT 3 (52.212-1 Addendum) NOTE TO VENDORS: Italicized text indicates sample/instructional guidance to assist with filling this out prior to sending to the POCs. Cover letters should be issued on the vendor’s letterhead and dated.

SUBJECT: Present/Past Performance Questionnaire for the XXXX Program Solicitation Concerning Contract Numbers(s) (CONTRACT AND ORDER NUMBER (IF APPLICABLE) TO BE EVALUATED). The effort was performed by (SPECIFIC ENTITY THAT PERFORMED THE WORK) under (CAGE CODE).
(a) We are currently responding to 76 Commodities Maintenance Group, Oklahoma City Air Logistics Complex, Tinker Air Force Base, Oklahoma, Request for Quotation (RFQ) FA8125-21-Q-0002 for the Overhead Preventative Maintenance / Remedial Maintenance requirement. This acquisition is being conducted as a FAR Part 12 & 13 acquisition utilizing past performance and price as evaluation factors. The RFQ specifically requires that we, as an interested vendor, send the attached questionnaire to several points of contact (POC) on efforts that we have performed. As such, please take a few moments of your time to fill out the attached questionnaire (Parts B – E).
(b) DO NOT RETURN THE COMPLETED QUESTIONNAIRE TO US. The information contained in the completed Past Performance Questionnaire is considered sensitive and cannot be released to us, the interested vendor. The completed Past Performance Questionnaire should be sent directly to the Government not later than (DATE TO BE INSERTED BY VENDOR). The questionnaires should be emailed to: CARRIE.BROWN.8@US.AF.MIL.
(c) If you have any questions regarding this FAR 12 & 13 competitive acquisition, contact the Contracting Officer, Carrie Brown.
