EOS 3D Printer Service Agreement i

DESCRIPTION OF SERVICES

809 Maintenance Support Squadron (809 MXSS)/MXDTAD Reverse Engineering, Advanced Manufacturing, Prototyping, Innovation and Design (RAPIDS) has a requirement to purchase the Performance Assurance service agreement for Electro Optical Systems (EOS) 3D printers model numbers P 450 and M 290, located at Hill Air Force Base (HAFB), UT.

BASIC SERVICES

The contractor shall provide the service agreement specified below.

The Service Agreement shall include annually:

Technical support throughout the Period of Performance (PoP) Software updates throughout the PoP Two (2) Preventative Maintenance (PM) visits per year Reactive Service Support (parts and labor) Period of Performance The service agreement shall be for one (1) year plus four (4) one (1) year options for five (5) years total.

Location All maintenance shall be performed at the 3D printer’s location at Hill AFB, UT. All needed repair parts shall be shipped to Hill AFB, UT with no charge to the Government.

WARRANTY

The contractor shall provide a standard commercial warranty.

Government Point of Contact The Government Point of Contact (POC) shall be:

809 MXSS/MXDTAD RAPID

ADDITIONAL REQUIREMENTS

Government provided support The Government will provide necessary working space, including heat, light, ventilation, and electrical powered outlets for the use of contractors’ maintenance personnel. The Government will provide access to the necessary facilities where the equipment resides. The place of performance shall be Hill AFB, UT.

The contractor shall be responsible for safeguarding all Government property provided for contractor use.

Base Access Contractor shall schedule all visits and coordinate Base entry by contacting 809 MXSS/MXDTAD RAPID. The contractor shall be escorted at all times.

Removal of Contractor Personnel The contractor understands that any contractor employee may be summarily and immediately removed from or denied access to any Government property, facility, and installation at the sole discretion of competent Government authority if that authority determines such action is necessary to protect or preserve Government property, safety, and security, or to maintain good order and discipline.

Operations Security (OPSEC) No photography, or videotaping, is allowed on the installation without approval from the owning organizations Group Director/Commander on an approved photography letter from Public Affairs. This includes the use of personal cell phones with picture and video capability.

GENERAL INFORMATION

Hours of Operation Normal Hours of Operation: The contractor shall perform the services required under this contract during the following hours: Monday through Friday 0600-1630 – EXCEPT FEDERAL HOLIDAYS and other base closures.

Federal Holidays: Unless otherwise required by the Government, the contractor shall not have access to the Government facilities on the following federal holidays: New Year’s Day, Martin Luther King’s Birthday, Washington’s Birthday, Memorial Day, Independence Day, Juneteenth, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If a holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday. Any other circumstances that may prompt base closure, such as Days of National Mourning, may also restrict opportunity to perform the exchanges.

ENVIRONMENTAL CONTROLS

Hazardous Work Environment Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect.
